The OAC’s Your Weight Matters National Convention is a three-day educational event designed to provide evidence-based strategies for individuals to eff ectively manage their weight and health. The entire weekend is dedicated to spreading the important message that “Weight and Health Go Hand-in-Hand” and giving attendees eff ective tools to take charge of their health. Last year, the OAC hosted its Inaugural Your Weight Matters National Convention where

we welcomed more than 275 individuals from 34 states across the country to come together for a weekend of top-notch education and often sought-after sense of community and support. The OAC is still

feeling the success of last year’s Convention, and the buzz continues to spread into 2013!

The OAC is a National non-profi t organization dedicated to helping individuals aff ected by excess weight and obe-sity through education, advocacy and support. We were formed in 2005 after a legislator pointed out the tremen-dous need to hear from those aff ected by obesity - because the voice of the individual aff ected was rarely heard.

With this obvious gap, the OAC was created as a membership organization and today, we focus our eff orts on providing unbiased and comprehensive education on obesity and weight-related issues, improving access to obesity treatments and leading the charge in fi ghting the unfair weight bias that millions of Americans face each and every day. Attendee Neck Wallets - $2,500

Each registered Convention attendee receives a name badge that must be visible to enter the educational sessions and Exhibit Hall. The OAC provides each registered attendee a neck wallet to place their name badge in for easy access to the sessions and exhibit hall. Neck wallets will feature an exclusive sponsor logo, along with the OAC’s logo and Convention logo. Sponsor name/logo may also be included on the lanyard that holds the neck wallet. This is a great souvenir item that attendees will take back home with them to use at other events/meetings.

OAC Member Mingle - $4,500

On Thursday, August 15, OAC Members and Convention attendees are invited to the OAC Member Mingle, an event intended to give attendees a place to come before the Convention offi cially begins to meet one another and catch-up with old friends. This social event will include an open bar, light refreshments and entertainment. Exclusive sponsor receives prominent name and logo placement on event signage, table tent cards (placed on all tables) and event postcards.

Attendee Canvas Tote Bags - $5,000

Convention canvas tote bags are included in all event registra-tions and are given to each attendee to use during the meet-ing to hold their Convention Program Book (agenda informa-tion) and all other materials and giveaways picked-up in the Exhibit Hall and at the meeting. This is an exclusive sponsor-ship and the sponsor logo will appear prominently on the canvas tote bag along with the OAC’s logo and Convention logo. This is a great item that is often re-used by Conven-tion attendees at other meetings.

National AdvocacyTraining Session -$40,000

The day before the edu-cational sessions begin (Thursday, August 15), the OAC will host its second National Advocacy Train-ing Session from 12:00 pm - 5:00 pm. This session is open to the fi rst 100 interested OAC members with a desire to become formally trained advocates to speak on behalf of the OAC to their elected offi cials and on Capitol Hill. Last year, this training session was held during our Inaugural Conven-tion and received nothing but favorable and rave reviews from the attendees. Those who typically attend are the most passionate and engaged OAC members, many of which have a tremendous following online through blogs and other events. The exposure received in this session exceeds beyond the core crowd. The session is free to pre-registered Convention attendees and includes the expert educational training, along with lunch and beverages. The sponsor of this session will receive prominent name recognition on session signage, table tent cards with sponsor logo and recognition in the offi cial advocacy training presentation. This is an exclusive sponsorship.

Morning Breakfast Sponsorship - $2,500 (2 available)Convention attendees that register for a Full Convention Registration will enjoy breakfast each morning (Friday and Saturday morning). Two sponsorships are available for either morning (or sponsor can opt to sponsor both). Lunch with the Experts - $3,500 (2 available) On each day of the educational sessions

(Friday and Saturday), the OAC will feature a Lunch with the Experts session where attendees will have the opportunity to sit one-on-one with the an expert to learn more about a specifi c topic of their choice.

This was the most popular event during last

year’s Inaugural Convention. There are 15 diff erent lunch table topics each day, with spots for eight attendees at each table. Attendees must pre-register (additional fee) for this event and will also receive a boxed lunch. Sponsors can choose to sponsor either Friday or Saturday (or both) and will receive exclusive name recognition on Lunch with the Experts signage, logo placement on table tent cards (placed on each of the 15 tables) and recognition on event tickets. Please see the Convention agenda to view the topics for each day.

Welcoming Ceremonies Event Sponsor - $5,000

On Friday evening, the OAC will kick-off the Convention and social events by hosting the Welcoming Ceremonies themed dinner party. This is a fun event that will invite all Convention attendees to join for a buff et dinner, dancing and costume contest. This popular event engages all Convention attendees and is an opportunity to get

additional exposure to the crowd. This will be a fun night to let loose and kick-off the Convention! Attendees also bring their family members and travel companions along with them to this event. Those that register for a Full Convention Registra-tion automatically receive one ticket to this social event and individual tickets are also available for purchase.

The exclusive sponsor of this event will receive prominent name and logo placement on Welcoming Ceremonies signage, logo inclusion on table tent cards placed on each dinner table (approximately 20 - 30 tables), and recogni-tion by the OAC’s Master of Ceremonies in event welcome speech.

OAC 2nd Annual Awards Dinner Sponsor - $7,500

On Saturday evening, the OAC will host its 2nd Annual Awards Dinner where key OAC volunteers and mem-bers will be honored for their contributions to the OAC and those aff ected by obesity. This special night will culminate the two full days of educational sessions. It will be a time to come together to refl ect on the overall messages sent from the Convention.

This is a semi-formal, plated dinner event for Convention attendees and family members/travel companions. Those that register for a Full Convention Registration automatically receive one ticket to this social event and additional tickets are also available for purchase. The exclusive sponsor of this event will receive prominent name and logo placement on Awards Dinner signage, logo inclusion on table tent cards placed on each dinner table (approximately 20 - 30 tables) and recognition by the OAC’s Chairman and President/CEO in the speech and awards presentation.

Free Wi-Fi Weekend Sponsor - $7,500

By far the most requested perk at the Inaugural Convention was complimentary Wi-Fi in the meeting room. Convention attendees like to stay connected and talk about their Conven-tion experience while at the meeting. This sponsorship would provide free Wi-Fi access to ALL meeting attendees in the main session ballroom, Exhibit Hall, breakout sessions and foy-er for all three days of the Convention. The exclusive sponsor of Wi-Fi will receive prominent name and logo placement in the Convention Program Book, recognition on event signage throughout foyer with Wi-Fi access information, recognition on social media postcard distributed to each attendee, acknowledgement of support in announcement slides in between sessions and ability to customize the access code with your company name.

Flyer/Product Tote Bag Inserts - $500-$1,000/insertion

Companies and organizations are invited to insert items into the offi cial meeting tote bags received by all Convention at-tendees. The OAC’s has selected Brede Exposition Services as its exhibit services provider. The exhibit fl oorplan is included with this prospectus. Contact information and ordering instructions through Brede will be provided to exhibitors once the application is processed.

Space Assignment:

Space is assigned fi rst to previous Convention Exhibitors based on a Priority Points system. Points are awarded to exhib-iting companies according to the previous booth price paid, number of years participating, and when applications are received. For new exhibitors, space is available on a fi rst-come, fi rst-served basis. National sponsors receive preference in booth selection, with Platinum Sponsors guaranteed premium booth location. Exhibitors are asked to select their top three booth location choices. We will make every eff ort to accom-modate your top selections, space permitting. It is recom-mended to submit your exhibitor contract early to ensure the best booth selection.

Exhibitor Eligibility:

Exhibiting at the OAC’s Your Weight Matters National Conven-tion is by invitation-only. The OAC allows companies/organiza-tions to request an invitation to exhibit, but the OAC reserves the right to reject or cancel any exhibit at any time. Exhibitors must provide products or services that support individuals aff ected by excess weight and obesity. Products and services must be educationally focused. Selling of any items or services beyond educational references or consumables is strictly forbidden. Exhibited products must be compliant with the regulations in the United States. Exhibitors for pharmaceutical products that are subject to the U.S. Food and Drug Admin-istration (FDA) oversight must comply with FDA regulations regarding advertising and promotion.

All exhibits must clearly and prominently identify the exhibi-tor by trademark or signature. Exhibits from individual prac-tices and or hospitals will not be accepted. These entities may choose to sponsor items such as tote bags, lanyards, meals, breaks, etc. Please see exhibitor guidelines section for full information on OAC’s policy with exhibiting.

Housing:

A room block at the Arizona Grand Resort & Spa has been reserved for all OAC Convention attendees and exhibitors. The OAC has secured a competitive rate of $105/night (single and double occupancy). The room block will fi ll quickly, so we strongly suggest making your arrangements as soon as possible The Convention room block closes July 22, or when the block sells out, whichever comes fi rst.

Exhibit Install and Dismantle:

Install: Thursday, August 15 11:00 am - 5:00 pm

All exhibits must be fully installed by Friday at 7:30 am.

Dismantle: Saturday, August 17 3:30 pm - 8:30 pm

Exhibit dismantle may not occur during exhibit hours. Exhibi-tors who are found in violation will not receive an invitation to exhibit at future OAC meetings.

Admission to General/Breakout Sessions

As part of your exhibit booth, the OAC allows exhibit person-nel to attend the general and breakout educational sessions. Meals are not provided to exhibitors. The OAC dinner func-tions are ticketed events and exhibitors will not have access to these events unless a separate ticket is purchased.

Admission to Social/Dinner Events:

Tickets to the OAC’s social/dinner events are NOT included with your booth registration fee. Exhibitors who want to attend the OAC’s dinner functions must purchase separate event tickets.

Exhibit Personnel:

Each 10 x 10 exhibit space is allotted two exhibitor badges. 10 x 20 spaces are allotted four exhibitor badges. Exhibitors must register their exhibit personnel in advance. Exhibitors must wear their badges at all times during the Convention to gain access to the hall. Additional badges may be purchased for $50/each.

Sharing Space:

Sharing or subletting of booth space is not permitted.

Terms of Payment:

Payment in full is required with the Application andContract for Exhibit space. If alternate payment schedule is needed, please contact the OAC to make arrangements. No requests for exhibit space will be accepted without payment, unless authorized by the OAC. Failure to submit full amount results in release of booth space reserved. All funds must be drawn on a U.S. bank.

Cancellation of Space:

Notifi cation of an exhibitor’s decision to cancel or reduce exhibit space must be submitted in writing. If cancellation or reduction of space occurs prior to June 1, 2013, a refund will be issued minus a $500 administrative fee. There will be no refund for cancellation or reduction of space after June 1, 2013, unless the exhibit hall is sold-out and the booth space is able to be re-sold.

Insurance and Liability:

It is the sole responsibility of the exhibitor for any damages, claims, losses, liabilities or expenses arising from any injury to any person or property that arises out of or is in any manner connected with the exhibitor’s participation in the OAC Conven-tion. Exhibitor acknowledges and agrees that it will obtain, at it own expense, any and all licenses or permits to comply with all local, state and federal laws, ordinances and regulations for any of its activities in connection with the OAC Convention.

Exhibitors should maintain general public liability insurance against claims for personal injury, death, or property damage incident to, arising out of, or in any way connected with their participation in the exhibition, in an amount of not less than one million dollars ($1,000,000) for personal injury, death, or property damage in any one occurrence. Such insurance should include coverage of the indemnifi cation obligations of exhibi-tors under the policies and procedures and should cover OAC as an additionally named insured.

Each exhibit company is responsible for obtaining, for its protec-tion and entirely at its expense, such property insurance for its exhibit and display materials as the exhibitor deems appropri-ate. Any policy providing such property insurance must contain an express waiver by the exhibitor’s insurance company of any right of subrogation as to any claims against OAC , its offi cers, directors, agents, members, or employees.

By executing the Application & Contract to Exhibit, Exhibitor agrees to indemnify, hold harmless and defend OAC, and its members, offi cers, directors, employees and agents from and against any and all liabilities, claims, losses, damages, causes of action, demands, costs and expenses of any kind whatsoever including, without limitation, court costs, interest and attorney’s fees, which OAC or its members, offi cers, directors, employees or agents may incur, suff er or be required to pay, incident to or arising out of, directly or indirectly, any intentional, reckless or negligent act or omission or breach of the terms, conditions or provisions of these Rules and Regulations by Exhibitor or its agents, servants or employees.

Exhibitor further agrees that it is solely responsible for the con-duct, actions and omissions of its employees, agents, servants and independent contractors, whether acting within or with-out the scope of their authority, and agrees to indemnify, hold harmless and defend OAC and its members, offi cers, directors, employees and agents from and against any and all liabilities, claims, losses, damages, causes of action, demands, costs and expenses of any kind whatsoever including, without limitation, court costs, interest and attorney’s fees, resulting directly or indirectly from such acts or omissions.

In no event shall OAC be liable or responsible for any loss or interruption of business, business opportunities, or any other type of direct or consequential damages alleged to be due from OAC. Exhibitor agrees and understands that the sole liability of OAC for any claims of Exhibitor shall be limited to a refund of the amount paid by Exhibitor under the Application & Contract to Exhibit. Nothing in the Application & Contract to Exhibit, Exhibitor’s Prospectus, or Rules and Regulations is intended or shall be interpreted as creating a partnership, joint venture or agency relationship between Exhibitor and OAC.

Show Cancellation Policy:

The parties agree that if the OAC Convention is cancelled in whole or in part, or its scheduled opening is cancelled or delayed, due to fi re, explosion, strike, freight embargo, act of God, act of public enemy, act of war, act of terrorism, civil dis-turbance, act of any government, de jute or de facto, or gov-ernmental declaration or regulation, epidemic or other event over which the OAC has no control, then the exhibitor contract may be immediately amended by the OAC and the exhibitor hereby waives any and all claims against the OAC for damages, additional reimbursement beyond exhibit space reservation fee, refunds or compensation.

Security:

Each exhibitor is responsible for safeguarding its goods, materi-als, equipment and exhibit at all times during the Convention. The Exhibit Hall will be locked/monitored during non-exhibiting hours, however, the OAC is not responsible for loss of or dam-age to any property.

Product Claims/Substantiation of Health Benefi t Statements/Guidelines:

1. Exhibitors are encouraged to provide and have available substantiation of all health benefi t statements or ingredient content statements to any attendee.

2. All testing to validate health benefi t statements or ingredient content state-ments should be performed by an independent, certifi ed laboratory with published Good Laboratory Practices.

Product Claims/Superlative Statements/Guidelines:

1. Superlative statements should only be made if they are true, and the exhibi-tor has substantiation of such statements.Examples: Superlative statements include (but not limited to): “best in the world,” “best product in the U.S.,” “the only,” “the most potent” and “the most eff ective.”

2. Superlative statements should not be misleading.

Product Claims/Disease or Health Benefi t Statement/Standards:

In accordance with the Dietary Supplement Health and Education Act of 1994 (DSHEA), and the U.S. Food, Drug and Cosmetic Act, dietary supplement, food and cosmetic products and their labeling must not claim to treat, prevent, mitigate, cure or reduce the risk of diseases, unless the complete FDA-approved unquali-fi ed or qualifi ed health claim is used on the labeling. The FDA considers that if a statement about a product or ingredient, claims to diagnose, mitigate, treat, cure, or prevent disease or if it has an eff ect on a specifi c disease, class of diseases or on the characteristic signs or symptoms of a disease, the product will be subject to regulation as a drug unless the claim is an authorized health claim for which the product qualifi es. Please remember that obesity is a disease under the defi nition of the law.

1. Product labeling including sales and marketing material must not express or imply claims to diagnose, treat, cure, prevent or mitigate any diseases, including (but not limited to), obesity, cancer, heart disease, AIDS, diabetes, Alzheimer’s disease, etc.

2. Substantial scientifi c data, to be determined at the discretion of an OAC representative, will be required to support benefi t statements pertaining to such diseases. The data if published must include journal name, date, volume and authors’ names.

Product Claims/“No” and “Free” Statements/Standards:

1. “No,” “low,” “lite” and “free” statements must comply with all applicable FDA and FTC regulations.

2. Such statements must not be deceptive and must fully disclose relevant facts. Examples: A statement of “no salt added” is deceptive if the product contains a high amount of sodium from hydrolyzed vegetable protein. “No oil” is deceptive when the product is naturally high in fat such as peanuts.

3. Products stating to be free of ingredients with a particular activity must disclose the presence of ingredients with similar activity. Example: Products that contain no caff eine but do contain guarana.

4. Such statements must be substantiated with documentation available at the request of a designated OAC representative.

Product Claims/Disallowed Statements/Standard:

1. The following products may not be exhibited: - Products that claim to “cure” or “prevent” obesity - Products containing unproven and misleading information - Products that do not meet current FDA standards (if applicable) - Products that compare themselves as the “best” or “better” obesity treatment (in comparison to others)

2. The OAC evaluates all promotional literature and/or labeling as a whole, including text, product names and images used. - Imagery and language used by exhibiting companies must comply with the OAC’s written weight bias policy/statement, located on the OAC Web site at www.obesityaction.org.

Labels and Labeling/FDA and USDA Compliance / Standard:

All labels and labeling must be truthful and not misleading and must comply with current FDA and USDA regulations.

Labels and Labeling:

1. Product labels and promotional literature must include the full name of the manufacturer or distributor and mailing address.

2. Literature that does not list a trade name must have the exhibitor’s name,

city, state and zip code and/or the phone number; or it must have the name and address or phone number of the party responsible for compilation and dissemination of the information. Published “third-party literature” must bear a full citation.

3. Product labels and promotional literature should include a phone number for the manufacturer or distributor.

Labels, Labeling and Literature/Quotes/Testimonials:

1. Quotes must not be misrepresented by being taken out of context.2. Endorsements or testimonials by consumers must represent what the

typical experience of customers would be, not the experience of just a few customers. Simply stating that “Not all consumers will get these results” or “your results may vary” is not enough.

3. Endorsement quotes must cite the speaker and source of the quote.

1. Photos and illustrations may not be deceptive or misleading.2. Photos and illustrations must comply with the OAC’s written weight bias

Images prohibited for use include: - Intentionally degrading/derogatory depictions of individuals aff ected by obesity - Individuals depicted as “headless” or intentional photos taken from behind - Exaggerated and unfair depictions that target individuals aff ected by obesity (over-indulgence, appearance of laziness, etc.)

3. Before/after and comparison photos must be used in the following manner: - Include a caption stating the time of the fi rst photo and the time of the second photo. - Exposure and print techniques must be identical for each photo.

4. Photos and illustrations should not contain additional misleading features or characteristics.

Advertising/Standard:

1. The OAC discourages the use of negative advertising. All comparison and negative advertising must comply with FTC regulations. FTC defi nes comparison advertising as that which “compares alternative brands on ob-jectively measurable attributes or price and identifi es the alternative brand by name, illustration or other distinctive information.”

1. Data cited should be made available on request to any attendee.2. Negative statements about companies or products should be thoroughly

documented and such documentation made available at the request of a designated OAC representative.

1. Guarantees must be easily understandable and must not be misleading and must include relevant terms and conditions.

2. Companies off ering guarantees must honor the guarantee.

1. The exhibitor is responsible for the accuracy of all information present in the booth that pertains to products or services off ered.

1. Information that is primarily educational and helps to better understand the product is encouraged.

2. Information should state product limitations.3. Information from an impartial third party is encouraged.4. Information about the cultural context of use or philosophical basis of

formulation is encouraged.5. Information should explain technical terms and cite references for any state-

ments made.

Trade Show Activities/Sale of Products/Standard:

1. Food, beverages and other products intended for immediate consumption must be off ered free of charge.

Trade Show Activities/Ethical Considerations/Guideline:

1. Exhibitors are requested to adhere to ethical considerations in areas includ-ing but not limited to the following:

• Weight Bias• Racism• Sexism• Sexually explicit materials• Profanity• Nudity

2. Exhibiting companies may not conduct consultations of any kind on the exhibit fl oor or in any meeting room on the grounds at the Arizona Grand. All customer leads and consultations must be conducted at a later date.

3. Exhibiting personnel are prohibited from off ering medical advice if not authorized or credentialed. Inquires of a medical nature must be handled by a qualifi ed medical professional.
