SHARON LYNNE WILSON CENTER fOR THE ARTS (SLW)Summer classes are held at the Sharon Lynne Wilson Center for the Arts in Brookfield. This beautiful arts and education facility is nestled just 20 minutes west of downtown Milwaukee in Mitchell Park at 19805 W. Capitol Drive, Brookfield, WI 53045.

First Stage Theater Academy is the nation's largest high impact theater training program for young people.

Dear Parents,We know that you and your loved ones have many choices as to how to spend your sum-mer days. Our students and their families know that the Theater Academy experience is like no other. The First Stage philosophy of "life skills through stage skills" provides students with a foundation in theater as well as a deep under-standing of human nature and how to relate to each other in a positive, supportive, productive way. All classes are designed to foster self-con-fidence, creative thinking, problem-solving skills, collaboration, trust and discipline. The creativity cultivated in theater classrooms is carried into every aspect of our lives, and we believe this makes us all more connected, more empathetic, and more prepared to embrace the joys and challenges the world will bring.

Thank you for believing in First Stage and our ability to transform lives through theater. We’ll see you this summer!

• Interns are returning Academy students who serve as a role model in the classroom. Students must be entering grades 8–12 in the fall of 2013 to be considered.

• An application can be sent to you or downloaded from our website. All interns will receive a $50 tuition credit per week of work that may be used beginning fall of 2013.

reGistration proCedure Registrations are accepted by mail, fax, or online at www.FirstStage.org. If you choose to register online, you will receive immediate confirmation that you have been placed in a class, whether it is your first, second or third choice. First Stage Theater Academy will continue to process paper registrations, but would strongly urge you to register online to ensure placement in your first choice class.

If you wish to register two or more students in the same group, you will be given the option to do so online, or you may mail the registrations together and note the student(s) with whom he or she wishes to enroll on the top of the form. First Stage Theater Academy will review these requests and make every effort to honor them, although we cannot guarantee placement in the same group.

There is no special consideration extended to registrations delivered in person. Please do not register online and mail or fax a second paper copy. This will duplicate your registration and may result in an accidental double charge.

Please note, any information omitted from the registration form will delay registration and your desired class might fill before your enrollment is completed.

tuition Tuition must be paid in full, unless signing up for automatic payment. A $75 deposit is due on date of registration, if signing up for the automatic payment method. The automatic payment will deduct a $75 deposit, and then on Friday, May 31, 2013, the remainder of the tuition will be charged. You may change the credit card payment method or submit full payment via cash or check before the May 31 deadline.

refund poliCYThere is a $25 administrative fee for all refunds. No refunds are given after May 31, 2013. Students withdrawing after May 31, 2013 will receive tuition credit toward another class. Credits offered will be applicable for up to one year. No refunds or tuition credits are given after the first day of class. The deposit will be forfeited if a student cancels after the session has begun.

Class CanCellationsFirst Stage Theater Academy reserves the right to cancel any course if enrollment is insufficient. Any tuition paid will be refunded in full.

• Switching Classes: If families find that they have a scheduling conflict or other emergency circumstance and need to switch their course selection, they may contact the Academy office to request this change. Students will not be moved from their assigned group after the first day of class. Please be aware that switching class is contingent upon space availability in the desired class.

• Attendance/Make-up Policy: If your student is unable to attend a day(s) of class, we do not offer make-up classes. Please contact the Academy office if you know in advance that your student will be absent. If First Stage Theater Academy needs to cancel one or more days of a class due to circumstances beyond the Academy’s control (e.g., severe weather, power outage, etc.), there will be no refunds or make-up classes.

• Sending Students Home: Please use good judgment about bringing your student to the Academy. If your student is sick or contagious, please keep them home. Parents of students who become ill during the day will be promptly notified and are expected to arrange to pick up their student immediately.

First Stage Theater Academy also reserves the right to send students home due to inappropriate or disrespectful behavior.

Class siZe and wait listsWe do maintain enrollment limits to ensure the best possible Academy experience for each student. Wait lists are often established for classes that have reached the enrollment limit. Students on wait lists are commonly called for enrollment, sometimes as late as the day before classes start. Even if the class is filled, we encourage you to be added to our wait list. As we strive to make space for as many students as possible, we are unable to add students to a wait list if they are already enrolled for a different class.

Please print clearly and duplicate this form to enroll more than one student in a family. Any omitted information below will delay registration and jeopardize your chances to enroll in the class before it fills.
