1. EXECUTIVE SUMMARY
This year has been one of change in IBAS, with two key people stepping down into what will probably be (for both) a not so “retiring” retirement. Kevin Ferguson FCPA has played a key role in IBAS since its inception and has steered the IBAS ship very firmly and well over the last 13 years since then. After years of loyal and incredibly “hands on” service, Kevin resigned from his position as IBAS Board Chair in April 2014, in order to concentrate on regaining his health – which, I am pleased to report ‐ has improved significantly since then. Fortunately Kevin remains an active Life Member of IBAS and we would like to thank him publicly for his remarkable drive and determination in overseeing IBAS’ development over the years since it was simply a “concept.” Kevin has supported the organisation, the Board and me personally over the years since IBAS’ inception in 2000. It has been a privilege to work with him.
We are most fortunate that Mr Bill Noonan OAM stepped into the role of Board Chair in April 2014 and feel confident that IBAS will continue to grow and flourish under his careful guidance.
Another IBAS stalwart, in the form of our Finance Manager, Bruce Abell, retired at the beginning of the 2014‐15 financial year. We are grateful for Bruce’s quiet, steady hand on the financial tiller over the last decade or so and wish him well in retirement. We are fortunate to have welcomed Rosalinda Polivka, former CFO of Ambulance Victoria as our new Finance Manager in June 2014. Rosalinda is rapidly becoming familiar with the issues and challenges associated with taking over the financial reins of an independent Medical Research Institute and we welcome her warmly.
On another personal note, one of our dearly loved IBAS Fellows (and former Director of the Respiratory Function laboratory here at Austin Health), Dr Jeff Pretto, sadly passed away suddenly from heart disease in early 2014. Several of the IBAS team attended his funeral in Newcastle. We honour his memory and continue to publish collaborative manuscripts that he has worked on. Vale Jeff!
In terms of research, IBAS researchers have had another productive year‐with evidence for this reflected in the full Annual Report accompanying this Summary. In later February 2014 IBAS hosted the annual Australasian Upper Airway Symposium which focuses on expanding understanding of the mechanisms underpinning the development of obstructive sleep apnoea with Dr O’Donoghue as Convenor.
A key educational activity was the Respiratory and Sleep Preceptorship for general practitioners, convened by Dr Christopher Worsnop. The RJ Pierce Prize for Best Respiratory or Sleep Poster at the Austin Research Week was won for the second year in a row by Justine Westlake. Dr Jeanette Tamplin was awarded one of the prestigious University of Melbourne Chancellor's Prizes for her doctoral thesis on the effect of group singing on respiratory function, voice, mood and quality of life for people with chronic quadriplegia. Dr David Berlowitz was successful in receiving an NHMRC Project Grant to complete the study entitled Continuous Positive Airway Pressure for Obstructive Sleep Apnoea in Quadriplegia: A Randomised Trial. This study is the final project of the large Victorian Neurotrauma Initiative‐funded suite of Sleep Health in Quadriplegia (SHiQ) studies which have involved over a hundred collaborators across Australia, New Zealand, Canada and the UK and have been a triumph of global co‐operation. The SHiQ program winds up in December this year, but the multi‐national study supported by the NHMRC grant will continue through 2016 until the full participant recruitment target is reached.

The Co‐operative Research Centre for Alertness, Safety and Productivity is ramping up with IBAS involved in three projects within the Healthcare Platform and two projects within the Sleep Disorders Phenotyping Platform. The first of the Healthcare Platform projects will be initiated at Austin Health, assessing the impact of shiftwork on intensive care unit staff.
The immediate future for IBAS looks bright but the medical research and health landscape is undergoing significant change and we must continue to build on our past successes with innovative strategies to ensure we thrive and grow. Initiating new and intensifying extant collaborations with national and international partners, the enhancement of strategic ties with other independent research institutes, and increasing our success in obtaining research funding from a range of government and other sources should assure IBAS’ continued growth in the medium and longer‐term.

The mission of IBAS is: To promote, facilitate and co‐ordinate research,
education and public advocacy in respiratory and sleep health.
The objectives of the Institute are:
1. To promote and provide a focus for research and education in respiratory and sleep health. 2. To reduce morbidity and mortality and improve quality of life for persons with respiratory

3. BOARD OF DIRECTORS
Mr Kevin David Ferguson, FCPA, (Chairman to 14 April 2014) Mr Ferguson is a Fellow of CPA Australia, having been admitted as a member in 1967. From 1978 to 2013 Mr Ferguson specialized in Litigation Support Reports, including Business Valuations, primarily for Family Law matters. Now semi‐retired he holds current Public Practice Certificate with CPA Australia and Tax Agents Registration to continue preparing financial accounts and tax returns. Mr Ferguson was Chairman of the IBAS Board of Directors since its inception in 2000 until April 2014. Mr Ferguson has been a Director of Mental Health First Aid International since August 2012. Other interests include being mentor for L2P Drivers and Parkville – Carlton Probus
Mr Bill Noonan, OAM (Chairman from 15 April 2014) Mr Noonan served as Branch Secretary of the Victorian/Tasmanian branch of the TWU from 1994 until his retirement from the Union in November 2009. He received an OAM in July 2006 for services to the Road Transport Industry particularly in respect to Road Safety and the Trade Union Movement. Mr Noonan is Chairman of the Victorian Police Blue Ribbon Foundation, and Vice Chairman of Western Melbourne Regional Development Australia Committee. In 2009, the Victorian Government appointed Mr Noonan as an Australia Day Ambassador. Mr Noonan was a founding Director of IBAS and was appointed Chairman and Secretary of the Board of Directors in April 2014.
Professor Christine McDonald, MBBS, FRACP, PhD, FCCP (CEO) Professor McDonald is the CEO of IBAS and the Director of Department of Respiratory and Sleep Medicine at Austin Health. Her special area of interest is chronic obstructive pulmonary disease (COPD) and its management, including respiratory outreach programs, pulmonary rehabilitation, smoking cessation, oxygen therapy and telehealth. Professor McDonald is also involved in collaborative research projects in the areas of lung cancer and interstitial lung disease.
Mr David Stanley, Grad Dip Adolescent Health Founding director and CEO of Convenience Group, an international public health agency, Mr Stanley has expertise in international, national, regional and community based health interventions that seek to change and/or sustain positive health behaviours in a range of contexts including sexual health, blood borne viruses, tobacco control, problem gambling, cancer prevention, and road safety. Mr Stanley is a member of the Australian Drug Law Reform Foundation. The founding Chairperson of the SEED Project (Sustainable Employment and Economic Development), and founding Director of COPE (Centre for Perinatal Excellence). David’s interests are public and population health based prevention, drug policy, leadership, politics, technology, cycling, boxing and sailing.

Dr Mark Howard, MBBS, FRACP, GDEB, PhD Dr Mark Howard is the Director of the Victorian Respiratory Support Service at Austin Health. He is a specialist physician in respiratory and sleep medicine with interests in long term ventilation of patients with respiratory failure and the impact of sleep disorders on driving. Mr Howard is a current board member for the Australasian Sleep Association and Theme Leader for the Co‐operative Research Centre for Alertness Safety and Productivity, in which IBAS is a partner
Dr Andrew Howard, MBBS, MBA Dr Andrew Howard is the Chief Information Officer at the Department of Health, Victoria. Previous roles include community and health ICT consultancy in the private sector, hospital management and hospital based medical practice.
Mr John McClelland, LLB, BA
Mr McClelland is principal of Carew Gartlan McClelland Solicitors. He is a former President of the Northern Suburbs Law Association and a Family Law Specialist accredited by the Law Institute of Victoria.
Mrs Tara Piazza GDAIS, MBIT Tara is an executive officer and program director at Public Transport Victoria where she is leading a team of specialists to implement complex changes to Victoria's public transport services. Tara was previously a senior Management Consultant with KPMG where she was the Victorian lead for organisational change in Financial Services. Her experience includes senior management in Victoria Police focused on strategic planning in Road Policing and the design and implementation of new business and governance frameworks in Professional Standards Command. Significant experience in top tier Financial Services has provided Tara with broad exposure to a range of practices, policies and methodologies, bringing expertise in strategic planning, business transformation, organisational change and project and program governance. Ms Piazza was appointed to the IBAS Board of Directors in October 2012.
Ms Eszter Vasenszky BA Ms Vasenszky is principal of Science PR Australia, a consultancy that helps science and medical research organisations better communicate their work to the public. She is a former News Corp journalist with extensive public relations experience in the not‐for‐profit sector. Ms Vasenszky was appointed to the IBAS Board of Directors in February 2014.

5. NEW PROJECTS
The following projects have been ratified by IBAS’ MSRC this year. Some of these projects are currently underway and some are still in the planning stage.
Influence of gender and bodyweight on lung volumes during sleep Ratified: July 2013 Abbreviated Name: Lung volumes during sleep IBAS Principal Investigator: Dr Amy Jordan
Obstructive sleep apnea (OSA) is a common disorder of repetitive upper airway collapse that occurs during sleep. The reason why the upper airway collapses during sleep in OSA is not completely understood. One factor that has been suggested to be important is a reduction in the amount of air in the lungs (lung volume) during sleep. This has been suggested because the upper airway is more collapsible when lung volume is low, and healthy young men have a reduction of lung volumes from wakefulness to sleep. However, lung volumes have been minimally studied during sleep. Many patients with OSA are overweight or obese and being overweight/obese is known to lower lung volumes during wakefulness. Therefore, the amount lung volumes fall during sleep may be different in people who are overweight compared to the healthy normal men who have been studied before. Furthermore, lung volumes are known to change with body position and gender during wakefulness and these factors also influence OSA. Therefore, we wish to accurately measure lung volumes in different body positions during wakefulness and sleep in people who are overweight/obese or of normal weight.
The findings of this study will be important for OSA but will also be relevant for other respiratory illnesses which are worse with obesity and sleep, such as asthma.
Do treatment interventions for obstructive sleep apnoea reduce depressive symptoms in patients with comorbid depression? Ratified: August 2013 Abbreviated Name: OSA, CPAP and Depression (COSAD) IBAS Principal Investigator: Dr Melinda Jackson
Obstructive Sleep Apnoea (OSA) is a highly prevalent, chronic illness, characterised by repetitive collapse of the airway during sleep producing oxygen desaturations and sleep disruption. OSA is associated with cognitive deficits, reduced quality of life, and depressed mood. The prevalence of both depressive symptoms and major depressive disorder (MDD) are significantly higher in OSA patients compared to the general community. Both conditions pose a significant health risk and significantly impact on brain function and general well‐being. Currently there is little clinical evidence to support whether treatment of OSA improves depressive symptoms in OSA patients with comorbid depressive symptoms or MDD. The aim of this study is to determine whether CPAP treatment improves depressive symptoms and cognitive functioning over the short and long term. This study will provide much‐needed clinical evidence of the efficacy of CPAP therapy for improving both depression and cognitive functioning in OSA.
Implementation of rapid on‐site evaluation for linear EBUS Ratified: August 2013 Abbreviated Name: Bronchoscopy ROSE TBNA project IBAS Principal Investigator: Dr Celia Lanteri
A bronchoscopy is a procedure that allows a doctor to look at a person's airway through a thin instrument. Bronchoscopies are done to help find out the cause of problems like difficulty breathing, cough or bleeding. Tissue samples can be taken during the procedure.
Traditionally, tissue samples are examined in the laboratory after the bronchoscopy has finished. The aim of this project is to evaluate the possible benefits of having the tissue samples examined during the bronchoscopy. The research is being undertaken with patients at Austin Health who are already booked in for bronchoscopies.

Does sleep disturbance bias memory towards negative events? Ratified: August 2013 Abbreviated Name: Sleep disturbance, memory and mood IBAS Principal Investigator: Dr Melinda Jackson
Insomnia is a common sleep disorder that affects 1/3 of Australians. It is associated with daytime impairments, including memory deficits and depressed mood. There is accumulating evidence that sleep disturbance is a precipitating factor in the onset of depression, however, the mechanisms underlying this link have gained little research attention. It is proposed that sleep disturbance influences the structure of memory, causing negative memory bias (where negative events are recalled more often) and producing a negative mood state (emotions including depression, sadness, anger, guilt). This study aims to determine whether Insomnia causes negative memory bias in patients with and without depression, compared to healthy controls. These results will provide a theoretical framework for understanding the role of sleep as a mechanism for depressed mood, and have implications for understanding the higher rates of depression in insomnia patients.
Music therapy as a potential aid in transition to NIV ‐ a pilot study Ratified: October 2013 Abbreviated Name: Music for NIV in MND pilot IBAS Principal Investigator: Dr David Berlowitz Overall Principal Investigator: A/Prof Felicity Baker (University of Melbourne)
Motor neurone disease (MND) is a condition characterised by muscle weakness. There is no known cure and weakness of the breathing muscles is usually the cause of death. Once a person's breathing muscles are no longer strong enough, breathing support can be provided by a machine which provides air via a mask placed over the person's nose and mouth. This is called non‐invasive ventilation (NIV).
As you can imagine, starting this kind of breathing support can be distressing. Negative experiences early on can result in someone not using the machine as much as they need, therefore affecting their health and quality of life.
Clinical experience shows that music therapy may help people make a smoother transition to using ventilation support. As well as reducing anxiety, music can help people sleep better, something that is important as night time is the most common time the ventilation support is used.
This project is an initial small study based at Austin Health and Calvary Healthcare Bethlehem to collect some information on how music therapy might help people starting on ventilation. The plan is for these results to inform an application for funding for a larger project.
Sleep disturbance effects on cognition, affect, metabolism and physiology Ratified: February 2014 Abbreviated Name: SCAMP IBAS Principal Investigator: Dr Melinda Jackson
Obstructive sleep apnoea (OSA) is a disorder characterised by repetitive upper airway collapse and frequent arousal from sleep. OSA adversely affects various health domains including neurocognitive function as well as cardiovascular, metabolic and airway physiology. These consequences are all likely to be at least in part a result of the sleep disturbance that occurs in OSA. Although several prior studies have investigated the effects of total or partial sleep deprivation on cognition and physiology, sleep disturbance, such as that which occurs in OSA has been studied much less. The overall aim of this study is to investigate the effects of sleep disturbance on previously un‐investigated aspects of cognition, mood, metabolism and physiology.

The effect of functional electronic stimulation (FES) cycling on periodic leg movements (PLMs) during sleep in patients with acute spinal cord injury: An observational pilot study Ratified: March 2014 Abbreviated Name: FES and PLMs in SCI IBAS Principal Investigator: Dr David Berlowitz
Periodic Limb Movement Syndrome (PLMS) is a common condition that occurs during sleep. The movements are abnormal and in many cases are associated with brief awakenings from sleep causing poor quality sleep, sleepiness and significant impairment of functioning during the daytime.
The diagnosis of PLMS usually involves overnight testing in a sleep laboratory to monitor leg movements. In 2007, the American Academy of Sleep Medicine (AASM) published a major revision of their guidelines for measuring sleep and associated events. However, there is little available evidence to support many of these guidelines and this is the case for PLMS.
This current project application focuses on methods for measuring PLMS. One of the methods is recommended by the AASM and the alternative is widely used in clinical practice. We expect our results to lead to improvements in diagnosis and treatment of this disorder and also inform the development of future standards.
Autobiographical memory in obstructive sleep apnoea: a neuroimaging study Ratified: March 2014

Obstructive sleep apnoea (OSA) is a common respiratory disease, caused by collapse of the airway during sleep, which produces dips in oxygen levels and sleep disruption. Untreated OSA may lead to unrefreshing sleep, sleepiness during the day and difficulties with memory, learning and mood. Recent studies have shown that some memory changes in OSA patients are linked with depression. OSA patients have also been found to have structural and functional changes in the brain on neuroimaging compared to healthy people.
The aim of this study is examine memory and brain structure and function in OSA patients compared to healthy people without OSA. Specifically, this study will determine whether changes in brain structure and function observed in OSA patients are associated with memory impairments. This study will provide preliminary evidence about how functional changes in the brain affect memory in patients with OSA.
Investigation of relationship between respiratory muscle strength and body mass index (BMI) Abbreviated Name: Obesity and respiratory muscles Ratified: April 2014

It is known that there is a relationship between obesity and how well some parts of the breathing system work. It is less well known what effect obesity has on the muscles that are used for breathing. This project aims to investigate the relationship between body mass index (a measure of obesity) and respiratory muscle strength.
We will also investigate any differences in respiratory muscle strength between those that do and don't have obesity hypoventilation syndrome (OHS). Other factors which could influence this relationship such as heart function and lung function will also be taken into account.
It is expected that there will be a relationship between BMI and respiratory muscle strength, with respiratory muscle getting weaker as BMI increases. It is also expected that those with OHS will have weaker muscles than those who don't.

6. IBAS GRANTS
IBAS offers internal grants to facilitate the meeting of its objectives. This year, IBAS offered funds for a Research Grant, Conference Funding, an Education Grant and Writing Scholarships.
RESEARCH GRANT IBAS Research Grants are intended to provide funding for projects that align with the aims, objectives and strategic direction of the Institute. Grants can be made for stand‐alone project, pilot projects, top‐up funding or equipment. Six projects applied for the 2013‐14 Research Grant, with a maximum of $40,000 available. The projects that were successful in gaining funding are:
1. Project Title: Lung Volume Recruitment in Neuromuscular Disorders
Lead Investigator: Nicole Sheers Amount awarded: $20,000
Not being able to take deep breaths or cough forcefully are two examples of the breathing problems that people with a disease affecting the nerves and muscles, such as Duchenne Muscular Dystrophy or Motor Neurone Disease, may face. In these diseases, breathing complications are one of the main causes of discomfort, disability and ultimately death. A technique called 'lung volume recruitment' (or 'breath stacking') uses a specially designed bag to deliver breaths of air to people via a face‐mask or mouthpiece, thereby allowing people to take a series of deep breaths occasionally. This may improve lung volume or breathing capacity, coughing ability and ease of clearing phlegm in this group of people. This project will investigate the effect of breath stacking on these factors. Improving breathing function is an important aim of treatment for people with neuromuscular diseases and may reduce symptoms, improve quality of life and survival.
2. Project Title: Exercise training and immunity in COPD
Lead Investigator: Anne Holland Amount awarded: $13,408
'Flare‐ups' of chronic obstructive pulmonary disease (COPD) are important events which often lead to deterioration in lung function and admission to hospital. Whole body exercise training prevents 'flareups' of COPD, but the mechanism by which this is achieved is not known. It is possible that exercise training can improve immunity in the lungs of people with COPD, preventing them from getting chest infections; however this hasn't ever been tested. The aim of this study is to see whether exercise training alters immunity in the lungs of people with COPD, by collecting a marker of immune function in saliva. If we can find out how exercise training alters immunity in COPD, this will allow us to tailor exercise programs so that people with COPD can stay well and stay out of hospital.
3. Project Title: The feasibility, reliability and responsiveness of tests of physical function in individuals with acute exacerbations of COPD
Lead Investigator: Christian Osadnik Amount awarded: $6,000
Individuals with COPD commonly present to hospital due to exacerbations of their respiratory symptoms. Quadriceps weakness, in particular, has been linked with increased likelihood of exacerbation, poorer health outcomes and increased mortality. Performance of basic mobility (e.g. walking) during an exacerbation of COPD is common, however the benefits of formal physical exercise regimens (e.g. lower limb endurance and strength training) are not well defined in the medical literature. A significant problem inherent in such literature is the absence of an appropriate tool to measure physical function. Traditional measures of exercise tolerance (e.g. 6‐minute walk test, incremental shuttle walk test) and muscle strength (cybex isokinetic ergometry) can be difficult to perform in the acute inpatient setting. A simple and quick instrument capable of measuring both physical function and strength could enable clinicians and researchers to better prescribe and measure the effect of physical rehabilitation during this important phase of their condition. Two emerging outcomes for use in COPD are the 60 second sit‐to‐stand test and four metre gait speed test. Both tests have been validated for use when clinically stable, however neither has been investigated during hospitalisation for an acute exacerbation of COPD. This study aims to determine the feasibility, reliability and responsiveness of the 60 second sit‐to‐stand test and four metre gait speed test for individuals with acute exacerbations of COPD.

4. Project Title: Examining brain structure in obstructive sleep apnoea
Lead Investigator: Fergal O’Donoghue Amount awarded: $592
Obstructive Sleep Apnoea (OSA) is a common disorder where a person experiences pauses in their breathing during sleep. OSA can result in daytime sleepiness and impaired thinking as well as lead to high blood pressure and other poor health outcomes.
This research will look at whether untreated OSA has an effect on the brain. Previous research has provided conflicting answers to this question but, with the development of new technology, more subtle changes can now be seen. This area of research is worth pursuing as it may help explain the impaired thinking that is associated with OSA and create an even greater incentive for people to seek treatment.
This project is being run in collaboration with the Brain Research Institute and is part of IBAS' Sleep Health in Quadriplegia program of research.
Calls for applications for the 2014‐15 Research Grant have been made, again with $40,000 available.
CONFERENCE FUNDING IBAS Conference Funding aims to facilitate attendance at relevant conferences by IBAS Personnel, in order to disseminate findings of IBAS research, increase knowledge and learning and facilitate the formation of collegial links. 2014‐2015 Conference Funding was decided in June 2014. Funding will support more than twenty five people to attend conferences (some people are supported to attend more than one conference).
WRITING SCHOLARSHIP IBAS Writing Scholarships are designed to facilitate submission of publications by decreasing barriers to the process of writing.

As noted in the introduction to this report, this has been a good year for awards for IBAS personnel, with the following awards having been received this year:
September 2013: Christian Osadnik was awarded a European Respiratory Society Travel Grant at the ERS conference, based on his oral presentation: A multi‐centre, randomised controlled trail of positive expiratory pressure mask therapy for inpatients with acute exacerbations of COPD and sputum expectoration with co‐authors (including IBAS personnel Christine McDonald, Caroline Chao, Cristino Oliveira, Nadia Gagliardi, and Anne Holland).
October 2013: Christian Osadnik was awarded a Cardiorespiratory Physiotherapy (chronic disease) Prize at the APA conference, based on his oral presentation The effect of positive expiratory pressure for airway clearance on ventilation inhomogeneity in individuals with stable COPD and chronic sputum expectoration with co‐authors (including IBAS personnel Christine McDonald, Caroline Chao, Cristino Oliveira, Nadia Gagliardi, and Anne Holland).
November 2013: Anne Holland was awarded a Mid‐career Researcher Excellence Award from La Trobe University, in recognition of her international achievements in pulmonary rehabilitation research.
November 2013: Justine Westlake was awarded the Rob Pierce Memorial Award at Austin Research Week, for her poster titled Shift Do shift workers fare as badly as obstructive sleep apnoea patients: investigating sleepiness, mood and performance with co‐authors Gerard Kennedy, Paul Emerson, Phillip Swann and Mark Howard.
