Enhancing Patient Care, Transforming Our Specialty

As physicians, we are called to help people. That internal calling is amplified when tragedies occur, such as earthquakes or tornadoes, or when we get everyday glimpses of those living in stark poverty. Our clinical colleagues can simply roll up their sleeves and immediately provide hands-on care. For pathologists the gut response can be more challenging. The College of American Pathologists (CAP) Foundation provides the conduit to overcome those challenges. For nearly 50 years, the Foundation has been our philanthropic avenue, and this year we’ve embarked on a new mission of connecting people to our life-saving skills. The crux of our patient-centered mission is clearly exemplified in See, Test & Treat. Our program enables

volunteer pathologists to connect directly with women who face difficult decisions in their daily lives. These women have to choose between getting a mammogram or buying food for their children. They must weigh whether to get a Pap test or put gas in the car. See, Test & Treat empowers women to get the medical attention they deserve without having to make financial sacrifices. The women can receive free cervical and breast cancer screening as well as a follow-up care plan all in one day. The See, Test & Treat program also puts the Foundation on the forefront of the transformation of our specialty. We can, and should, spend time with patients. It gives us the rewarding opportunity to interact with the people we help. It also helps patients better understand their condition, which strengthens their resolve to partner with their health care providers. The entire spectrum of medicine and its delivery is changing, and our transformation is critical for us to lead the field in the right direction. There has been no greater time in which we stand perfectly poised to create such a large, sustainable impact. Our awards and grants further that transformation as they place our members on the cutting edge of innovative patient care and pathology informatics, which ultimately leads to better patient outcomes. Together, we can transform our specialty and the entire health care system. This year, the Foundation has transformed into a stronger nonprofit entity that will provide us with a sense of great pride. I want our success to flourish so that the organization is the place where pathologists give their support, specifically to do humanitarian work in pathology. I realize that not everyone is able to go to Haiti or bring

a health care program to his or her community. But all of us can support those who volunteer their time and use their expertise to create a better world. As this report looks back on our accomplishments of 2011, let us be inspired to create new dreams for 2012 and beyond. I invite you to turn these pages, read on, and learn more about the CAP Foundation. Let us share all we have seen and achieved this year. You are sure to see that we owe much

of the balance of our success to our dedicated volunteers, our generous donors, our committed board of directors, and our enthusiastic staff, as well as the CAP’s Board of Governors and staff leadership. Creating a Lasting Impact on Community Health

Before Angie and Elida came to our See, Test & Treat screening, phrases that we speak fluently were for them quite foreign—phrases like preventive care, Pap tests, and routine screening. But that changed when they participated in See, Test & Treat. Thanks to the vision of the CAP and CAP Foundation leaders, as well as the commitment of CAP members, donors, and staff, Angie and Elida joined the growing number of underserved women who gained newfound access to preventive care and patient education through the Foundation’s signature program, See, Test & Treat. CAP member pathologists volunteer their time and talent for each See, Test & Treat same-day health screening. These busy physicians help identify patient communities in need, coordinate interdisciplinary clinical teams, and lead the delivery of free, same-day cervical and breast cancer

testing, diagnoses, and follow-up care. Each program also includes services to help overcome barriers to care endemic to the specific communities. These can include translation services for immigrants or childcare and transportation services for working mothers taxed to find time for self care.

Connecting care to communities in need Much of the 2011 success you will read about in this report traces back to the Foundation’s visionary leadership. For nearly 50 years, we have championed the pathologist’s role as the skilled specialist who obtains, interprets, and communicates the diagnostic tests that define each patient’s health and healing. Helping these specialists develop and apply skills where they are needed most, the Foundation has long been committed to connecting pathologists to resources, patients, education, and technology. But this year, guided by a committed board of directors, we sharpened our focus. With the launch of a bold strategic plan in 2011, we began to raise the visibility of the critical connections between pathologists, patients, and quality care. See, Test & Treat is the focal point of this strategy. It’s a focal point rooted in the CAP Foundation’s leadership’s careful 2010 analysis, which recognized the great domestic potential inherent in our then-bantam See, Test & Treat program. Here was an existing program we could scale up, one that can have measurable, lasting impact on public health. And better yet, in a very visible, valuable way, the program shines a light on the pathologist’s position at the heart of health care. Here, at See, Test & Treat screenings, pathologists are changing the face of community care. Bringing that bold vision from the

Futurescape IV: Preparing for Medicine’s FutureMore than 200 pathologists, pathology residents, and industry professionals shared energizing perspectives on pathology’s future at the CAP Foundation’s Futurescape of Pathology IV Conference, April 2011, in Chicago.Through the CAP Foundation’s William R. Dito, MD, Fund for Informatics, 16 young pathologists received resident travel awards to attend the conference. “Initially, when Dr. Hunt talked about pathologists making treatment recommendations to patients, I asked myself, why are pathologists recommending treatments? Are we crossing the line here?” wrote award recipient Jiong Zhang, MD, University of Tennessee. “However, by the end of the conference, I was asking myself another question: Indeed for many malignancies, most of the information needed to make treatment recommendations is in pathology, so why aren’t more pathologists making treatment recommendations?” Exploring cutting-edge topics from medical informatics to digital microscopy and personalized medicine, Futurescape put the dynamics of the resident’s role in a fast-forward perspective. In closing the conference, CAP President Stanley J. Robboy MD, FCAP, then president-elect, summed it up best: “Never has it been more important for us to step up to the plate and take ownership of the unique expertise we bring to the table.”

Advanced Training Grants: Translating Research and Innovation Into Improved Patient Outcomes

Given a sputtering world economy and dynamic US health policy, today’s young pathologists find it increasingly difficult to garner the time and resources for research and targeted training that can deepen capabilities. And yet diagnostic developmental research and subspecialty expertise continue to be essential to pathology’s role in improving patient outcomes. We are grateful to report that in 2010 and 2011, the CAP Foundation was able to help.

Leading in the LaboratoryBolstered by generous commitments from our corporate partners, the Foundation awarded Advanced Training Grants to 13 pathology residents. Through these, the next generation of pathologists gain real-world industry exposure and help forge the discovery of potentially practice-changing techniques and technologies. Emily O’Rourke, MD, Cedars-Sinai Medical Center, Los Angeles, was among those recipients. Backed by a Beckman Coulter, Inc., Advanced Training Grant in Laboratory Management, Dr. O’Rourke headed to the University of California San Francisco (UCSF) clinical laboratories, which incorporate multiple hospitals. From one-on-

one didactics to state public health department advisory meetings, her deep-dive rotation ultimately challenged her to serve as acting director of the laboratory. “This type of dedicated time was extremely valuable and is rare in residency training,” she said. “Also, the importance of affecting change through advocacy has prompted me to become more involved in professional committees at the state and local levels.”

Both Sides WinMade possible through the Foundation’s corporate partnerships, such opportunities prove as valuable to the grantor as they are to the grantee. “Fresh perspectives across the field are proving essential to prepare future innovators of science,” explains pathologist Eric E. Long before health care reform topped the news, the CAP and CAP Foundation recognized that laboratory quality assurance through ongoing quality improvement initiatives was essential to excellent patient care.

The CAP Foundation John H. Rippey Grant for Quality Assurance funds innovative projects that aim to improve laboratory processes and quality.

The 2011 Rippey Grant was awarded to a team from the University of Mississippi Medical Center, Jackson, focused on an initiative to reduce errors while increasing laboratory efficiency, productivity, and employee satisfaction.

Scholars Research and Residents Research Grants: Advancing Knowledge and Skills

The titles of projects funded through the CAP Foundation’s Scholars Research and Resident Research Programs are rife with phrases no one wants to hear applied to a loved one: aggressive tumor, myelodysplastic syndrome, and pediatric lymphoma. And yet by affording young pathologists opportunities to conduct research that moves discoveries from the lab bench to the patient bedside, the CAP Foundation helps build the body of specialty knowledge. Since the program began more than 20 years ago, the Foundation has awarded grants totaling more than $1.5 million. Scholars Research grant recipient Patricia Ann-Ramaley Brunker, MD, PhD, FCAP, of the National Institutes of Health in Bethesda, Maryland, sought to unravel the genetics of blood proteins among African American patients. Practically, her research focused on clinical utility for a potentially lasting impact. “Applying our discoveries to test techniques could achieve more accurate blood product matching for transfusion recipients,” she explained. Leadership and Research Grants: Raising Pathology’s LeadersThese grants and awards help harness the talents of young pathologists to develop their skills as tomorrow’s leaders of quality patient care. Already gaining notice on the global health stage, Ranjana Arora, MBBS, MD, MPH, received the 2011 CAP Foundation Leadership Development Award. This travel grant allowed the University of Kentucky pathology resident to attend the CAP ’11 Residents Forum meeting in Texas, and the 2011 CAP Policy Meeting in Washington, DC.

Though Haiti was flooded with international aid following the historic January 2010 earthquake, the CAP Foundation’s giving focused on building long-term sustainable value.

Through CAP member volunteers and generous donations, the Foundation awarded nearly $40,000 in grants in 2011 to aid Haiti’s healthful recovery.

Grace Children’s Hospital in Port-au-Prince was among its grant recipients. With 80% of the hospital destroyed after the quake, children were treated in temporary structures vexed by spotty electricity that compromised care.

In a project led by Raymond D. Aller, MD, FCAP, with a Foundation grant, a volunteer team delivered and installed power supplies that restored clean, stable electricity to the hospital laboratory. As a result, today the lab’s technicians are delivering more test results, with improved speed and reliability, to more children and their families. “We just want to do what we can to help them,” said Dr. Aller, who also helped distribute five microscopes donated by Nikon Instruments, Inc.

Here at home, CAP member volunteers and the Foundation’s See, Test & Treat grants have provided free cervical and breast cancer screening to more than 1,000 uninsured and underserved women to date.

Recent grant-based screenings at Minnesota’s NorthPoint Health and Wellness Center and at Massachusetts’ Tufts Medical Center exemplify the program’s growth and achievement. And even more 2012 screening programs are already in the works.“Cancer survival rates are directly related to the stage of the disease at the time of diagnosis, so it’s important to remove all barriers to early diagnosis and treatment for all women,” said Bradley M. Linzie, MD, FCAP, a participating pathologist at Hennepin County Medical Center in Minneapolis. Read the details on page 19.

See, Test & Treat is a free cervical and breast cancer screening program for underserved women. Forward-thinking CAP member pathologists partner with gynecologists, radiologists, and other medical professionals to deliver same-day screening, results, and follow-up care. Through this program, pathologists meet the needs of women right here in our own communities, women who lack fundamental access to health services. Women are seen, tested, and treated in one visit—removing significant barriers to preventive and follow-up care, such as arranging childcare or additional time off work. Through this program women receive a pelvic exam, along with a Pap test to screen for cervical cancer and a mammogram for early detection of breast cancer. Women have an opportunity to discuss their test results with a pathologist who is on site to provide counsel, diagnoses, and interpretation of the results.

BostonSee, Test & Treat came to the Tufts Medical Center on October 15, 2011, and made life-changing discoveries. The program was aimed at reaching women in Boston’s Chinatown and nearby neighborhoods. More than 40 women were screened and 31% received abnormal results from their Pap tests and 10% had abnormalities identified through mammograms. Pathology services were provided by Barbarajean Magnani, PhD, MD, FCAP; Nora V. Laver, MD, FCAP; Barbara J. Weinstein, MD, FCAP; and Stephen P. Naber, MD, PhD, FCAP.

MinneapolisDuring Breast Cancer Awareness Month, the NorthPoint Health & Wellness Center hosted its sixth annual See, Test & Treat program that served 156 women. Of the 156 patients served, 71 had Pap tests, of which four percent were abnormal. One patient was diagnosed with invasive cervical cancer. In addition to the free cervical and breast screenings at the October 4–6, 2011, event, women were screened for high blood pressure, cholesterol, and diabetes. Bradley M. In 2011, the Foundation introduced the Gene and Jean Herbek Humanitarian Award, an annual award to provide a $25,000 grant to a CAP Fellow who provides leadership in a See, Test & Treat program. The purpose of the award is to build the impact and extend the reach of the See, Test & Treat program beyond its core services. The Herbek Humanitarian Award is inspired by CAP President-elect Gene N. Herbek, MD, FCAP, who brought the first See, Test & Treat program to Native American women in South Dakota, and by his wife, Jean, who shares his commitment.

The award is represented by the breathtaking steel sculpture, entitled “The Circles of Life,” forged by pathologist and artist Ann T. Moriarty, MD, FCAP. While the original sculpture will reside at CAP headquarters in Northfield, Illinois, recipients of the Gene and Jean Herbek Humanitarian Award will receive a personal representation, also created by the artist. The first presentation of the award will take place at the September 2012 annual meeting.

The Founder’s Circle Launches the Herbek See, Test & Treat Fund A Founder’s Circle has been established by generous donors who share the Herbeks’ vision to provide health care to women in underserved communities. Donors who pledge their support to the Herbek See, Test & Treat Fund provide the financial resources necessary to support both the operations of See, Test & Treat programs across the nation and the Gene and Jean Herbek Humanitarian Award presented annually.Founder’s Circle members help build the capacity of the program, while providing the financial resources to actualize the vision of a humanitarian pathologist who dares to take a See, Test & Treat program one step further.
