DEAR DR. ROACH: My question is about my sister, who 66 years old with aortic disease and who’s had four aneurisms: an ascending aneu-rism 4.5 x 4.6 cm, a descend-ing at 4.2 x 4.7cm, and also an abdominal at 2.4 x 3.2 and another just at 2 cm. I missed an in-depth story on the local evening news, but its hook was “a breakthrough on treat-ing aneurisms that dramatical-ly shrinks them.” — K.K.

ANSWER: An aneurism is a dilation of a blood vessel at a site of weakness. This can happen in virtually any blood vessel, but they are most con-cerning in the heart, the brain and the aorta, the main blood vessel that takes the blood from the heart to the body. In your sister, the large aneurisms are in the thoracic aorta, the part of the aorta that is in the chest. The major risk factors for thoracic aneurisms are high blood pressure, high cholester-ol and smoking. However, ge-netic diseases like Marfan’s syndrome and Ehlers-Danlos syndrome should be suspected in younger people with thorac-ic aortic aneurisms. Infectious and inflammatory diseases like syphilis and giant cell arteritis are uncommon causes.

Medical therapy includes blood-pressure reduction, es-pecially by beta blockers, management of high cholester-ol and diabetes if present and tobacco cessation. The deci-sion of when and how to treat aortic aneurisms surgically should be made by an expert.

I don’t know what the “breakthrough” referred to on your news was. However, I suspect it was one of several new devices that are placed in-side the blood vessel to let blood flow through while pro-viding strength to the wall. Re-pair also can be done through an open chest procedure. The decision of when to operate is made by carefully reviewing the size of the aneurism and how quickly it is growing.

DEAR DR. ROACH: Will colloidal silver (suspen-sion of ultra-fine silver) help a sore throat if sprayed or gargled? My camping friends say this will sanitize drinking water. — J.E.

ANSWER: I am aware that other media doctors have recommended this product. However, I am unable to find enough evidence that it is safe

and effective for me to do so, and I recommend against us-ing it for either a sore throat or for sanitizing drinking water.

For sanitizing water, I recommend filtering with a 0.1 micron filter, available at camping supply stores.

DEAR DR. ROACH: Should I be concerned with blood tests that show MCV, MCH and mono numbers in the high range? What do high readings indicate? — C.P.

ANSWER: “MCV” is “mean corpuscular volume,” and “MCH” is “mean cor-puscular hemoglobin” — both indicators of the size of red blood cells. The mono-cyte (“mono”) is a type of white blood cell.

Large red blood cells hap-pen with deficiencies of vitamin B-12 and folic acid. They also come about with many kinds of uncommon and rare blood dis-eases. Monocyte levels can be high in infections and certain other kinds of blood diseases.

All that being said, the “nor-mal” range from a laboratory is based on a sample of people in the population. By definition, 5 percent of normal, healthy peo-ple will have a lab test result that comes out as abnormal. If you get 15 or 20 lab results, there’s a very good chance that at least one will come out ab-normal. Lab tests always must be interpreted keeping the whole situation in mind.

That mutual fund you bought, figuring it would shield you from pain in the stock market, may turn out to be a wolf in sheep’s clothing.

I’m talking about the mutu-al funds in 401(k) plans, indi-vidual retirement accounts and 529 college savings plans that carry the soothing words “moderate allocation” in their names or descriptions. These are the no-brainer funds that have become popular because novices don’t have to know much about investing. They simply buy a relatively mild-mannered fund containing both stocks and bonds and then they’re done making decisions.

People with these funds may assume they can go on with their lives, while rely-ing on a fund manager to avoid taking big chances in the stock market.

But many funds that were cautious after the financial crisis have begun to morph into something very differ-ent. Fund managers that run a number of “moderate” funds are bulking up on stock and seem to have forgotten they are choosing invest-ments for those who might be afraid of sharp losses.

So if the stock market turns ugly, risk-averse peo-ple could be stunned by large losses in retirement and college savings.

Morningstar analyst Greg Carlson recently found moder-ate allocation funds that toned down the risk they were taking when investors were afraid in 2008-09 were only temporari-ly cautious. In February 2009, according to Carlson, the typi-cal moderate-allocation fund had only 55.3 percent of inves-tors’ money invested in stocks,

with the rest in safer alterna-tives — bonds and cash.

But by the end of Novem-ber last year, many funds with conservative reputations had shirked the conservative approach and channeled a rel-atively risky 70 to 75 percent into stocks. Some of the mod-erate allocation funds that have added risk are Dodge & Cox Balanced, Fidelity Puri-tan, Invesco Equity and In-come, and Oakmark Equity and Income, Carlson said.

Keeping 70 percent of an investor’s money in stocks is a fairly large proportion and could mean big losses in a sharp stock market downturn. It is far higher than the 60 percent average that’s usually considered “moderate.”

For a taste of the difference, consider the financial crisis. If a person had $10,000 invested just before the stock market started falling 50 percent in late 2007, and put 70 percent into stocks and 30 percent into bonds then, he would have had only about $6,540 left by March 2009. If instead he had been more conservative, and had 60 percent in stocks and 40 percent in bonds, he would have had $7,140 left at the scariest moment in 2009. Be-cause the person with a bigger chunk of money in stocks lost more money, it took him lon-ger to get back to even.

Three years after suffer-ing the worst losses, the per-son with 60 percent in stock (the Standard & Poor’s 500 index) and 40 percent in long-term government bonds had not only recovered, but had amassed a total of $12,340 in the moderate stock and bond combination. The person who had the riskier 70 percent allocation in stocks hadn’t regained as much. He had just $11,725.
