Medical Release: I hereby consent to emergency medical

and/or hospital service for the registered minor to whom I

am a legal guardian that may be rendered by or at

accredited hospitals by appointed physicians in the event

such need arises in the opinion of a duly licensed

physician.

Injury and Image Display Waiver and Indemnity

Agreement: I accept that my ward’s participation in this

program relieves responsibility of any kind by the Tennis

Center at Steamboat and any other sponsoring entity. I do

hereby for and on behalf of my heirs and legal

representatives RELEASE and forever discharge the

Tennis Center at Steamboat, its officers and

representatives, representatives of the sponsoring entities,

or third parties, which injuries may be in any way related

to my activities during the program and any period

traveling to and from the events described and all such

claims are hereby waived and released and covenant not

to sue therefor. Additionally, I accept and understand that

The Tennis Center may use my child’s photo and or image

in promotional material. I have read and understand the

foregoing release and indemnity agreement.

Campers will be taught basic strokes, rules, and etiquette playing tennis. There will be agility, balance and speed activities every day. Games will be played while kids prac-tice sportsmanship and fair play. Kickball, cap-ture the flag, and more field games will also be played to keep the kids moving. Board games will be played during breaks to cool off!
