Patient Navigator is the name under which all Patient Advocate Employee Population Health and Wellness Management Services and Reports are delivered

Patient Navigator is a unique employer management system that captures data, activity, measures outcomes and brings them all under one umbrella to provide integrated medical services to employees as well as high level executive summary reports to our clients.

Electronic personal health record – The Patient Navigator portal provides each employee their life-long, personal electronic health records, automatically updated with their GP records and hosted by N3 security, for complete confidence and complies with information governance requirements.

With patient consent, patients can share their personal health information with members of the health professional team over multiple devices. Personal electronic health records, care pathways and care plans are driven by authoritative data from local provider systems with interactive, comprehensive alerts and follow up to every patient.

Patient Navigator is supported by an app – for Apple, Android and interfaces with a desktop, smartphone or TV, providing full communication facilities for chat, phone and messaging which is interactive and free.

Records are updated nightly – from GP systems allowing for interactive, multilingual alerts to remind, inform or follow up individuals for the purposes of early interventions, screening and long term condition health checks.

Data from physician systems – drives personal care pathways to provide evidence based comprehensive support, online access to chat message, telephone or video call to a Patient Advocate Nurse Case Manager.

Wellness, lifestyle and prevention – support through free access to ‘The Picture of Health’ self-assessment tool to measure your health and wellbeing. In less than 30 minutes you can gain total clarity about the healthiness and balance of your current lifestyle and where you are gaining and losing your precious life energy. You will see crystal clear where healthy changes could be made to get you feeling full of life and well protected from health risks. Absence Management – experienced Nurse Case Managers provide early intervention strategies and support to employees spanning from day one absence reporting through to long term chronic health or disability case management. Avoiding delay to diagnostics and treatment options improves health outcomes and leads to a co-ordinated rehabilitation process. Patient Advocate Nurse Case Managers work with our Occupational Health Team to manage ill health retirement, complex illness or injury and long term vocational rehabilitation in the workplace. Providing a complete absence management programme offering proactive, accurate collection of data and rapid response interventions through to complex long term rehabilitation.

Accelerated Nurse Case Management – seamlessly utilises both NHS and private treatment pathways for employees to access safe, appropriate and expedient care at the point of need. Our experienced Nurse Case Managers provide collaborative, evidence based support to address an employees health, social care, educational and employment needs, using communication and available resources to promote quality cost effective outcomes.

Access to the Patient Advocate Health Creation Programme – companies can elect to pay for at onset for all employees or it can be employee subscribed.

Through the Patient Advocate Health Creation Programme you will be helped to recognise and remove self-limiting beliefs and behaviours that prevent you from fulfilling your potential for conscious living. As you work through the four areas of body, mind, spirit and environment you will gradually improve and

develop a healthier relationship with yourself and others, whilst increasing vitality to enjoy those things in life that bring you fully alive.

This online programme can be used independently or with the help and guidance of a Patient Advocate Health Creation Mentor to support and encourage you through your journey to better health.

Access to the Patient Advocate Cancer Lifeline Kit – the Kit is aimed at those newly diagnosed with cancer and their carers. The interactive kit provides three workbooks; one for the time of diagnosis and treatment, one for the time of recovery; and one for those in the caring role. The kit supports people to positively cope with cancer treatment and make good treatment and self-help decisions with two guidebooks: The Frontier Cancer Medicine Guide looking at current advances in cancer treatments and ‘The Alternative Cancer Treatment Guide’ providing an overview

of the most popular alternative cancer treatments for which there is the most evidence.
