1. Safety Team General Hazards of the location of work should be identified (e.g., close to HV equipment, working at height, work in confined space, etc.) and control measures developed should be specific.

No lifting work with 2.75m or above the level of OHL cable shall be conducted. (Section 6 viii) Manholes will be constructed in open cut method. Safety measures for confined space work inside manhole will be implemented. (Section 5 ix)

2. Safety Team General Plant and equipment should state, related hazards are identified and control measures should be specific.

Excavator will be used. (Section 3) Banksman will be appointed and stand outside the entrance of fence off area. (Section 6 iii)

3. Safety Team General Materials used, stored, handled and transported should be stated, related hazards shall identify and control measures are specific.

GI, concrete & clay pipes etc. will be used. (Section 3) All pipes shall be wedged in temporary storage area (Appendix F), and lifted by mobile crane. (Section 3)

4. Safety Team General Requirements of supervision shall clearly be stated (especially for confined space)

CP and foreman shall supervise the confined space work, during dismantle of formwork of manhole. (Section 9.2)

5. Safety Team General Emergency plan or contingency arrangements must be stated for confined space

Please see section 9.2 for the contingency arrangement.

6. Operations Railway Protection

General For excavation deeper than two metres and within 10 metres from the edge of the railway furniture, please bring up the ELS for our comments.

Metal shoring will be erected to both sides of excavation. (Section 8 v)

7. CM team General Please clearly show which portion of the works would be within 10m from the edge of the railway furniture.
