    Special Presentation D. Mosbarger The 2007 IPLAN objectives included screening for risk factors for heartdisease, including blood pressure, cholesterol, blood sugar, and BMI/bodyfat. Based on screening results, the population could be educated on how toimprove their cardiovascular health. Sherwin-Williams, a local industry,

    conducted its first employee health fair in 2009. Using the results of thehealth fair, they implemented several programs aimed at reducing bloodpressure, reducing BMI, and reducing cholesterol.

    In October, 2011, Sherwin-Williams conducted another wellness fair fortheir staff. The results were OUTSTANDING!

    27% of their staff dropped their cholesterol below 200 overprevious scores and another 11% brought their cholesterol below250;

    17% of their staff improved their blood pressure to a normal level; 52% of their staff decreased their BMI below the level considered to

    obese.In addition, according to a study conducted by Chenoweth and Associates,Sherwin-Williams has saved an average $14,000 annually through improvedemployee health! This is one small step toward achieving our IPLANobjectives of reducing cardiovascular deaths among Clay County residents.

    Continue to recognize anddocument successes in healthimprovement throughout thecommunity.

    Community Health

    Survey

    G. Paule Health Department has collected approximately 125 surveys to-date. Ourgoal is to collect a total of 370 surveys. We are at about one-third (1/3) ofthe target goal. Committee members are asked to distribute the CommunityHealth Survey and collect 10 surveys each. Survey respondents must meetthe following criteria: Clay County resident age 18 or older. Paper copies

    of the survey were distributed to Committee during the meeting.

    An electronic version of the surveywill be emailed within one week.Each Committee member will collectten (10) completed surveys byDecember 7th.

    Visioning L. Call Laurie Call, Director, Center for Community Capacity Development of theIllinois Public Health Institute is introduced to Committee and beginsVisioning process.

    L. Call gave brief presentation of MAPP process.

    Visioning Process:

    L. Call asked each Committee member to write a value on a stickynote and post it on the wall. These values should reflect what is

    important to the Committee and guide the MAPP process.

    Collaboratively, the Committee reviewed 50+ values and groupedthe values together into similar categories.

    In pairs, Committee members selected a category and created avalue statement. As a group, Committee reviewed each value

    statement and revised and condensed statements together to create

    comprehensive value statements as well as a mission statement.

    Mission Statement: Our mission is to develop an actionable, realistichealth plan based on valid information that results in positive health

    outcomes for Clay County.

    Value Statements:

    We are accountable to each other and the community by beingethical, honest and respectful while maintaining confidentiality.

    We will strive to work together to achieve the most effectiveprograms, based on our available resources that produce the greatest

    outcome.

    We strive to impact the communitys well-being through theachievement of valid and sustainable results. We value open, transparent communication that includes diverse

    ideas, shared in a manner that is easily understood.

    We seek to create a health plan that is both inclusive of all ClayCounty residents

    We will strive to monitor the communitys health status through adata-driven process that uses accurate and updated information.

    Committee will revisit creation ofVision Statement after conductingthe four MAPP assessments.

    Committee will review valuestatements and suggest changes ifnecessary. Committee will adoptfinalized value statements at nextmeeting.

    Community Health

    Status AssessmentG. Paule Committees next meeting is scheduled for Wednesday, December 7

    th from12 pm1 pm. We will conduct the Community Health Status Assessmentduring this meeting. G. Paule will prepare a Community Health StatusReport, which will contain data, compiled from multiple sources, on ClayCountys current health status.

    Committee members will reviewCommunity Health Status Reportbefore next meeting. G. Paule willemail this report one week prior.
