    This application document checklist details the information and supporting documents(personal, health and character) required to assist with the lodgement of a completeapplication with the department. You may also need to provide additional information anddocumentation after you have made your application if the department requires it.

    Forms, fees and charges

    Form 47CHApplication for migration to Australia by a child

    A completed form 47CH including details of all dependents. The form must be signedin all the relevant places and each person included in the form, who is aged 18 yearsor over, has signed the Australian values statement.

    Personal documents sponsor

    Evidence that the sponsor is; an Australian citizen, the holder of an Australianpermanent visa, or an eligible New Zealand citizen, such as:

    certified copy of birth certificate Australian passport or foreign passport containing evidence of permanent

    residence

    for New Zealand citizens; evidence of length of residence in Australia and ofcontinuing links with Australia.

    Photocopies of one of the following documents to show the sponsors employmentduring the last two years:

    Australian income and/or overseas tax assessment notice a letter from the sponsors employer confirming length of employment and

    annual salary

    payslips if the sponsor is self-employed or self-funded from other sources, business

    documents or a letter from your sponsors accountant.

    If applicable: a statement from the sponsor regarding any other person for whom thesponsor has child support payments and/or an Assurance of Support. This statementmust:

    detail the sponsors relationship with this other person or persons specify the dates of lodgement of any sponsorship or nominations (including

    any current sponsorship) or Assurances of Support

    specify the amount and frequency of child support payments.

    If the sponsored child is under 18 years of age, the sponsor must provide:

    a) an Australian National Police Check, if the sponsor has spent a total of 12months or more in Australia since turning sixteen years of age; or

    b) if not required to provide an Australian National Police Check, police certificatesfrom each country in which the sponsor has spent a total of 12 months or morein the last 10 years since turning sixteen years of age.

    Personal documents - applicant

    If the child is applying in Australia, certified copies of the passport or traveldocuments the child used to enter Australia, and any passports held since then.

    Two recent passport size photographs (45mm x 35mm) of the child (four photos ifhealth examinations have not been completed). These should be of the head andshoulders only, and should show the child facing the camera and against a plainbackground. Print the name of the child on the back of each photograph.

    A certified copy of the childs birth registration showing both parents names. If a birthcertificate is unavailable, a certified copy of the identification pages of at least one ofthe following documents must be provided:

    passport family book showing both parents names identification document issued by the government document issued by a court that verifies the childs identity.

    If the childs name or anyone included in the childs application has been changed (forexample by marriage or deed poll) a certified copy of evidence of the name change.

    If the child is 18 years of age or over and is a full time student, evidence of theirenrolment and active participation in a post-secondary course of study.

    If the child is 18 years of age or over and is not a full time student, an explanation ofwhy they are not studying.

    Note: If there has been a gap of over six months in study between finishing the finalyear of secondary school and commencing further studies, please provide astatement explaining this timeframe.

    If the child has served in the armed forces of any country, certified copies of militaryservice records, or discharge papers.

    If the child was adopted before the sponsor became an Australian citizen, apermanent resident or an eligible New Zealand citizen, certified copies of theadoption papers or the Adoption Compliance Certificate.

    If the child was adopted with the support of an Australian state/territory adoptionauthority, certified copies of relevant adoption papers.

    If the child has been adopted by expatriate adoptive parents without the support of anAustralian state/territory adoption authority, provide a written statement outlining thereasons for the adoptive parents residence overseas.

    If the child is a step-child of the sponsor, the step-child must be under 18 years of ageand the following must be provided:

    evidence that the childs parent is the former partner of the sponsoring step-parent, and

    evidence of the childs parentage, and evidence that the step-parent has legal responsibility to care for the child.

    If the child is 18 years of age or over and has a disability, evidence from a qualifiedmedical practitioner that the child has total or partial loss of bodily or mental functionsand this stops them working.

    If the child is 18 years of age or over, evidence that they are financially dependent ontheir sponsor for the basic needs of food, shelter and clothing, and how long thissupport has been provided. Evidence may include bank statements, money transfers,rent receipts etc.

    If the child is under 18 years of age, one of the following must be provided:

    evidence that the law of the country in which the child resides supports thechilds migration to Australia (eg. a court order)

    a statutory declaration from each person with a legal responsibility to the childstating that they have no objection to the childs migrationNote: where a non-migrating parent/ guardian has the legal right to determinewhere the child can live, please provide a statutory declaration from the non-migrating parent/ guardian granting permission for the child to migrate toAustralia, together with a certified copy of the non-migrating parents/guardians identification (example, passport or drivers licence)

    an Australian court order supporting the childs migration.
