• It must reproduce in extenso the official text elaborated by the European commission, the Council of Europe and UNESCO :

“ This diploma supplement follows the model developped by…an explanation should give the reason why.”

Information on the holderInformation on the holder

• Family Name

• Given names (all)

• Date of Birth

• Student identification number

Do not indicate sex and nationality because it might be considered as discriminatory.


Information on qualificationInformation on qualification

• Precise name of the qualification (in the home language) and title confered if guaranteed by law (no abreviations)

• Main fields of study for the qualification (not for the Institution)

• Name and status of the awarding institution (with geographic localization)

• Name and status of institution administering studies

• Language(s) of instruction and examination


Information on the level of qualificationInformation on the level of qualification

• Give the exact level respect to the national certification framework and the EQF. Graduate/postgraduate ; 1st, 2nd, 3rd cycle level 8, ecc + information about the level descriptors.

• Official length of programme : number of semesters, credits, full or half time study…

• Access requirements


Information on contents and resultsInformation on contents and results

• Mode of study : full or part time presence; e learning ; continuing or training education ; block-release training.

• Programme requirements : in terms of profile, learning outcomes and expected competences

• Programme details and individual grades/marks/credits obtained ; give the complete list of modules or course units , and the transcript of records.

• Applicable local grading scheme and translation to ECTS grade.

• Overall classification of the qualification : only if there are distinction/honours, ranking

• Access to further studies ; is it a final or intermediate diploma ?

• Professional status :important if the diploma gives access to a regulated profession ; it is possible to give the list of the different jobs
