• There is no such thing as a care record• All records of care aren’t electronic• Electronic records of care are not routinely
shared between clinicians• You have the right to see your care
record/s*• Repetition, repetition, repetition!!
Where we are now?
• Suffolk Shared Care Record has started!• Paper mountains are being digitised• More than 5000 shared records were
viewed in urgent care in Suffolk last month• 97% of patients can request digital access
to their GP record today• Repetition still exists
Fast forward a year….
• Suffolk Shared Care Record will be used routinely across health and social care
• Every clinician will be able to access relevant information for the patient in front of them
• Repetition reduced
So what?
• You will be able to access your full care record
….which means you are better informed
….which means you can look after yourself better
• You can choose to allow your carer to access your record
• You / carer can communicate with your clinician through the record
