ELIGIBILITY Producer members meeting a minimum score for the criteria detailed in this award packet are eligible for the award. Entries may be submitted for each site at which the company operated in 2019 (e.g. portable or stationary plant). For example, a company with five (5) sites may submit an application for each site. CRITERIA The application packet consists of a checklist which identifies the core environmental aspects that impact our industry. In order to demonstrate that a site satisfies a specific environmental aspect, a form of documentation is required. Sites that meet the point scale requirements will receive an award. SUBMITTAL There is no set format for submitting your award application; however, the MLPA recommends that you assemble your award application and submit it electronically in a Word or PDF file to the MLPA office ([email protected]). Most criteria may be satisfied with a written description of activities and supporting documents and photos, where available or required on the checklist. The application can also be submitted via paper copy by mailing to the MLPA office. Note that the checklist has a "Documentation Required" column which specifies the supporting documents needed to complete your application. The application packet must be received at the MLPA office no later than December 31, 2019 to be considered for the 2019 awards. JUDGING A panel of judges made up of members of the MLPA Environmental Committee will evaluate all applications. At least two (2) judges must agree on the merits of the entry for it to be successful. Each criterion has a designated point value. There is a total of 38 points available. A company site must attain a total of 10 to 19 points to qualify for a Silver Environmental Award; 20 points or more to qualify for a Gold Environmental Award. Supporting Documentation Instructions

ENVIRONMENTAL ASPECTS 1. Does your company or site have a written Environmental Policy? [2 pts.] Provide a

copy of the policy. SWPP and SPCC plans may not be submitted for the awarding of points. The policy may be a statement on your website or published literature. Written policies should be signed by senior-level management and dated.

2. Does your site conduct an annual environmental audit or inspection? [3 pts.] Briefly describe the procedure for conducting an audit or inspection. An environmental management system or continuous tracking system is eligible.

3. Do your site’s employees receive annual environmental training? [3 pts.] Briefly describe training program, including substance/agenda, frequency of training and if the training is administered by internal or external personnel. Note: this is for training administered in addition to or outside of SPCC/SWPP plan requirements. SPCC/SWPPP training is required; these points are awarded for training EXCEEDING the required training.

4. Does your site have an air permit issued by the Missouri Department of Natural Resources (MDNR) or a local, delegated permitting agency (Kansas City, St. Louis County, or City of Springfield) or an agency-issued exemption from permitting? [1 pt.] Provide a copy of the permit cover page with permit ID number only, or agency-issued exemption. Do not provide a copy of the entire permit. The permit may cover other support activities such as an asphalt plant or concrete batch plant but must be specific to quarry operations.

5. Does your site have an employee certified in EPA Method 9 - Visible Emissions Evaluation? [1 or 2 pts.] Provide a copy of the employee's current certification and anticipated schedule for re-certification. 1 point will be awarded for an employee located at a site other than this site. 2 points will be awarded for an employee located at this site.

6. What form(s) of dust control does your site utilize (i.e. wheel wash)? [3 pts.] Describe site-specific dust control measures and/or inventory of current dust control equipment and best management practices (BMPs) in use at the site. Examples of dust control measures include wheel wash, water truck, sweeper, or processing plant dust sprays. Provide a typical photograph for each dust control measure used at the site.

7. Does your site have the appropriate National Pollution Discharge Elimination System (NPDES) Water permits, if applicable? [1 pt.] Provide a copy of the permit cover page with permit ID number only. Do not provide copy of entire permit. The permit may cover other support activities such as an asphalt plant or concrete batch plant, but must be specific to quarry operations.

8. Does your site have a Storm Water Pollution Prevention Plan (SWPPP), if required? [1 pt.] Provide a copy of the cover page and a copy of a recent signed/completed internal inspection form. Do not provide the entire plan document. Please note that the SWPPP must be current (within the term of the current NPDES water permit).

9. Does your site require and have a Spill Prevention, Control, and Countermeasures Plan (SPCC), if applicable? If a SPCC Plan is not required for your site, does your site have an emergency spill response program/plan in the SWPPP or as a stand-alone document? [1 pt.] Provide a copy of the cover page and the signed and dated certification page of the SPCC plan. Do not provide the entire plan document. Please note that the SPCC plan must be current (within the last 5 years). If an SPCC is not required, provide a brief description of the spill response program, including training procedures and personnel responsibilities.

10. Does your site have a mine plan on file with the MDNR Land Reclamation Program,

if applicable, or do you have an internal mine plan? [1 pt.] If you pay an annual permit fee to the MDNR and maintain bonding specifically for the purpose of land reclamation, you will receive 1 point. Provide a copy of the current permit or documentation of payment of fee (i.e. invoice, etc.) and permit renewal application. If you are not required by MDNR to file a mine plan but have a company internal mine plan, answer yes.

11. Does your site submit an annual Community Right-to-Know EPCRA Tier II report? [1 pt.] If you submit an annual inventory report of hazardous chemicals to the Missouri Emergency Response Commission (MERC), your LEPC and local fire department, you will receive 1 point. Provide first page of submitted report or proof of payment.

12. Does your site submit annual Major Water Users reports for surface or ground-

water use and/or pit dewatering? [2 pts.] If you report water use or diversion from any stream, river, lake, well, spring, or other water source, you will receive 2 points. Provide a copy of the most recent report.

13. Does your site have a formal complaint response program? [2 pts.]

Describe the components of the complaint response program, including procedures followed to respond to a complaint, the process used to follow up on complaints, roles and responsibilities for personnel who respond to complaints, and

14. Does your site have an office recycle program (i.e. paper, glass, plastic, cans) or shop recycling program (i.e. tires, metal, used oil)? [2 pts.] Briefly describe the site's recycle program and what materials are recycled.

15. Has your site received a Letter of Warning (LOW), Notice of Violation (NOV), or

Notice of Excess Emissions (NOEE), or entered into a compliance agreement during the past calendar year? [up to 3 pts.] This pertains to any local, state, or federal agency-issued enforcement action. Examples could include: MDNR, City of Springfield, St. Louis County Department of Health, Health Department of Kansas City, or U.S. Environmental Protection Agency. If your response is “No”, 3 points will be awarded. If your site received a LOW and no enforcement action took place, you receive 2 points. If your site received a NOV or NOEE but was returned to compliance with no fine or compliance agreement, you receive 1 point. If a compliance agreement or fine was issued, no points will be awarded. Provide a description of the circumstances and the resulting outcome for a determination of points to be awarded.

16. Does your site conduct recycling of construction and demolition waste? [2 pts.] Describe how your site accepts, sorts, processes, and returns recyclable materials to the economic mainstream. Some examples of recyclable materials include concrete, asphalt, and shingles.

17. Has your site received an environmental award, certification, or other form of recognition in the past year? [1 pt.] Provide a copy and brief description of any awards, certifications, or forms of recognition your site has received from the local community, state, or federal level (MLPA awards do not count). 1 point will be awarded for an award from other local, regional or national associations or agencies.

18. Has your site completed any engineering improvements, or implemented any behavioral changes that reduce energy consumption in the past year? [2 pts.] Provide clear evidence of these improvements or behavioral changes (i.e., before and after pictures, receipts for LED bulbs or other equipment purchases, write-up documenting the behavioral changes and agreements by employees, energy reduction calculations, energy reduction awards, etc.).

19. Has your site engaged in community outreach or open house events to educate the public on environmental impacts of mining? [3 pts] Provide a description of the event(s).
