During the first quarter of FY 2016, the Jackson Bond Citizens’ Advisory Committee met once to review the sole project presented by staff. Financial Update

The first tranche of bonds for the Jackson Miracle-Building Bond Program was approved for sale by the Board of County Commissioners on June 3, 2014. Staff from Jackson and the County worked jointly to secure the sale of these bonds on January 21, 2015. The par amount of this 2014 issuance was $94,915,000, with a final maturity on July 1, 2044, and a true interest cost of 3.45 percent. Proceeds from this sale are now being held in escrow by the County and are available for immediate draw-down by Jackson pursuant to appropriately documented requests for reimbursement.

Reimbursements received during the report timeframe totaled $12,574,799, bringing to $61,893,706 the total reimbursement received since the bond program began.

Small-Business Enterprise Contracting

From the beginning of the Miracle-Building Bond program through the end of this reporting period on Dec. 31, 2015, 14 projects have been awarded totaling $4,874,212. Eight of those projects, or 53 percent, were awarded to prime contractors who are certified small business enterprises (SBEs). Those eight contracts had a total value of $4,499,595, or 92.3 percent of the combined value of all bond-funded contracts. It is important to note that some subcontractors on those projects are not registered SBEs.

The names and demographics of the prime and sub-contractors for all 14 projects are detailed below. Future quarterly reports will provide the same level of detail for new contracts awarded during the reporting period.

These totals do not include bond-funded expenditures on non-construction items such as equipment, technology, and the land in the City of Doral for Jackson West. The totals also do not include projects approved by the Trust Board that have not yet completed the procurement process.

Status of Major Projects

Rehabilitation Center

The rehabilitation center is planned as a state-of-the-art facility to unite the clinical work now done at Jackson Rehabilitation Hospital with the research performed by the University of Miami and the development resources of The Miami Project to Cure Paralysis.

The project is currently in the design phase, with a committee composed of representatives from all stakeholders working with HDR, Inc., which has been contracted to design the facility. The space program has been finalized, with theoretical stacking and floor plans showing the feasibility of aligning the programs within the projected space. These do not reflect final architectural designs. Samples of those space utilizations and updated artist renderings of the facility are shown below.

Jackson West

The vision for Jackson West now calls for the addition of a 100-bed community hospital, which would supplement the previously announced outpatient facilities: a free-standing emergency room for adults and children, a pediatric specialty outpatient center, physician offices, diagnostics, imaging, and other ambulatory programs.

In December 2015, Jackson received approval from the Florida Agency for Healthcare Administration to open the inpatient hospital, a decision that is still subject to further legal and administrative appeals. The approval allows Jackson to review its first-phase facilities plan for Jackson West in terms of business strategy and community needs. The program architect will subsequently adapt those conclusions into space plans, a project scope, and design standards, all of which will be converted to design documents by the project architect. Procurement for the project architect was completed in February 2016, details of which will be provided in the subsequent quarterly report covering that period.

Designs have been completed for the first pair of urgent care centers, located in North Miami and Country Walk, and permitting review was underway during the reporting period. Construction began shortly thereafter and will be addressed in the next quarterly report, with ribbon-cuttings scheduled for summer 2016.

The second pair of urgent care centers, located in South Beach and in Palmetto Bay, were in the design phase during the reporting period, with target opening dates in fall 2016.

The below design is an artist’s rendering of the standard appearance being planned for all urgent care locations:

In order to keep pace with the fast-changing world of healthcare, we have avoided creating strict rules or timelines for identifying urgent-care locations. In general, we pursue sites in neighborhoods not already being served by urgent-care facilities or with too much demand to be met by existing facilities. Moreover, we are focused on areas that lack access to the existing Jackson network or that need a non-emergency entry point into the Jackson system. Of course we also demand competitive rental or purchase prices and high-quality facilities that will attract the full range of patients.

Additional Project Approvals

One additional project was approved during this quarter. The project approval forms, which include overviews and dollar allocations, is attached to this report.

Refurbishment of Jackson South Community Hospital Operating Rooms 1 and 2

This $1.8 million project was approved to make upgrades necessary to reactivate two decommissioned operating rooms at Jackson South Community Hospital. The work included mechanical, electrical, plumbing, equipment, and finishes. Additional operating-room capacity was necessary to address both the growing surgical volumes at Jackson South and the potential to open at trauma center, which requires having an operating room available at all times. The refurbished operating rooms are also closer to the emergency department, reducing transport time and infection risks associated with emergency surgery. The additional capacity likewise allows Jackson South to consider opening an ambulatory surgery center, which would increase care access in the South Miami-Dade community while addressing Jackson’s strategic goal of investing in financially sustainable service lines. The project was completed in February 2016, and photos of the completed project will be provided in the next quarterly update.

Additional Project Updates

Both the Public Health Trust Board of Trustees and the Citizens’ Advisory Committee were briefed during the reporting period about the contracting strategy for the remainder of the Jackson Miracle-Building Program. Under the supervision of Jackson staff and a program-management team from URS (now known as AECOM), Jackson initiated a public process to retain architect/engineering firms and contractors specific to each major campus: one each for Jackson West, Jackson North Medical Center, and Jackson South Community Hospital, plus two for the Jackson Memorial Medical Center, one of which will focus on floor modernizations and the other on new construction.

The completion of the architect/engineering contracts was approved in February 2016 and will be detailed in the next quarterly report. The construction contracts are expected to be submitted for approval this spring.

AECOM is also continuing to lead a master-plan that is expected to produce a system-wide scope, timeline, and budget by summer 2016.

1. Nanda Gowda – Miami-Dade Resident Varian Medical Systems District Sales Manager Oncology Systems 2250 Newmarket Parkway Suite 120 Marietta, GA 30067

Mr. Gowda stated that Varian Medical Systems was awarded a contract for services with Jackson Health System (JHS) and look forward to doing business with JHS. Mr. Gowda further stated that his presence at meeting was to hear information regarding the Radiation Therapy Cancer Center Project.

4. Purchasing and Facilities Subcommittee Update

Ms. Baker reported that the Purchasing and Facilities Subcommittee met on August 31, 2015. The focus of the facilities portion of the Subcommittee meeting was the three (3) projects for funding under the Jackson Miracle- Building Bond program. All of the projects were reviewed and approved by the staff, Capital Steering Committee, Purchasing and Facilities Subcommittee, and Fiscal Committee with a favorable recommendation to the JHS GOB Citizens’ Advisory Committee.

5. Miracle-Building Bond Quarterly Report April to June 2015 Matthew Pinzur, Associate Vice President, Communications and Outreach Division reported that virtually the report is self-explanatory and includes some of the additional features and enhancements requested by the Committee. Consistent with the ordinance that created the Committee, Mr. Pinzur recommended that the Committee approve the report with a favorable recommendation the Mayor and Board of County Commissioners. A detailed copy of the report was included with the agenda. Mr. Freidman questioned when it is expected for reimbursement of bond funds for constructions projects. Mark T. Knight, Executive Vice President and Chief Financial Officer stated that the first reimbursement was received on July 27, 2015 in the amount of $14 million for expenditures approved by the Committee. Mr. Knight reminded everyone that a retro reimbursement request was made earlier during the year totaling $11 million, and additional funds were received during the month of August 2015. Mr. Freidman questioned if reimbursement of bond funds will periodically occur throughout the year. Mr. Knight stated that each month bond proceeds will flow through the P&L statement and reflected in the quarterly report.

Miracle-Building Bond Quarterly Report April to June 2015 cont… For the purpose of monitoring the bond funds being utilized for small businesses Mr. Frazier questioned if there is an up-to-date report (chart presentation) that shows the utilization and expenditures associated with small businesses. Mr. Pinzur stated that the Procurement Department can generate a report showing up-to-date information regarding the utilization and expenditures associated with small businesses and include in the next Miracle-Building Bond Quarterly Report. Following the discussion Mr. Gomez requested a motion to accept the report with a favorable recommendation to the Mayor and Board of County Commissioners. Ronald E. Frazier moved approval; seconded by Sigfredo Delgado, and carried without dissent.

6. Miracle-Building Bond Projects Isa Nunez, Vice President, Facilities, Design, and Construction presented a summary describing in detail the three (3) projects. The total fiscal impact for the 3 projects is $3,951,788. A detailed summary of each project was included with the agenda. A summary of the 3 projects was presented as follows:

1. Jackson South Community Hospital – 40-Year Building Recertification

Scope and Description: The scope encompasses required construction for correction of all the identified deficiencies in the report performed by JSK Architectural Group, which is an assessment of all existing deficiencies required for 40-year recertification. Total Project Cost and Bond Allocation: Miracle Bonds: $572,000. Statement of Need: The 40-year building recertification for the original buildings at Jackson South is due in the next six months. As assessment of the buildings has already been completed that identifies the structural and electrical repairs that need to be addressed. These repairs will ensure structural and electrical integrity of the existing building for Miami-Dade County recertification; Projected Project Schedule: Permitting/AHCA: 1 month; Contractor Procurement: 1 month; Construction Phase: 6 months

2. South Beach Urgent Care Center

Scope and Description: Jackson Health System will design and provide construction services to build out 3,224 square feet of retail space located at 615 5th Street in Miami Beach. Total Project Cost and Bond Allocation: Miracle Bonds: $1,523,328; Renovations and Upgrades: $1,523,328. Projected Project Schedule: Design Procurement: 1 month; Design Phase: 3 months; Permitting/AHCA: 2 months; Contractor Procurement: 2 months (concurrent); Construction Phase: 6 months. Statement of Need: This initiative is part of Jackson’s overall retail strategy. The development of access points in various communities for the residents of Miami-Dade County to receive Jackson healthcare is an integral part of the long-term strategic plan.

Clock Tower Plaza Urgent Care Center (UCC) Scope and Description: Jackson Health System will design and provide construction services to build out 4,968 square feet of retail space located at 18910 S. Dixie Highway in Miami. Total Project Cost and Bond Allocation: Miracle Bonds: $1,856,460; Renovations and Upgrades: $1,856,460. Projected Project Schedule: Design Procurement: 1 month; Design Phase: 3 months; Permitting/AHCA: 2 months; Contractor Procurement: 2 Months (concurrent); Construction Phase: 6 months. Statement of Need: This initiative is part of Jackson’s overall retail strategy. The development of access points in various communities for the residents of Miami-Dade County to receive Jackson healthcare is an integral part of the long-term strategic plan. With there being no significant discussion or issues of concern raised regarding the 3 proposed projects Mr. Gomez requested a motion for the items.

• Procurement Manager Selection

Mr. Gomez questioned the status of the selection process for a Program Manager. Mr. Migoya stated that the contract for a Program Manager Owner’s Representatives Services for JHS Capital Plan was awarded to URS Corporation Southern. The Senior Program Manager and Program Executive who will oversee JHS Capital Plan Projects have over 20 years of experience working with hospital clients locally and nationwide. There has been a lot of conversation with the Senior Program Manager and Program Executive regarding how to start construction and how to best expedite the construction project processes. The Committee will be kept informed on a regular basis regarding the processes necessary to begin and expedite construction projects. The Committee and members of the PHT Board of Trustees will be introduced to the Senior Program Manager and Program Executive in the month of September or October 2015. Mr. Frazier questioned if the Senior Program Manager and Program Executive will present the Committee with a CPM schedule to be able to see how the construction projects are being monitored. Mr. Migoya stated overtime yes.

• Specific of Need Application After completing a detailed needs analysis Mr. Migoya stated that JHS is putting together a Specific of Need Application for a 100-bed hospital at Jackson West. Based on the results of the needs analysis it was evident that it was necessary to develop a small level acute care facility. The amount of funds needed to develop the facility exceeds what was originally projected from the bond funds, however JHS is reviewing how best to generate funds through a retention of capital.

• Level II Trauma Center at Jackson South Community Hospital (JSCH) Mr. Gomez questioned the status of developing a Level II Trauma Center at JSCH. Mr. Migoya stated that appeals process to develop a Level II Trauma Center at JSCH is ongoing. It is anticipated that the appeal will be heard in court in early October 2015. The Committee will be kept informed as the appeals process continues.

• National Consultant to Identify Opportunities for SBE Firms Mr. Frazier questioned the status of the national consultant hired to identify opportunities for SBE firms. For the purposes of understanding how best to move forward with identifying opportunities for SBE services Mr. Migoya stated that a meeting will be scheduled to meet with representatives from the County Attorney’s Office to review Miami-Dade County restrictions associated with SBE firms and report back to the Committee in the month of September or October 2015.

4. Purchasing and Facilities Subcommittee Update Martha Baker, RN, member of the JHS GOB Citizens’ Advisory Committee and Ex-officio Member of the Purchasing and Facilities Subcommittee reported that the Subcommittee was presented with detailed information regarding the Jackson South Community Hospital-Refurbishing of Operating Rooms 1 and 2 Miracle-Building Bond Project for November 2015. With there being no significant issues regarding the project the Subcommittee agreed to forward the item with a favorable recommendation to the Fiscal Committee and JHS GOB Citizens’ Advisory Committee.

7. Miracle-Building Bond Project David Clark, Corporate Director, Capital Projects presented the following single Miracle-Building Bond Project for the month of November 2015 the JHS GOB Citizens’ Advisory Committee’s consideration and favorable recommendation to the PHT Board of Trustees:

1. Bond Project Name: Jackson South Community Hospital-Refurbishing of Operating Rooms 1 and 2;

Scope and Design: Professional design and construction services for the refurbishment or Operating Rooms 1 and 2 for trauma services. The scope of the project involves the recommissioning of two old operating room suites. The work will include the upgrade of interior finishes throughout the surgical suites and mechanical, electrical, and plumbing certifications. This will also include equipment purchases or upgrades as required, such as surgical lights. Total Project Cost and Bond Allocation: Renovation and Upgrades: $1,814,009, Other: $49,250; Projected Project Schedule: Design Procurement: Zero months, Design Phase: 1 month; Permitting/AHCA: Zero months: Contractor Procurement: 1 month, and Construction Phase: 5 months.

Following the presentation Mr. Gomez requested a motion for the item. Ronald E. Frazier moved approval; seconded by Kareem T. Brantley, and carried without dissent.

5. SBE Construction Matchmaker Amber Lawhorn, Manager, Small Business Enterprise (SBE) Program reported that on September 30, 2015 Jackson Health System (JHS) hosted a Small Business Enterprise Matchmaker Workshop (Workshop). The purpose of the Workshop was to educate construction firms how to participate in future JHS projects, and meet and greet between SBE and Job Order Contract general contractors. Over 600 Miami-Dade County Certified SBE in construction were invited. Out of the over 600 invites 70 registered and 41 attended representing 36 firms. In addition to the general contractors that attended eight (8) JHS job contractors with active contracts attended, as well as JHS Procurement Management Team, Facilities Design and Construction Team and Engineering and Plant Operations Team. URS representatives, URS SBE Coordinator, and Miami-Dade County SBE representatives also attended. Ms. Lawhorn stated that the Workshop was a success and anticipates an increase in SBE participation in the coming year. Following the SBE Construction Matchmaker update Mr. Frazier expressed thanks for the event. Mr. Frazier stated that he learned a great deal about the SBE process and procedures as well as how to utilize services through the Job Ordering Contracting (JOC) Program. Also Mr. Frazier spoke about including guidelines for 25% use of diversity in the RFP’s for JOC and other construction firms.

Program Manager Presentation Tom Koulouris, Senior Project Manager presented an update regarding the approach to the Miracle-Building Program: Strategy, Controls and Timeline which included completing in 5 to 7 years Small Projects (Jackson South, Urgent Care Centers and Renovations); Medium Projects (Garages, Floor Modifications, Site Work and Jackson North); and Major Projects (Rehab Hospital, Jackson West, and ER/ICU/Transplant Tower). Koulouris presented an overview of a Speed-to-Market concept which is an institutional approach that yields a faster project at a lower cost; is data driven and controlled by the owner with positive results; and require all scope changes to be approved by the Chief Executive Officer. Phase 1 of the Speed-to Market concept showed phases of the nearly completed business plan as well as a diagram highlighting the various paths necessary to timely complete the business plan. It is anticipated that the five (5) architectural firms selected will begin the design phases at the end of January or beginning of February 2016 and a month later the six (6) construction managers selected will begin. With an emphasis on diversity Mr. Koulouris described in detail the programs URS has developed to increase SBE participation that includes an Architectural Program, Construction Manager Program, and a Mentorship Program. As part of the update presentation was an overview of the procurement strategies that included a diagram showing the RFQ processes, description and responsibilities of bundled construction managers, descriptions showing how the architects, construction managers, and owner stakeholders will be managed, diagram showing program controls (owners risk versus cost), outline of the cost and schedule control, timing schedule (Validation – December 2016, Award Architects – January 2017, and Projects Launch – February 2017), and overview of the strategy benefits. Mr. Frazier requested that members of the JHS GOB Citizens’ Advisory Committee receive a copy of the presentation by Mr. Koulouris including his comments having to do with diversity opportunities particularly as it relates to the language in an RFP referring to diversity.
