WHAT IS CHIRO-TEL?

Chiro-T el is a chiropractic information and referral service that is free to the public.

The general public is provided with a toll free telephone number to dial 24 hours a day, 7 days a week.

A tape library of chiropractic subjects of general interest has been created and is listed by topic and tape number. The caller dials the Chim-Tel number and asks for his tape selection by number. Our operator then activates the Chiro-Tel unit. This unique multi-channel information retrieval system combines modern telephonic electronics with the latest audio playback technology and is capable of receiving multiple calls simultaneously.

The prerecorded message on the magnetic tape cartridge that the caller hears lasts for approximately two minutes and has been carefully prepared by chiropractic experts. The tapes are of the highest broadcast quality and are studio recorded by professional announcers.

If the caller wishes more information on a particular health problem or would like a doctor referred to them, they simply redial the toll free number and leave their name and address with the Chiro-Tel Operator. The following day the Chiroprac­tic Educational Society will forward the information requested in the form of a personal letter. As a member of Chiro-Tel, the doctor is automatically enrolled as a member of the Chiroprac­tic Educational Society.

HERE'S HOW CHIRO-TEL WORKS

Through the use of toll free 800 numbers, Chiro-T el now makes available, on a 24 hour basis, a free tape library on chiropractic health care, providing a better understanding of what chiropractic health care can afford your present patients and future patients as well.

Your patient simply dials the toll free 800 number from the con­venience of his own home and requests by number the tape he wishes to hear. Our operator will insert the tape cartridge in a special broadcast quality playback unit, allowing the patient to hear the chiropractic message he has selected.

Personalized tape library cards, brochures, directuries and other tools, each displaying the Chiro -Tel tape library, will be provided to allow his patients to promote the use of Chiro -T el, plus giving them the greatest referral tool that has ever been available to the patients of chiropractic.

Many doctors use their promotional materials by simply reviewing the library in the presence of their patients and asking if their friends or spouses suffer from any of the ailments covered by the tape selections. If so, the doctor suggests to the patient that he request his friend or spouse listen to the chosen tape message and afterwards, perhaps, come in for a check-up.

The objective of this program is to: (1} introduce Chiro-Tel to the genera ,! public; (2) provide an educational atmosphere for the genera,! public; (3) cr-eat·e a "good news" armospher,e for the chiropractic profession.

THE PROGRAM APPROACH

The concept to be used wil'I be one of a professional service providing public .information. Some advertising will be done as weH as publicity and public r,elations. All advertising and public relations will be controHed in a sophisticated manner aimed at ,educating the general public to the benefits of chiropractic.

Chiro-T el Tape 111 - The Migraine Headache

Migraine headaches can be one of the most frustrating and excruciating pains known to man. Many not only have a headache when they get a 'migraine' but also have nausea, dizziness, and blurred vision. The medical approach to this difficult disorder is primarily varying doses of pain killing drugs, but historically they are rather ineffective in a true migraine headache. Unfortunately, many apply the old axiom, "if a little is good, a lot is much better," in a desperate attempt to relieve the blinding pain that seems to get worse by the minute.

The effect of blocking nerve flow, which these pain killers sometimes accomplish and sometimes relieve but temporarily, makes the patient feel 'dopey' and very sluggish for days. Perhaps the most serious consideration in such an approach to the problem is the danger of covering symptoms which may be warning signals of a more serious problem. There is always the possibility of kidney, liver or circulatory conditions which might be increasing in severity while you are treating the symptoms and ignoring the cause. Your chiropractor has found that a spinal examination usually reveals misaligned vertebra causing irritation of the upper neck nerves which lead to the head, in migraine conditions. Such irritation of the important cranial nerves can also produce the symptoms other than headache so often associated with migraine. A reflex action affects a bundle of nerve fibers which can affect the stomach, the heart, and other internal organs. This type of condition where nerves are irritated, is often seen by your chiropractor in his daily office practice. His expert examination will usually pinpont the misalignment and then he will scientifically correct the cause of the irritation by a scientific adjustment of the misaligned vertebra in your spine.

You can rest assured that chiropractic is the most logical and sane approach to migraine headaches because it corrects the cause instead of masking the symptoms.

WHAT DOES llT COST?

The Chiro-Tel Agreement is a contract that runs for a period of 12 months. Th.e contract is billed monthly and the first and last two months service fees are required to initiate the program. There is a one time only enrollment fee of $150 which covers the doctor's share of the system set-up and the tape library maintenance. All literature that is used in the office will be billed separately. Initially, the cost of the literature is $100.00 and the doctor reorders additional material as he finds it necessary. Materials will be sent out United Parcel Service, collect. An example of the cost of start-up on the program is as follows:

HOW TO JOIN CHIRO-TEL

It's simple! All that is required to join Chiro-Tel is to sign the agreement, attach a copy of your business card and send them to us along with your check as indicated above. Your monthly payments will start 30 days after your receipt of your literature and will continue for a period of 9 months. At the termination of the agreement the contract may be renewed.

TAPE INFORMATION CENTERS, INC. A CALIFORNIA CORPORATION, Al<A CHIRO-TEL, HEREINAFTER REFERRED TO AS "COMPANY ", AND

A LICENSED CHIROPRACTOR, HEREINAFTER REFERRED TO AS "DOCTOR"

I. CHIRO-TEL UNIT

COMPANY agrees to provide, on a 24 hour basis, multiple toll free telephone lines to provide adequate service (or existing subscribers. COMPANY will pay all costs relating to telephone lines, answering service, eqwpment, repair costs and general maintenance, public relations, National advertising, publicity and promotional efforts connected with the Chiro -Tel system and all COMPANY administrative costs, excluding the cost of local advertising.

The system will be provided with the existing chiropractic tape library which includes forty (40) tape messages dealing with various subjects and there will be no additional charge for additional tapes when they are added to the existing libraries.

2. MONTHLY FEES & TERM

DOCTOR shall pay to COMPANY the sum of $75.00 per month for a period of twelve (12) months. Ir no written notice of cancellation is given within sixty (60) days before the end of the period covered by this Agreement, the contract shall be automatically extended for a period of one (1) year. First and last two (2) months ' fees shall be paid to COMPANY by DOCTOR to activate this Agreement. A payment book will be provided by COMPANY to DOCTOR to assist DOCTOR in making the remaining monthly payments.

3. NON-RECURRING START UP FEES

DOCTOR agrees to pay COMPANY the sum of One Hundred Fifty& 00/ 00 ( 150.00) Dollars to cover the intitial cost of joining the Chiro-Tel Program. This fee includes the purchasing of the library, additional costs required to add DOCTOR to the system, and non-recurring costs, as well as membership in the Chiropractic Educational Society for a period of twelve (12) months.

4. ADDITIONAL MONTHLY TELEPHONE BILLING

The regular month ly service fee is based upon each state unit receiving and processing up to 3500 telephone calls per month. In the event the numb er of telephone calls received by said state unit exceeds the above number of calls per month . said add ition al calls will be billed al the rate of .3lit per call, said lee to be divided equally among the total number of doctors participating in that state.

5. INITI A L ORDERING OF CHIRO-TEL PERSONALIZED OFFICE MATERIALS

The cost of all materials used by said DOCTOR initially will be included in the start-up fees of either Plan A or Plan B.

6. REFERRAL SERVICE

Chiro-Tel has provided a National chiropractic referral service at no add itional expense. Only names, addresses and phone numbers will be given to prospective patients. No info rmatio n regarding techniques, schools or methods will be ex tende d by the Referr al Service.

7. LOCAL ADVERTISING

Each DOCTOR is au thorized to use, either singly or jointly with other sponsoring Chiro-Tel members, any of the advertisi ng materials provided; but, m no event will DOCTOR advertise the Chiro-Tel program with any other than approved ads provided by Chiro-Tel without pnor wntlen permission from COMPANY.

8. LOAD LIMITATIONS - TELEPHONE

Due to the limited numb er of WATS lines, it is extremely important that no doctor shall or cause to allow the public to rece ive, in the following medias. the Chiro-Tel main phone number: Television, radio or any other media that might effect a l,1rge. numb er of people calling in a concentrated period of time. Notice must be given to Chiro-Tel in writing ten ( 10) days prior to public release of any ads costing more than One Hundred and 00/0 0 ($100.00) Dollars. This is to enable COMPANY to afford time to advertis ing DOCTOR for access to clear telephone lines.

9. NOTICES

Any notice required here under shall be in writing and shall be delivered to the respective parties hereto by personal delivery thereof or by deposit in the United States mails as certified or registe red matters, postage prepaid, addressed lo the respective parties as follows, until otherwise adve rt ised in writing:

10. GOVERNING LAW

The laws of the State of California shall govern this contract and any interpretations or constructions thereof. Further, the place of performance and transaction of business shall be deemed to be in the County of Orange , State of California , and in the event of litigation, the exclusive venue and place of jurisdiction shall be the State of California, and more specifically, Orange County, California.

11. ATTORNEY'S FEES

If suit or arbitration is commenced concerning any portion of this agreement or if there is a dispute hereunder without the commencement of a suit or arbitration, the preva iling party shall receive from the other party its reasonable attorney's fees , costs and/or necessary disbursements whether or not such attorney's fees, costs or disbursements are incurred as a result of litigation, arbitration or by way of settlement or negotiation without litigation or arbitration and whether or not any litigation or arbitration is terminated by judgement or settlement.

12. It is understood by both parties that the materials covered by the Chiro-Tel copyright remain the property of COMPANY for the term of this Agreement and any doctor in default of this Agreement , upon written notification from COMPANY will return all unused materials and will discontinue the use of the Chiro-Tel phone numbers whether directly or indirectly.
