The proposal form would be filled by the field animator and the

demand draft of the premium amount favoring the assigned LIC office

would be taken by the field animator and submitted to the respective

SKDRDP Project office.

The Office staff at Project office would fill in the details in the software

and prepare the back up of the software.

The proposal forms, premium amount, software back up and the

printed statement of the software would be sent to the assigned LIC

office on the same day.

Any claim settlement would be considered from the day of the

registration in the software.
