version 3 May 2015• Already signed up 1,800 people in Highland as

test users• Target 13,200 by end May 2015• Target population over 50s, family, friends,

carers and service providers

Flourish The Flourish service will support theHealth and social care element of Livingit Up, some ideas so far;

• VC consultations, PC/Tablet• Mobile health

monitoring/Telehealth/Telecare • Digital postcards ( video clips)• Self management tools (shared)• Doctors appointments • Experience guides • Games/Health Apps • Access to trusted health information

Some services will be prototyped inFlourish - Autumn 2013.

• Betty, 58 yrs old with COPD, and recently moved to Mull a few months before being widowed, daughter lives in Edinburgh. Betty feels anxious and isolated and has had recent exacerbations of her COPD

LIU could help Betty• Connecting her to family and respiratory nurse• Access pulmonary rehab at home (VC)• Find out about local groups and activities, transport to help get her

out of the house• Find some volunteering opportunities• Find out more about her condition, share information about COPD

with her family• Develop an online self management plan that she shares with her

practice nurse and respiratory nurse • Monitor her condition remotely • Share her experiences about what has worked well for her• Access on line tools to help reduce her anxiety• Access a self management course/group by VC • Connect up to forums and interest groups, peer support, self

management courses• Find services to support SDS• Complete a telecare assessment

Recommendations

• Local support required to do local community engagement and promotion of Living it Up in communities to support recruitment

• Support required at all levels to get staff involved in LIU and signing up patients/clients people they work with

• Living it up needs to be embedded in local services
