Any clinical data exchange requires a comprehensive security infrastructure. The NextGate XDS offering supports security and access control through a number of industry standards, and provides both simple and complex access control so that you can manage sharing at specific levels. For example, providers, administrative staff and other care team members can trust document access through simple scenarios such as global level opt-in/opt-out consent. But more complex scenarios are also possible, including VIP patient access, consent override through purpose of use, access based on established relations, and explicit access grants by provider or organization. These access control policies can even be enforced on data access, such as querying for and retrieving documents.

CHALLENGE

The period following a hospital discharge is crucial for patients, as readmission rates, adverse events and medication errors can disrupt the care plan and hospital to home transition. One HIE was looking for a solution to support its care transitions initiative. Capturing and sharing discharge summaries would give their participating physicians relevant, timely medical data needed during patient follow-up visits. The HIE needed a solution that would standardize and store this discharge summary information.

Today's use of XDS Registry & Repository

Through XDS, this HIE can now facilitate the storage and exchange of a patient's discharge summary. Participating hospitals upload the patient's discharge summary that is associated with the correct patient through an EMPI patient matching solution. The PCP accesses the discharge summary and is prepared for care plan and medication discussions during the patient's follow-up appointment. In addition to HIE access, XDS Registry & Repository can publish patient discharge summaries to an IHE-compliant PHR. This capability allows patients to have access to their own discharge summaries, creating a medical history that can guide their path to wellness and better collaboration with physicians and other care team members. 

This HIE can confidently support smoother patient care transitions by storing and exchanging the patient's discharge summary. As a result, inpatient and outpatient physicians have a way to securely share timely medical data while patients and providers can collaborate at a higher level.
