be healthier, and wellness challenges to make getting healthy fun! Limeade also manages

our Be Fit for Bucks wellness incentive program.

WHAT IS LIMEADE?

Limeade is an employee engagement company known for its refreshing approach to driving productivity, well-being and health at work.

Important Note About ConfidentialityAll personal health information is completely confidential, as required by federal law. Personal Health Assessment (PHA) and biometric screening results are not shared with VF. Limeade will receive notifications that an individual completed an action, and the applicable points will be awarded on the Be Fit for Life website.

What is Be Fit for Bucks?Be Fit for Bucks is a points-based reward system through the Be Fit for Life website. When you earn points, you can earn medical plan premium discounts and gift cards.

Who is Eligible?All employees and/or spouses/domestic partners can participate in the healthy activities found on the Be Fit for Life website but only employees and/or spouses/domestic partners enrolled in a VF medical plan qualify to earn medical plan premium discounts and gift cards for completing the specific healthy activities highlighted in Be Fit for Bucks.

Welcome to Be Fit for Life

Be Fit for Bucks is part of Be Fit for Life, and allows you to earn rewards for participating in healthy activities. Each of the three Be Fit for Bucks levels has a goal for you to reach, and you can earn points by completing healthy activities like getting a biometric screening or speaking with a health coach. Once you reach the goal, you advance to the next Be Fit for Bucks level, and your points from the previous level roll over. Completing each level also helps you earn rewards like medical plan premium discounts and gift cards. For example:

Level 1 – Get Started Personal Health Assessment (PHA) = 450 pts Biometric Screening = 450 pts Complete a Be Fit for Life challenge = 100 pts

Total 1000 pts = $50 Visa gift card and $250/$500 off of your medical plan premiums in 2017

Level 2 – Get Fit

Level 1 = 1000 pts Annual Physical = 500 pts Health Coach = 500 pts Early PHA completion bonus (by March 1, 2016) = 250 pts Two Be Fit for Life challenges = 250 pts

Total 2500 pts = an additional $350/$700 off of your medical plan premiums in 2017

All activities can be completed at anytime during the year, but you must complete all activities in the Get Started level before earning rewards. Visa gift cards are taxable — the value of any gift card you and/or your spouse/domestic partner earn will be included as income on your W-2 and subject to Social Security and Medicare taxes, and federal and state income tax withholding.

Here’s how you can earn points and rewards at each level for completing healthy activities.

All employees and/or spouses/domestic partners of employees enrolled in a VF medical plan are eligible to participate and receive rewards.

VF understands that, in some instances, you may not be able to participate in various activities due to circumstances beyond your control. If it is not medically appropriate to participate in any of these activities, please contact a VF Health Coach at 1-855-376-3740 to determine the best course of action.

What you need to do:

• Schedule an appointment for an annual physical with your physician or ObGyn.

• Print out the form from the Biometric Screenings page under Be Fit for Life on www.vfcbenefits.com.

• Bring the form to your appointment.

• Have your physician complete the form.

• It is your responsibility, not your physician’s, to fax the completed form to the number on the Health Provider Screening Form.

Get a Biometric Screening A biometric screening is completed as part of your annual physical exam with your doctor and measures your body mass index (BMI), cholesterol, blood pressure and glucose levels to help you identify potential health risks. You will need to bring a Health Provider Screening Form to your doctor’s appointment. You can print out the form from the Biometric Screenings page under Be Fit for Life on www.vfcbenefits.com.

Healthy Body Mass Index (BMI) GuidelinesBMI measures your weight in relation to your height to calculate the percentage of body fat. A number of factors contribute to weight, including environment, family history and genetics, metabolism and behaviors or habits. Maintaining a healthy weight can help lower the risk of developing chronic conditions such as heart disease, high blood pressure, type 2 diabetes, breathing problems and certain cancers.

The National Institutes of Health has established the following guidelines to determine whether your BMI falls in an ideal range:• Below 18.5 — Underweight• 18.5 to 24.9 — Normal• Between 25 and 29.9 — Overweight• 30 or above — Obese

Healthy Metrics #1: You will be awarded points if your BMI is under 25 or you show a 5% improvement over your last screening.

Healthy Cholesterol Levels Cholesterol levels are measured in milligrams (mg) of cholesterol per deciliter (dL) of blood. The American Heart Association’s general guidelines to determine whether your total cholesterol falls in an ideal range are as follows:• Below 200 mg/dL — Desirable• 200 – 239 mg/dL — Borderline High• 240 mg/dL and above — High

Healthy Metrics #2: You will be awarded points if your total cholesterol level is below 200 mg/dL or you show a 5% improvement over your last screening.

HDL Cholesterol High-density lipoprotein (HDL) — or “good” cholesterol — helps protect you against coronary heart disease. For HDL cholesterol, higher levels are better.• Above 60 — Desirable• Above 40 — Moderate Risk• Below 40 — High Risk

Healthy Metrics #3: Points are awarded for HDL levels greater or equal to 60 or you show a 5% improvement over your last screening.

Blood PressureBlood pressure is one of the most important screenings because high blood pressure usually has no symptoms and can’t be detected without being measured. High blood pressure greatly increases your risk of heart disease and stroke.• Below 120/80 — Desirable• Below 160/100 — Moderate Risk• Over 160/100 — High Risk

Healthy Metrics #4: Points are awarded if your blood pressure is 120/80 (or below) or you show a 5% improvement over your last screening.

Blood GlucoseBlood glucose measures the amount of a type of sugar, called glucose, in your blood. High glucose levels can indicate your risk for diabetes. You will need to fast prior to your blood glucose test.• Below 99 — Desirable• Below 125 — Moderate Risk• Above 125 — High RiskHealthy Metrics #5: Points are awarded for fasting blood glucose levels below 99 or you show a 5% improvement over your last screening.

Participate in Be Fit for Life ChallengesA variety of personalized wellness challenges will be available based on your specific interests and health status. Make sure you visit www.befitforlife.limeade.com all year long to participate in new challenges as they become available. Here’s a few to look out for.

Participate in Health CoachingTalking to a Be Fit for Life Health Coach helps you make healthier decisions every day. Health Coaches encourage you to focus on the thought process behind the choices you make. Once you know why you fall into certain habits, your Health Coach can recommend healthier alternatives or tips for improving your health decisions.

Qualified Health Coaches can provide support for a number of health issues, including: • Nutrition and fitness• Weight management• Tobacco cessation• Prenatal/postnatal care

• Women’s health • Stress, sleep and energy balance• Work-life balance

If you and/or your spouse/domestic partner have not previously participated in Be Fit for Life, you both need to register on the Be Fit for Life website.

Register with Be Fit for Life: 1. Go to www.befitforlife.limeade.com.

2. Click Get started.

3. Input the requested information and click on Find My Account.

If your spouse/domestic
