Weddings Ceremony Questionnaire This questionnaire is intended only to ensure that I create and execute the wedding ceremony of which you have dreamed. 1. Are there any familial traditions which you would like incorporated into your wedding ceremony? 2. Are you religious? Are you spiritual? 3. How long have you been together? 4. How did you meet? 5. Is there an anecdote regarding your meeting which you would like referenced in your ceremony? 6. Is the bride’s father giving her away? If not, is someone else? 7. Will you have a processional? Will you have a formal seating of parents and/or grandparents? How are your guests to be attired? 8. Is the ceremony to be indoors or outdoors? 9. Do you intend to read your vows? Do you intend to repeat your vows (meaning: the Officiant speaks, you repeat)? 10. Do you want to write vows to one another? Do you want to write letters to each other? 11. If you wish to write something, but not read it, would you like the Officiant to do it? 12. How many guests are you expecting? 13. How many people are in the bridal party? 14. Are there any young children involved? If so, please list their ages.

16. Do you have an idea regarding the length of your ceremony?

17. Is your wedding to take place in the morning? Afternoon? Evening?

18. Would you like anyone to perform a reading? If so, whom? Do either you or the reader(s) have something specific in mind?

19. Please describe how you visualize your ceremony -- light-hearted? Solemn? Brief? Spiritual

20. Are there any specific elements which you either must have included in your ceremony, or which you feel must be excluded?

21. Are you interested in any of the smaller ceremonies that can go on during a wedding? (i.e., roses, wine, candles, and so on)

22. Will music be a part of your ceremony? If so, please describe (i.e., a DJ will play “Here Comes The Bride”, musician performing or singer, singing).

23. In your opinion, what is the key ingredient for a long and happy marriage?
