Event: Sport & Outdoor Activities

In consideration of the acceptance of the above entry in Yip’s Music & Montessori Elementary Private School, of whom I am the legal guardian, and the permission to participate in the outdoor programs organized by Yip’s Music & Montessori Elementary Private School. I hereby waive and forever discharge the Yip’s Music & Montessori Elementary Private School, the sponsors, representatives, their agents, officers, employees and any other person or organization assisting in the event from all claims, damages, costs and expenses in respect to damages or injury suffered by my/their person or property, however caused, which may occur as a result of my/their participation in the program in any location where the program is being held (i.e. sports & outdoor activities ).

I acknowledge and agree that the Yip’s Music & Montessori Elementary Private School may use photographs of the participants therein for promotional purposes.

4. Fees are non-refundable and non-transferable for sickness or any other reasons, unless the Centre is canceling classes.5. In case of severe weather condition (i.e. snow storm), the school will be closed.

REGULATIONS

1. The Centre reserves the right to alter the class schedule or cancel any class; parents will be notified of any changes.2. Lunch fee is $5 per meal (each meal includes main course and drink).3. Special arrangements for additional hours: arriving between 8am to 8:45am and/or leaving between 4:15pm to 6:30pm can be arranged with the office. Students will be charged at $5 per session. $5 per 15 minutes after 12:15/1:15/ 4:15pm and $1 per minute after 6:30pm will be charged for late pick up.
