Frequently asked Questions About tooth whitening

How long does whitening last?Whitening results are very stable, but not permanent, and results will vary by individual. Every day our teeth are ex-posed to foods, beverages and lifestyle habits that cause staining. Due to this exposure, it is advisable to periodically repeat the bleaching process and do touch-ups. To make the time frame between touch-ups longer, avoid food and beverages that cause staining.

Will whitening harm existing dental work?Studies have proven that whitening will not damage exist-ing restorations. As for changing the color of your existing restorations, (i.e. tooth-colored fillings, veneers, crowns, bonding, bridges) your whitening gel will NOT have any ef-fect on them. However, this could mean that you may want to replace some of the pre-existing dental work to match the new shade of your teeth.

Are there side-effects?Whitening of the teeth, while safe and effective, does have side-effects which vary according to individuals. Toothsensitivity, to hot and cold temperatures and even air, is a very common side-effect. This sensitivity will be temporary. You may also experience some gum sensitivity like a burning sensation and a grey discoloration.

These side-effects will be temporary andneither will cause long-term damage.

Tissue dissolution by sodium hypochlorite: effect of concentration, temperature, agitation and surfactant addition.

Aim: Sodium hypochlorite is the most commonly used endodontic irrigant due to its antimicrobial and tissue dissolving activity. The aim of this study was to evaluate and compare the effects of concentration, temperature and agitation on the tissue dissolving ability of sodium hypochlorite. in addition, a hypochlo-rite product with added surface active agent was compared with conventional hypochlorite solutions.

Methods: Three sodium hypochlorite solutions from two different manufacturers in concentrations of 1%, 2%, 4% and 5.8% were tested at room temperature, 37°C and 45°C with and without agitation by ultrasonic and sonic energy and pipetting. Distilled and sterilized tap water were used as controls. pieces of bovine muscle tissue (68±3) were placed in 10 ml of each solution for five minutes. in selected samples, agitation was performed for one, two or four 15 sec periods per each minute. The tissue specimens were weighed before and after treatment, and the percentage of weight loss was calculated.

Results: Weight loss (dissolution) of the tissue increased almost linearly with the concentration of sodium hypochlorite. higher temperatures and agitation considerably enhanced the efficacy of sodium hypochlorite. The effect of agitation on tissue dissolution was greater than that of temperature, continuous agitation result-ing in fastest tissue dissolution. hypochlorite with added surface active agent was most effective in tissue dissolution in all experimental situations.

Antimicrobial and Tissue Dissolving Capabilities of Different NaOCl Concentration

The objective of this in vitro study was to establish the antimicrobial efficacy and the potential to dissolve vital pulp using different concentrations of sodium hypochlorite (NaOCl) solutions. The fol-lowing solutions were tested: (1) 2% NaOCl (Thick Jik); (2) 3.5% NaOCl (Jik); 3) 6% NaOCl (vista); (4) 6% NaOCl (Dent-ionics); (4) 6% NaOCl (Chlor-XTra™) and (5) Water (Control).

No significant inhibition of e. faecalis was observed with sterile water (control) and 3.5% NaOCl. however, the 2% NaOCl and all the other 6% NaOCl solutions showed significant inhibition against e. faecalis. Chlor-XTra™ dissolved the pulp tissue in significantly less time compared to all the other groups (p<0.05).

Chlor-Xtra™ offered the best antimicrobial properties and tissue dissolving capabilities.

Demineralizing effects of EDTA at different concentrations and pH.

The purpose of this study was to compare effects of concentration and ph variations of eDTa on dentin demineralization. Twenty extracted, human permanent teeth with single canals were used in this study. Demineralizing effects of eDTa solutions at 10% and 17% concentrations at ph 7.5 and 9.0 were determined by measuring the amount of liberated phosphorus 1, 3, 5, 10, and 15 min after exposure.

Results showed that the amount of phosphorus liberated from dentin was great-er with increased EDTA concentration and increased time of exposure, and was more effective at neutral pH than pH 9.0. The ph of eDTa solutions did not display any significant alterations during the demineralization process.

The final irrigation step for long-lasting cleansingThis study developed a biofilm model that closely mimicked in vivo biofilm to determine its susceptibility to endodontic antimicrobial irrigants by three-dimensional quantita-tive analysis. Collagen-coated hydroxyapatite (C-ha) and uncoated hydroxyapatite (ha) disks were inoculated with dispersed subgingival plaque for three weeks. Thick biofilms rich in spirochetes were formed on both substrates. Biofilms were subjected to 1-, 3-, and 10-minute exposures to ChX-plus™ (vista Dental products, racine, Wi) and 2% chlorhexidine (ChX). after treatment, the volume ratio of dead bacteria to all bacteria in biofilms, indicated by the ratio of red and (red + green) fluorescence, was analyzed by confocal laser scanning microscopy for each medicament.

The proportion of killed bacteria depended on type of irrigant and time of exposure in both C-ha and ha biofilm models (p = 0.00). CHX-Plus™ showed higher levels of bactericidal activity at all exposure times than 2%CHX (p < 0.001). The C-ha bio-film was thicker than the ha biofilm. less bacteria were killed in C-ha biofilm than in the ha model.

Evaluation of the effect of two chlorhexidine preparations on biofilm bacteria in vitro: a three dimensional quantitative analysis.

The synergistic antimicrobial effect by mechanical agitation and two chlorhexidine preparations on biofilm bacteria.

This study investigates whether mechanical agitation (ultra-sonic or sonic) improves effectiveness of chlorhexidine against biofilm bacteria in vitro. Collagen-coated hy-droxyapatite (Cha) disks were exposed to dispersed subgingival plaque for 3 weeks at 37 degrees C. The multispecies biofilms established were subjected for 1 and 3 minutes to ChX-plus™ (vista Dental products, racine, Wi) and 2% chlorhexidine (ChX), with or without mechanical agitation. after treatment, the amount of dead bacteria in biofilms was analyzed by viability staining and confocal laser scanning microscopy (ClSm). The morphology of biofilms, with or without mechanical agita-tion, was also examined by ClSm.

The structure of the biofilm showed no obvious change when the surrounding solu-tions were exposed to continuous ultrasonic or sonic agitation. Combined use of mechanical agitation and chlorhexidine had a more pronounced anti-microbial effect against the biofilms than either one alone.

Benefits of using the Piezo Pilot™ for Ultrasonic Scaling

Piezo technology - what is it?Piezoelectric technology operates in a linear push/pull motion, working with the tooth surface (just like hand instruments) and results in little or no patient sensitivity. Much less water is needed because piezo uses ceramic discs for energy and does not generate the kind of heat produced by electromagnetic units, which is a welcome feature for both patients and practicioners alike!

The magnetostrictive (Cavitron™*) has a vibrating, ellipitical motion similar to a figure 8, which produces considerable heat. The tip needs a constant flow of water for cooling and to prevent sensitivity or damage to the tooth structure.

Vista’s Piezo Pilot™ - so easy to useThe Piezo Pilot™ offers a vartiety of power settings which enable clinicians to utilize a wide range of very thin tips with uses ranging from routine preventative care to severe periodontal cases.

Unlike Cavitron™* tips which move quickly in a scribbling figure 8 motion, slow, methodi-cal and controlled strokes are more efficient with the Pilot™ which requires time to trans-fer the energy needed to shatter the calculus and remove adherent biofilms. Using the terminal end of the tip like a perio probe - parallel to the long axis of the tooth - strokes can be horizontal, vertical or oblique overlapping to cover the entire surface. Maneuver-ing around the line angle, the tip is rotated in a similar manner to rotating a curette.

The piezo tip to power relationshipWith piezo technology, the most active part of the tip is the last 1-2mm which is in con-stant motion. The thinner the tip, the more movement the tip will generate.

• For patient comfort, use the lowest possible power to complete the job.

• For lighter calculus or biofilm removal, use a thinner tip at lower power.

• For heavy calculus deposits, use a thicker tip at a higher power.

• For heavy or burnished calculus, apply the tip in a tapping motion to fracture the large deposit.

The Pilot™ also offers easy portability, complimenting its light weight and compact design. This is a great feature for practices where space is an issue or the unit will be shared between operatories.

Frequently asked Questions about Vista’s Ultrasonic Piezo Pilot™

How do I know that the Pilot™ can remove calculus if I can’t feel it vibrating?The vibration and noise of the Cavitron’s™* elliptical movement is not need-ed for calculus removal (actually, it can even burnish the calculus). Using a Pilot™, when a tip is placed on the lateral surface of a tooth, the linear motion

of piezo technology more easily and precisely removes the calculus.

How does the lack of fiber-optic light on the handpiece affect the Pilot’s™ effectiveness?While it appears to add benefit, the light does not actually shine into the working area of pockets or root canals. In addition, repair to broken lights can be frequent and costly. The Pilot™ more than makes up for the fiber-optics through enhanced tactile sensitivity offered by its ergonomically designed

handpiece, allowing for better deposit removal.

Can the handpiece be cleaned?Yes, the actual Pilot™ handpiece can be easily cleaned with a simple disinfec-tant wipe. The handpiece sleeve is removable and can be autoclaved.
