GENERAL NOTES

1. ALL ELEVATIONS REFER TO NORTH AMERICAN VERTICAL DATUM (NAVD) -1988.

2. THE CONTRACTOR MUST COMPLY WITH ALL PROVISIONS OF THE FLORIDA STATUE 556, THE

UNDERGROUND FACILITY DAMAGE PREVENTION AND SAFETY ACT, AND LEE COUNTY UTILITIES

OPERATIONS/DESIGN MANUAL. IN THE EVENT OF CONFLICT BETWEEN THE ACT AND THE MANUAL, THE

MORE STRINGENT REQUIREMENT SHALL APPLY.

3. IT IS THE CONTRACTOR RESPONSIBILITY:

A. TO REPLACE ANY EXISTING LANDSCAPING (I.E. SOD, BUSHES, TREES, ETC.) AND SPRINKLER PIPE,

SPINKLER HEADS, FENCING AND ALL IDENTIFIED IMPROVEMENTS THAT MAY HAVE BEEN REMOVED

OR HAVE BEEN DAMAGED OR INJURED DURING CONSTRUCTION.

B. TO REPLACE OR REPAIR ANY DAMAGE PROPERTY OR IMPROVEMENTS TO A CONDITION EQUAL TO

OR BETTER THAN EXISTED PRIOR TO CONSTRUCTION.

4. ROADWAYS, DRIVEWAYS, SIDEWALKS AND OTHER EXISTING RIGHT-OF-WAY IMPROVEMENTS DAMAGED

DURING CONSTRUCTION SHALL BE RESTORED AS SPECIFIED BY THE LEE COUNTY UTILITIES

OPERATIONS/DESIGN MANUAL AND TO THE SATISFACTION OF THE ENGINEER AND FDOT / LCDOT.

5. THE FOLLOWING WORK SHALL BE DONE AT THE EXPENSE OF THE CONTRACTOR:

A. DAMAGE TO EXISTING UTILITIES AND PROPERTY DURING CONSTRUCTION SHALL BE REPAIRED

AND/OR REPLACED AT CONTRACTOR EXPENSE.

B. ANY PRIVATE PROPERTY ENCROACHMENTS SHALL BE RESTORED EQUAL TO PRECONSTRUCTION

CONDITIONS OR BETTER AND TO THE SATISFACTION OF THE ENGINEER , FDOT AND LCDOT.

C. TRAFFIC SIGNAL SYSTEM, TRAFFIC LOOPS, INTERCONNECT CABLES AND STREET LIGHING SYSTMS

SHALL BE MAINTAINED AT ALL TIMES. IF ANY CABLES OR SYSTEMS ARE CUT OR DAMAGED AS A

RESULT OF SUBJECT PROJECT AS DETERMINED BY THE COUNTY, THE CABLES OR SYSTEMS SHALL

BE REPAIRED IMMEDIATELY BY FDOT/LEE COUNTY APPROVED CONTRACTOR. ALL NECESSARY

REPAIRS SHALL BE PERFORMED TO APPLICABLE FDOT AND LEE COUNTY TRANSPORTATION

STANDARDS.

D. ALL CONCRETE DRIVEWAYS IMPACTED SHALL BE RESTORED FROM THE EDGE OF PAVEMENT TO THE

FIRST CONTROL JOINT.

6. CONTRACTOR SHALL MAINTAIN 10' MINIMUM CLEARANCE FROM ANY ENERGIZED POWER CONDUCTOR.

SPECIFICATIONS

1. ALL WORK SHALL CONFORM TO:

A. THE LATEST REVISION OF THE LEE COUNTY UTILITIES (LCU) OPERATIONS/DESIGN MANUAL, WHICH IS

ON FILE AT THE DIVISION OF LEE COUNTY UTILITIES OFFICE (1500 MONROE STREET, P.O. BOX 398,

FORT MYERS, FLORIDA 33902), THE LEE COUNTY LAND DEVELOPMENT CODE AND TO THE LATEST

EDITION OF THE FLORIDA DEPARTMENT OF TRANSPORTATION (FDOT) STANDARD SPECIFICATIONS

FOR ROAD AND BRIDGE CONSTRUCTION AND SUPPLEMENTS THERETO.

B. IN THE EVENT CONFLICT BETWEEN LEE COUNTY UTILITIES OPERATIONS/DESIGN MANUAL AND THE

FDOT STANDARD SPECIFICATIONS, THE MORE STRINGENT REQUIREMENTS SHALL CONTROL AS

DETERMINE BY THE LCU AND FDOT.

2. ANY QUANTITIES SHOWN ON PLANS ARE NOT VERIFIED BY LCU.

3. AT THE SITE KEEP AND MAINTAIN ONE RECORD COPY OF ALL CONTRACT DOCUMENTS, REFERENCE

DOCUMENTS AND ALL TECHNICAL DOCUMENTS SUBMITTED IN GOOD ORDER. AS THE WORK

PROGRESSES, THE ENGINEER OR HIS DESIGNATED REPRESENTATIVE SHALL RECORD ON ONE SET OF

REPRODUCIBLE DRAWINGS ALL CHANGES IN VERTICAL AND HORIZONTAL ALIGNMENT BY OFFSETS AND

TIES AT EACH SEWER, WATER, ELECTRICAL, GAS, COMMUNICATION AND OTHER SERVICES BY OFF-SET

DISTANCEES TO PERMANENT IMPROVEMENTS SUCH AS BUILDING AND CURBS.

A. THESE RECORD DRAWINGS MUST BE CERTIFIED BY A FLORIDA PROFESSIONAL ENGINEER, WHO

PREPARED, SIGNS AND SEALS THESE PLANS. RECORD DRAWINGS SHALL INCLUDE VERTICAL AND

HORIZONTAL ALIGNMENT OF ALL WATER, SEWER, AND EFFLUENT REUSE LINES, VALVES, TEES,

BENDS, REDUCERS, HYDRANTS, PUMP STATIONS, SERVICE CONNECTIONS, METER BOXES, AND/OR

PADS AND OTHER PERTINENT STRUCTURES. PIPELINE RUNS IN EXCESS OF 100 FT, WITHOUT

FITTINGS SHALL INCLUDE VERTICAL ALIGNMENT INFORMATION AT 100 FT INTERVALS. SAID

ALIGNMENT SHALL BE TIED TO PERMANENT IMPROVEMENTS, SUCH AS ROADWAY AND/OR RAILROAD

CENTERLINES AND RIGHTS-OF-WAY, BUILDING AND PROPERTY CORNERS, AND SHALL BE CERTIFIED

BY A PROFESSIONAL LAND SURVEYOR, LICENSED IN THE STATE OF FLORIDA. THE PROFESSIONAL

LAND SURVEYOR CAN COORDINATE WITH THE CONTRACTOR TO INSTALL THE NECESSARY

APPURTENANCES ON BURIED UTILITIES TO FACILITATE THE SURVEY AFTER CONSTRUCTION IS

COMPLETED. ADDITIONALLY, PROPERTY STRAP NUMBERS AND STREET NAMES SHALL BE SHOWN ON

THE PLANS.

B. ON A CASE BY CASE BASIS, LEE COUNTY UTILITIES MAY WAIVE THE REQUIREMENTS FOR

CERTIFICATION BY A PROFESSIONAL LAND SURVEYOR, LICENSED IN THE STATE OF FLORDIA.

HOWEVER, PRIOR CONSENT MUST FIRST BE OBTAINED FROM LEE COUNTY UTILITIES. THE COUNTY

SHALL WITHHOLD FINAL ACCEPTANCE OF THE PROJECT UNTIL THE REQUIREMENT FOR RECORD

DRAWINGS AND RELATED RECORDS HAS BEEN MET. RECORD DRAWINGS WITHOUT DETAILED FIELD

VERIFIED HORIZONTAL AND VERTICAL LOCATIONS OF ALL FACILITIES WILL BE REJECTED.

4. A PRE-CONSTRUCTION MEETING SHALL BE HELD WITH LCU, THE CONTRACTOR AND THE ENGINEER

BEFORE WORK MAY BEGIN. A PRE-CONSTRUCTION SITE CONDITIONS VIDEO IS REQUIRED BEFORE

WORK MAY BEGIN. LEE COUNTY UTILITIES SHALL BE NOTIFIED 48 HOURS PRIOR TO PROJECT

MOBILIZATION.

5. ALL WORK AND MATERIALS, WHICH DO NOT CONFORM TO LCU OPERATIONAL MANUAL AND FDOT

STANDARD SPECIFICATIONS, ARE SUBJECT TO REMOVAL AND REPLACEMENT AT THE CONTRACTOR'S

EXPENSE.

6. ANY WORK PERFORMED WITHOUT THE KNOWLEDGE OF LCU IS SUBJECT TO RE-EXCAVATION, REMOVAL

AND REPLACEMENT OF SAME TO BE DONE AT THE CONTRACTOR'S EXPENSE.

7. THE CONTRACTOR SHALL PROVIDE SUFFICIENT PERSONNEL AND EQUIPMENT ON THE JOB AT ALL TIMES

DURING CONSTRUCTION TO SATISFY THE SPECIFICATIONS AND TO COMPLETE THE WORK.

8. LCU INSPECTION STAFF MAY OBSERVE PROJECT CONSTRUCTION.

9. CONTRACTOR SHALL VERIFY ALL QUANTITIES SHOWN ON THE PLANS. IF ANY DISCREPANCIES IN

QUANTITIES ARE FOUND, THE CONTRACTOR SHALL NOTIFY THE PROJECT ENGINEER AND LCU.

10. ALL REGULATORY AND PERMITTING AGENCIES' REQUIREMENTS SHALL BE COMPLIED WITH.

11. APPROPRIATE TURBIDITY CONTROL DEVICES (E.G. SILT FENCES, HAYBAILS) WILL BE UTILIZED DURING

ALL PHASES OF INSTALLATION AND GRADING. CONTRACTOR IS RESPONSIBLE FOR SUBMITTING THE

NOTICE OF INTENT AND NOTICE OF TERMINATION OF THE EPA IN COMPLIANCE WITH LEE COUNTY'S

NPDES PERMIT. CONTRACTOR IS RESPONSIBLE FOR DEVELOPING AND MAINTAINING AN EFFECTIVE

STORM WATER POLLUTION PREVENTION PLAN.

12. IN THE EVENT THAT HARD LIMESTONE FORMATION IS ENCOUNTERED, MAKING IT IMPOSSIBLE TO

EXCAVATE TO THE DEPTH REQUIRED UNDER THIS CONTRACT, THE CONTRACTOR MAY BE ALLOWED TO

REDUCE THE PIPE COVER TO NOT LESS THAN TWO (2) FEET, WHILE MAINTAINING THE LCU REQUIRED

THICKNESS OF BEDDING UNDER THE PIPE. SUCH DEVIATION FROM THE PLANS MUST FIRST BE

APPROVED BY LCU AND THE ENGINEER PRIOR TO THE LAYING OF THE PIPE. IF TWO FEET OF COVER OR

MORE CAN NOT BE ATTAINED, THE CONTRACTOR SHALL PROVIDE ANOTHER METHOD OF

CONSTRUCTION OR PIPE PROTECTION, WHICH SHALL FIRST BE APPROVED BY LCU AND THE ENGINEER,

AT NO ADDITIONAL COST TO THE COUNTY.

MINIMUM DEPTH MUST BE MAINTAINED WITHIN FDOT R/W AS DETERMINED BY THE FDOT UAM. ANY

CHANGES TO THE APPROVED FDOT UTILITY PERMIT MUST BE APPROVED BY THE FDOT UTIITIES OFFICE

IN ADVANCE.

13. LOCATIONS, ELEVATION AND DIMENSIONS OF EXISTING UTILITIES, STRUCTURES AND OTHER FEATURES

ARE SHOWN ACCORDING TO THE BEST INFORMATION AVAILABLE AT THE TIME PREPARATION OF THESE

PLANS, BUT DO NOT PURPORT TO BE ABSOLUTELY CORRECT. PRIOR TO CONSTRUCTION, THE

CONTRACTOR SHALL VERIFY AND BE FULLY RESPONSIBLE FOR ANY AND ALL DAMAGES, WHICH MIGHT

BE OCCASIONED BY HIS FAILURE TO EXACTLY LOCATE AND PRESERVE ANY AND ALL EXISTING UTILITIES,

STRUCTURES AND OTHER FEATURES AFFECTING HIS WORK. ANYTHING NOT SHOWN ON THESE

DRAWINGS SHOULD BE BROUGHT TO ATTENTION OF THE ENGINEER AND SHALL NOT CONSTITUTE AN

EXTRA, UNLESS APPROVED BY THE ENGINEER.

14. THE CONTRACTOR SHALL CONTACT THE ENGINEER AND LCU IMMEDIATELY CONCERNING ANY

CONFLICTS WITH LCU UTILITIES/STRUCTURES ARISING DURING CONSTRUCTION OF ANY FACILITIES

SHOWN ON THESE DRAWINGS.

15. TRAFFIC MUST BE MAINTAINED AT ALL TIMES AS PER LEE COUNTY DEPARTMENT OF TRANSPORTATION

(LCDOT) AND PER FLORIDA DEPARTMENT OF TRANSPORTATION (FDOT) STANDARD SPECIFICATIONS FOR

ROAD AND BRIDGE CONSTRUCTION, LASTEST EDITION.

16. THE CONTRACTOR SHALL VERIFY ALL UTILITIES AND PROVIDE AT LEAST 48 HOURS NOTICE TO THE

INDIVIDUAL UTILITY COMPANIES AND FDOT AND LCDOT PRIOR TO CONSTRUCTION.

17. THE CONTRACTOR SHALL REPLACE ALL PAVEMENT, CURB, DRIVEWAY, SIDEWALKS, FENCES, ETC., WITH

THE SAME TYPE OF MATERIAL THAT WAS REMOVED DURING CONSTRUCTION OR AS DIRECTED BY THE

ENGINEER AND FDOT. THE CONTRACTOR SHALL RESTORE ALL AREAS AFFECTED BY THE

CONSTRUCTION TO THE ORIGINAL CONDITION, OR BETTER.

18. WITHIN THE LCDOT RIGHT-OF-WAY, ALL DISTURBED AREAS SHALL BE RECEIVE SODDING MATERIAL IN

ACCORDANCE WITH FDOT SPECIFICATIONS. ALL DISTURBED AREAS WITHIN THE FDOT RIGHT-OF-WAY

SHALL BE SODDED IN ACCORDANCE WITH FDOT SPECIFICATIONS. THOSE AREAS THAT ARE CLASSIFIED

AS DRAINAGE DITCHES SHALL RECEIVE FULL SOLID SOD.

19. THE INFORMATION PROVIDED IN THESE PLANS IS SOLELY TO ASSIST THE CONTRACTOR IN ASSESSING

THE NATURE AND EXTENT OF CONDITIONS, WHICH MAY BE ENCOUNTERED DURING THE COURSE OF

WORK. ALL CONTRACTORS ARE DIRECTED, PRIOR TO BIDDING, TO CONDUCT WHATEVER

INVESTIGATIONS THEY MAY DEEM NECESSARY TO ARRIVE AT THEIR OWN CONCLUSION REGARDING THE

ACTUAL CONDITIONS THAT WILL BE ENCOUNTERED, AND UPON THEIR BIDS WILL BE BASED.

20. ALL FRAMES, COVER VALVES BOXES, METER BOXES AND MANHOLES SHALL BE ADJUSTED TO FINISHED

GRADE UPON COMPLETION OF PAVING OR RELATED CONSTRUCTION. ALL VALVE PADS SHALL BE

POURED IN PLACE. NO PRE-FORMED VALVE PADS ARE ALLOWED.

SPECIAL PROJECT REQUIREMENTS AND NOTES

1. SPECIAL ATTENTION SHALL BE PAID TO SECTIONS 5, 6 AND 13 IN THE LEE COUNTY UTILITIES

OPERATIONS/DESIGN MANUAL AND SPECIFICALLY TO:

A. UNLESS OTHERWISE NOTED, PIPE INSTALLATION METHOD WILL BE OPEN CUT.

B. PIPE TRENCH AS REQUIRED BY SECTION 6 STANDARD DETAIL 6.5 OF THE LEE COUNTY UTILITIES

OPERATIONS/DESIGN MANUAL.

C. WATER SERVICE INSTALLATION AS REQUIRED BY SECTION 6 STANDARD DETAIL 6.17.

D. CONNECTION OF THE NEW WATER MAIN INTO THE EXISTING AS REQUIRED BY SECTION 5.2D.

E. ARVs SHALL BE FIELD LOCATED AS CLOSE TO RIGHT-OF-WAY OR EASEMENT LINE AS POSSIBLE AS

SHOWN ON DETAIL 6.4.

F. FLUSH, PRESSURE TEST AND THEN DISINFECTING THE ENTIRE PIPING SYSTEM AS IN SECTION 5.2G

AND SECTION 13 (TECHNICAL SPECIFICATIONS 02675 AND 02676). DISINFECT, SAMPLE AND OBTAIN

BACTERIOLOGICAL CLEARANCE FOR THE NEW WATER MAIN AND TEMPORARY WATER LINE BEFORE

MAKING SERVICE CONNECTIONS AND HYDRANT CONNECTIONS.

G. OPEN ROADWAY CUTS SHALL MEET:

1) THE REQUIREMENTS IN SECTION 13 TECHNICAL SPECIFICATIONS 02230 IN THE THE LEE COUNTY

UTILITIES OPERATIONS/DESIGN MANUAL.

2) THE CONSTRUCTION RESTRICTIONS LISTED IN ITEMS 10 AND 11, BELOW.

H. DIRECTIONAL DRILLING

1) THE REQUIREMENTS IN SECTION 13 TECHNICAL SPECIFICATIONS 02300 DIRECTIONAL DRILLING.

I. PIPE JOINTS SHALL BE RESTRAINED AS REQUIRED BY SECTION 6 STANDARD DETAIL NO. 6.12.

2. THE CONTRACTOR IS REQUIRED TO SUBMIT MOT PLANS FOR REVIEW AND APPROVAL BY LEE COUNTY

DOT AND THE ENGINEER PRIOR TO CONSTRUCTION. FOR WORK WITHIN FDOT R/W AN MOT PLAN MUST

BE SUBMITTED TO THE FDOT UTILITIES OFFICE FOR THEIR REVIEW AND APPROVAL AS PART OF THE

PERMIT PACKAGE. MOT PLANS SHALL BE IN ACCORDANCE WITH THE LATEST VERSION OF THE FDOT

DESIGN STANDARDS AND INDEX NO. 612. MAINTENANCE OF TRAFFIC TO BE SUBMITTED BY SOMEONE

THAT HOLDS THE NEW WORK ZONE TRAFFIC CONTROL ADVANCED TRAINING CERTIFICATION. ALL

WORK ZONES TO BE SET UP AND MAINTAINED BY SOMEONE THAT HAS THE WORK ZONE TRAFFIC

CONTROL INTERMEDIATE LEVEL TRAINING. THIS IS REQUIRED AS OF JANUARY 2002.

3. THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING A ROW PERMIT FROM LEE COUNTY DOT. THE

ENGINEER SHALL BE RESPONSIBLE FOR OBTAINING A ROW PERMIT FROM FDOT. (FDOT PERMIT

#13H192-132)

4. THE CONTRACTOR MUST MAINTAIN EXISTING ACCESS POINTS TO ALL PROPERTIES.

5. SAWCUT CONCRETE OR ASPHALT DRIVEWAYS AS REQUIRED FOR CONSTRUCTION. REPLACE AS

REQUIRED FOR A SMOOTH CONNECTION.

6. CONTRACTOR IS RESPONSIBLE FOR OBTAINING A CONSTRUCTION DEWATERING PERMIT FOR UTILITIES

CONSTRUCTION AS REQUIRED BY S.F.W.M.D.

7. THE DISPOSAL OF ALL EXCAVATED MATERIAL IN EXCESS OF THE NEEDS FOR BACKFILL SHALL BE THE

RESPONSIBILITY OF THE CONTRACTOR. ALL EXCESS MATERIAL IS THE PROPERTY OF THE

CONTRACTOR.

8. ANY SIDEWALK DAMAGED DURING CONSTRUCTION WILL BE RESTORED TO PRECONSTRUCTION

CONDITION OR BETTER.

9. ALL DRAINAGE SHALL BE MAINTAINED DURING CONSTRUCTION.

10. TICE STREET WATER MAIN CONSTRUCTION

A. WITHIN THE FDOT I-75 RIGHT OF WAY, A 14" HDPE WATER MAIN (CARRIER PIPE) SHALL BE INSTALLED

WITHIN A 24" HDPE CASING. THE CASING SHALL BE INSTALLED BY MEANS OF DIRECTIONAL DRILLING.

THE DIRECTIONAL DRILL SHALL PROCEED FROM THE EAST SIDE OF THE RIGHT OF WAY TO THE

WEST.

B. CONTRACTOR TO FIELD LOCATE AND VERIFY EXISTING 18" CASING UNDER I-75. CASING TO BE ABANDONED INPLACE, GROUT FILLED AND CAPPED.

C. AREAS OF PAVEMENT THAT ARE DISTURBED DURING CONSTRUCTION SHALL BE RESTORED WITH

THE STANDARD DETAIL ON SHEET 19 OF THE DRAWINGS.

D. CONTRACTOR IS HEREBY NOTIFIED OF THE PRESENCE OF ASBESTOS CEMENT (AC) PIPE FROM WEST

OF DETAR LANE TO LEXINGTON. AC PIPE IN THIS LOCATION SHALL BE CUT AND CAPPED, GROUT

FILLED AND ABANDONED IN PLACE.

E. ALL FEDERAL AND STATE CODES, RULES AND REGULATIONS SHALL BE ADHERED TO WITH RESPECT

TO WORKING ON AND/OR THE DISPOSAL OF THE AC PIPE.

11. COUNTRY LAKES ROAD WATER MAIN CONSTRUCTION

A. CONSTRUCTION OF NEW WATER MAIN ACROSS THE ACCESS TO SUGARMILL SPRINGS SHALL BE

CONDUCTED SUCH THAT INGRESS AND EGRESS TO SUGARMILL SPRINGS IS MAINTAINED AT ALL

TIMES.

B. AREAS OF PAVEMENT THAT ARE DISTURBED DURING CONSTRUCTION SHALL BE RESTORED WITH

THE STANDARD DETAIL ON SHEET 18 OF THE DRAWINGS.

C. ALL MOT PLANS FOR LANE CLOSURES MUST BE APPROVED BY LEE COUNTY DOT PRIOR TO

IMPLEMENTATION.

D. WATER MAIN INSTALLATION WILL BE WITHIN EXISTING OR BOOK 2130 PG 3551 UTILITY "BLANKET"

STORMWATER POLLUTION PREVENTION PLAN (SWP3) GENERAL NOTES:

IN ACCORDANCE WITH LEE COUNTY LAND DEVELOPMENT CODE, CHAPTER 14 - ARTICLE VII,

THE STORMWATER POLLUTION PREVENTION PLAN (SWP3) HAS BEEN CREATED TO ENSURE

THAT POLLUTANTS FROM CONSTRUCTION ACTIVITIES WILL NOT ENTER STORMWATER

DISCHARGE FROM THE SITE.

1. CONTRACTOR SHALL ADHERE TO ALL SFWMD PERMIT CONDITIONS AS APPROVED

BY SOUTH FLORIDA WATER MANAGEMENT DISTRICT.

2. CONTRACTOR SHALL HAVE ON PREMISES A COPY OF THE SWP3 DURING THE COURSE

OF CONSTRUCTION.

3. CONTRACTOR SHALL BE RESPONSIBLE FOR THE MAINTENANCE OF POLLUTION

PREVENTION CONTROLS AND OPERATIONS DURING THE ENTIRE PERIOD OF

CONSTRUCTION.

4. BEST MANAGEMENT PRACTICES (BMP'S) SHALL BE IMPLEMENTED DURING THE

COURSE OF CONSTRUCTION TO ENSURE POLLUTION ABATEMENT OF

STORMWATER DISCHARGE FROM SITE ACTIVITIES, SUCH AS:

GOOD HOUSEKEEPING - SAFE, ORDERLY AND CLEAN ENVIRONMENT SHALL BE

MAINTAINED BY CONTRACTOR. CONTRACTOR SHALL STORE WITH EXTREME

CARE AN Y AND ALL POSSIBLE CONTAMINANTS;

STORMWATER MANAGEMENT PRACTICES - THE STORMWATER MANAGEMENT

SYSTEM IS DESIGNED TO PROVIDE WATER QUALITY TREATMENT PRIOR TO OFF-

SITE DISCHARAGE;

PREVENTATIVE MAINTENANCE - CONTRACTOR SHALL PROVIDE ROUTINE

INSPECTION OF OPERATIONS AND EQUIPMENT TO ENSURE STORMWATER

POLLUTION PREVENTION;

SEDIMENT AND EROSION CONTROL - CONTRACTOR SHALL INSTALL BMP'S TO

PREVENT STORMWATER POLLUTION RESULTING FROM SITE DISCHARGE OF

UNTREATED RUNOFF AND TO CONTROL SITE EROSION;

SPILL PREVENTION AND RESPONSE - CONTRACTOR SHALL IMPLEMENT A SPILL

RESPONSE PLAN WHEN SITE CONTAMINATION OCCURS AND SHALL MONITOR

REFUELING AND EQUIPMENT MAINTENANCE OPERATIONS;

EMPLOYEE TRAINING - CONTRACTOR SHALL BE RESPONSIBLE FOR TRAINING

ALL EMPLOYEES ON DETECTION OF STORMWATER POLLUTION AND RESPONSE

MEASURES;

RECORD KEEPING - CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING

INSPECTION AND MONITORING RECORDS, STORMWATER POLLUTION

PREVENTION PLAN PROCEDURES, AND LIST OF EMERGENCY CONTACT

AGENCIES AND PHONE NUMBERS ON THE PROJECT SITE DURING THE ENTIRE

CONSTRUCTION PERIOD;

5. CONTRACTOR SHALL NOTIFY OWNER AND ENGINEER UPON DETECTION OF

POLLUTION ACTIVITY, SPILL RESPONSE, AND /OR DEVIATION FROM STORMWATER

POLLUTION PREVENTION PLAN.

6. THE STORMWATER POLLUTION PREVENTION PLAN SHALL REMAIN IN COMPLIANCE

BY CONTRACTOR UNTIL ALL CONSTRUCTION SITE ACTIVITIES AND STORMWATER

IMPROVEMENTS HAVE BEEN COMPLETED AND ALL DISTURBED AREAS HAVE BEEN

PROPERLY STABILIZED AND VEGETATED.
