Guided Care and a Robust Suite of Online Tools and Resources

UnitedHealthcare Navigate plans offer comprehensive, patient-centered health care benefits. Members will choose a primary care physician (PCP) as their trusted partner in health care who also grants referrals for specialists’ services.

With UnitedHealthcare Navigate, there are a variety of plan design options that provide traditional benefits with the security and guidance of a primary care physician.

UnitedHealthcare Navigate offers network coverage for members who obtain a referral from their primary care physician prior to seeing another physician or specialist in the network. No coverage is provided without a referral. Non-network coverage is not available, except in the case of emergency care.

A trusted health care guide: the primary care physician

Upon enrollment in any Navigate plan, members must select a primary care physician from the Navigate network. Primary care physicians can be general practitioners, family practitioners, internists or pediatricians. If a member does not choose a primary care physician, one will be assigned to them. Each family member can choose a different primary care physician to manage their health care needs. The primary care physician will provide care for the majority of a member’s needs – routine care, such as annual well visits and preventive care, as well as care for sickness or injury. Since the primary care physician will be the member’s first and foremost source of care and will understand the member’s health needs thoroughly, they can best refer the member to other network physicians or specialists when additional care is needed.

Primary care physicians actively promote higher quality and efficient care for members by helping them get the appropriate care at the appropriate time. Members must obtain a referral from their primary care physician before seeing another network physician.

Referrals are not required for services from a network OB/GYN, from network behavioral health and substance use disorder clinicians or for routine refractive exams from network providers. Certain inpatient and outpatient procedures require that the network physician obtain prior authorization from UnitedHealthcare before the procedure is performed.

The primary care physician is the key to helping members live healthier lives, guiding them through a complex health care system and helping them get the right care at the right time from the right providers.

All health plan ID cards will carry the product name UnitedHealthcare Navigate. Th e name and telephone number of the member’s primary care physician will be listed on the health plan ID card. Members should present their health plan ID card to their provider at every visit so their provider bills UnitedHealthcare correctly.
