Important information

This checklist is not exhaustive – if there is additional information/documentation about your circumstances that is not in the checklist or the application form, you should include the additional information or a covering letter with an explanation.

As the primary applicant you are responsible for providing all documents and information you believe will support your application at the time of application lodgement

Please be aware that the decision on your visa application may be made by the Australian Department of Immigration and Border Protection (DIBP) based on the information and documentation that you include at the time of lodgement.

The Visa Application Charge will not be refunded if a decision is made to refuse to grant the visa because the applicant did not satisfy the criteria for grant of the visa.

How to use the checklist

1. When you are satisfied that you have correctly completed your visa application form and all of the relevant documentation is included, you should sign and date your visa application form.

2. Include the completed visa application checklist when you lodge your visa application form and relevant documentation.

Delivery and courier charges

You are responsible for all costs of delivering information to the Australian Consulate-General (ACG) by mail or courier, including any additional information that may be requested by the ACG. If you give additional information to the Australian Visa Application Centre, you may be charged a courier fee for delivery to the ACG.

Payment of the Australian Government Visa Application Charge. Advice on how to pay this charge can

be found at www.vfsglobal.com/Australia/GCC/ OR if the Visa Application Charge has been paid at

an office of the Department of Immigration and Citizenship in Australia, a copy of the receipt must be submitted.

From 1 July 2014, the Department of Immigration and Border Protection is moving to a full user-pays model at all biometric Australian Visa Application Centres. This means that the Service Delivery Partner will charge clients a service fee for visa application lodgement and biometric collection. This is in addition to visa application charges.

Copy of valid passport or travel documents of all people included in the application.

Evidence of international travel history in the last 5 years (e.g. visa stamp and/or immigration stamp)

Two recent passport-sized photographs, no more than 6 months old, of you and any children included in your passport and travelling with you.

These should be the head and shoulders only against a plain background.

Print the name of the person on the back of each photograph.

You will need to meet Australia’s health requirement in order to be granted a visa. Completing the My Health Declaration - http://www.border.gov.au/Trav/Visa/Heal/meeting-the-health-requirement/health-examinations/my-health-declarations - will confirm whether a medical examination will be required. If a medical examination is required please complete your health examination before you lodge a visa

application. Print the completed My Health Declaration indicating HAP ID and book an appointment

with a panel physician. For list of panel physicians http://www.border.gov.au/Busi/Pane/Pane-1. Please

take your current and valid passport with you to the medical appointment.

If you are employed: Letter from your employer – stating your position, length of employment and income) and the period of approved leave.

If you are self-employed: Copies of company documents. For example: business registration license, import and export permit, bank statements.

Evidence of your proposed Temporary Work (short stay) activity in Australia. For example: letter of invitation from an Australian company, trade contract, etc

Evidence that you have sufficient funds for your personal support during your stay in Australia. Please provide any of the following documents: personal bank statements, pay slips, audited accounts taxation records or credit card limit.

If your visit is supported by your employer:

Letter from Employer - the letter should include details of what travel costs your employer will cover. Details of travelling companions, copy of employer’s passport and copy of your residence visa must also be provided if you are employed as Domestic Worker.
