Unicity Nigeria Pre-Launch

Due to an overwhelming demand, on December 15th, 2011, Unicity International will start taking

orders for Bios Life Slim®. Below is some helpful information on how to get started. To ensure

prompt delivery and prompt registration, please follow these instructions:

How To Register:

To register as a Franchise partner in Nigeria you will need to download and complete the following

forms found online at www.unicity.net/nigeria and email them to nigeriasupport@unicity.net or fax

them to +1-801-836-2779:

1. The US Franchise Partner Application – All Nigerian Franchise Partners will be

registered as US Franchise Partners and there is a US$40 registration fee. This

application MUST have a US address. You may be able to use your US upline’s address,

or a US friend or relative’s address.

2. W-8BEN – All Nigerian Franchise Partners must fill out this form and include it with their

application. No ID numbers will be given unless this is filled out in its entirety. This form

MUST include your Nigerian address.

3. Copy of Passport – All Nigerian Franchise Partners must include a copy of their passport

or of a government issued ID with their application and W-8BEN.

Once we have received all the required information, you will receive an email with your ID number

and information on how to login to the website.

How To Order:

To place an order you will need to download and complete fill out the order form found at

www.unicity.net/nigeria and fax or email it to nigeriasupport@unicity.net.

Currently Unicity International only accepts bank wire/money wire payments for orders.

Important Pre-Launch Ordering Information:

During this pre-launch, all orders for all Nigerian Franchise Partners will be shipped to Nigeria

together. Once we have received the initial orders, the product will be shipped to the warehouse in

Lagos, Nigeria for pickup. We will inform each Franchise Partner when the orders have been

shipped and when they can be picked up in Lagos. Here is the address and contact information for

Receiving Commission Payments

All Nigerian Franchise Partners will follow the same compensation plan that is found in the United

States. More information about the compensation plan can be found at www.unicity.net/nigeria.

They will receive their commission payments on a ProPay MasterCard® that will work worldwide.

The ordering of the card should be done at the time of registering as a Nigerian Unicity Franchise

Partner. Details on registering can be found on the Unicity Nigeria website.

Pricing of Bios Life Slim®

The price of Bios Life Slim® includes all duties, shipping and taxes that are required to import the

product into the country.
