Underground Storage Tank (UST)  The following forms are required to be uploaded to CERS:

-UST Monitoring Site Plan (map) -UST Certification of Financial Responsibility -UST Response Plan -UST Owner/Operator: Written Agreement (if applicable) -UST Letter from Chief Financial Officer (if applicable) -Owner Statement of Designated UST Operator Compliance

 a) Log in to CERS Business Portal: http://cers.calepa.ca.gov/

At CERS homepage, find your business and click on the “Start/Edit Submittal” Scroll down to find “Underground Storage Tanks” and click on “Start”  

  

b) Complete “UST Facility Operating Permit Application”. Click “Save”.

c) Complete “UST Tank Information/Monitoring Plan”

-If you have the CA state forms Tank Information (Form B) and Monitoring Plan (Form D), you can use those to complete the online submission -Each individual UST requires its own form (e.g. If a tank has two compartments, each compartment requires a separate UST Tank Information form and corresponding monitoring plan).  To add another UST to CERS, click on “Add Tank” 

 Information from the previous tank submittal can be copied onto the next tank. Be sure to change the Tank ID#.

 Once the tank information is modified for the 2nd tank, an additional monitoring plan associated with that tank must be filled out. Information from the previous submittal can be copied.   

 Save the documents and continue the process until all UST information has been correctly accounted for.

d) Complete UST Certifications of Installation / Modification (if applicable)

-If necessary complete UST Certifications of Installation/Modifications

e) Complete additional required documents and upload to CERS The following forms are required to be uploaded to CERS:

-UST Monitoring Site Plan (map) -UST Certification of Financial Responsibility -UST Response Plan -UST Owner/Operator: Written Agreement (if applicable) -UST Letter from Chief Financial Officer (if applicable) -Owner Statement of Designated UST Operator Compliance

 To upload a document, click on “New”. When clicking on “UST Monitoring Site Plan” for example, the website will take you to another page to upload.

 After uploading the required documents, the Underground Storage tanks portion of CERS is ready to submit. After reviewing the elements and uploaded documents, you’re ready to click on the “Submit”.
