Final Evaluation
Purpose of Final Evaluations
The final evaluation will be submitted to the Faculty of Social
Work to inform a Pass or Fail in your 3150 or 4120 field placement.
Purpose of Final Evaluations• Opportunity to hear from peers and agency
staff members. Field instructors have an opportunity to call on co-workers and other staff /agencies to provide feedback on student performance.
• Opportunity to hear feedback about performance and identify areas that the student may concentrate on, in terms of professional ongoing development.
Completing Final Evaluation
Overview
1) Review learning contract and final evaluation information
2) Complete scoring and comments on evaluation independently
3) Final evaluation meeting & meeting with faculty field liaison
4) Submission of final evaluation
Look to the learning contract to provide examples in your evaluation comments.

The learning contract will be reviewed if there are discrepancies in the final evaluation
1) Connection to Learning Contract
• 3150 evaluations are extremely important as the evaluation will be provided to the student’s next field instructor.
• Strengths should be identified and examples provided.
• Areas requiring improvement can be transferred to the next placement and built into the 4120 learning contract.– Provide example of what needs to be accomplished
to meet the areas identified.
• Students should use their 3150 evaluation to build their learning contract for 4120.

2) Completing the Final Evaluation
• Student and field instructor should independently complete the rating scale and provide written comments to substantiate their ratings.
• Student and field instructor will then have a final evaluation meeting to discuss their ratings and comments.
• Consensus does not have to be achieved
• Compile comments and scores prior to submitting evaluation.

• Very important
• Provides feedback
• 3150 evaluation is provided to 4120 field instructor.
• When asked for references we go to the student‘s evaluation and use the comment section as a source of information.
• When completing your final evaluation it is important to discuss positive and negative experiences, feelings, and reactions to field.
• Opportunity for the student to articulate what was helpful about the placement and what areas could be improved to facilitate better learning.
• The final evaluation can be a good learning experience for both field instructor and student.
• The faculty / field liaison will have a telephone meeting with the student and field instructor at final evaluation.
3) Final Evaluation Meeting
Evaluation Should be Objective and Fair
• Should be connected to the learning contract.
• Address items from the evaluation form.
• Student should be given feedback and ways to improve performance.
• Evaluation should document examples of performance.
4) Submitting the Evaluation
• The forms MUST be completed electronically and placed in the dropbox as one document.
• Please ensure that you fax or scan the Grade Submission page of the evaluation to us.

Differences/Discrepancies• If a student and field instructor do not agree
and cannot come to an agreement regarding the outcome of the evaluation please contact me to discuss this.
• I would then become involved to help mediate the issue and come to a determination regarding the outcome of the evaluation.
Addendum• If a student receives an unsatisfactory grade
there are several options:– Student will receive a fail and be required to
repeat the practicum– Student will be given an opportunity to address
the failing areas, an addendum requiring additional hours to address the failing areas
The addendum is developed between the student, field instructor, field coordinator, and
field liaison.
Dates to Remember
• Final evaluations are due when 420 hours are completed - your timeliness in this matter is important in order for your final grade to be submitted.
• Please contact your Field Liaison to schedule your final evaluation telephone conference.
• Monthly journal and time/task sheets are due on the last day of every month.
