DIAGNOSIS OF COPD GOLD GUIDELINES 2011
Chronic Obstructive Pulmonary Disease (CODP), a common preventable and treatable disease, is characterized by
persistent airflow limitation that is usually progressive and associated with an enhanced chronic inflammatory
response in the airways and the lung to noxious particles or gases. Exacerbations and comorbidities contribute to
the overall severity in individual patients.
Diagnosis of COPD
• Clinical history:– Chronic cough– Chronic Decreased effort tolerance– Wheeze– SMOKER
• Unlike in asthma, Spirometry and CXR are mandatory for diagnosis and long-term management
Symptoms of COPD• Dyspnea:
– Progressive– Usually worse with exercise– Persistent– Many patients ( especially locally!) don’t complain
– they tailor activities to effort tolerance– Age per se does not cause dyspnea!
Symptoms of dyspnea
• Chronic cough– “smoker’s cough”– May be intermittent and may be unproductive
• Chronic sputum production– Any pattern of chronic sputum production may
indicate COPD
• History of exposure to risk factors– Smoking– Occupational dusts/chemicals– Smoke from home cooking and heating fuels
Causes of wheezing: top ten list• Bronchitis• Asthma• COPD• CCF• Vocal cord dysfunction• Foreign body• Recurrent aspiration• Bronchiectasis• Pulmonary embolism• Tumor• Broncholith/ airway stricture

Oral Prednisolone?
• Short bursts of oral prednisolone are effective in treating acute exacerbations
• Long-term oral steroids are NOT reccommended:– Long-term treatment leads to steroid myopathy,
which contributes to muscle weakness, decreased functionality and repiratory failure in severe COPD(level A evidence, GOLD guidelines)
Theophylline in COPD
• Well-established place in COPD treatment– Improves inspiratory mucle function– Decreases exacerbations– Evidence that it helps to reverse steroid resistance
in COPD– No effect on mortality or FEV1– Low therapeutic index

Physical Activity for COPD patients• Stretching exercises – improve flexibility, good warm-
up• Aerobic exercises – walking/ cycling• Strengthening exercises
– Tightening muscles to the point of fatigue– Upper body strengthening can improve respiratory

• Frequently patients’ SOB leads to deconditioning – a vicious cycle.
• Ten minute’s exercise 3 times a day adds up to 30 min
Breathing Exercises for COPD patients
• Pursed lip breathing:1. Relax your neck and shoulder muscles.2. Breathe in for two seconds through your nose, keeping
your mouth closed.3. Breathe out for four seconds through pursed lips. If this
is too long for you, simply breathe out twice as long as you breathe in.
• Use pursed-lip breathing while exercising. If you experience shortness of breath, first try slowing your rate of breathing and focus on breathing out through pursed lips.
Breathing exercises for COPD patients
• Diaphragmatic breathing:1. Lie on your back with knees bent. You can put a
pillow under your knees for support.2. Place one hand on your belly below your rib cage.
Place the other hand on your chest.3. Inhale deeply through your nose for a count of 3.
(Your belly and lower ribs should rise, but your chest should remain still.)
4. Tighten your stomach muscles and exhale for a count of 6 through slightly puckered lips.

3. Effective treatment is available for treatment of COPD
4. Treatment with inhalers will not be effective if pt doesn’t know how to use them!
5. Smoking cessation is VERY important – don’t give up!
6. It’s important to teach patients how to manage with a chronic disease like COPD
