  • Register your Babybefore they’re 8 weeks old!

    IT’S REALLY IMPORTANT!

The hospital doesn’t do this for you

    It’s online and it’s FREE!Go to www.bdm.nsw.gov.au/newborn before your

baby turns 8 weeks old. If you have problems,

we’re here to help. Just call 13 77 88 (Service NSW)

    7am - 7pm, Monday to Friday.

    Register your baby BEFORE completing

    the Medicare form.

    Registeryour baby at

    www.bdm.nsw.gov.au/newborn

NSW Registry of Births Deaths & Marriages

  • Here’s how to register your baby online.

    Register your Baby...this part’s free!!

Prepare ID documents for both parentsBefore going online, you’ll first need to have three forms of ID for each parent.

You need one from each Category: 1, 2 and 3. If you’re unable to provide ID

from Categories 1 and 2, you must still provide at least three forms of ID.

At least two of these must be from Category 3 and one can be from Category 4.

If you don’t have these documents, please contact us on 13 77 88 for help.

Scan or take a photo of each document and save the image files to a location on your computer or mobile

device where you can find them easily. It’s also a handy tip to name each file so you can identify which ones

are which. The files can be JPEG, TIFF or PDF (max. file size: 4MB each). Please make sure they’re clearly readable!

    IDENTIFICATION DOCUMENTS

It’s important you register your baby - the hospital doesn’t do this for you! If you don’t register your baby, you may not be able

to apply for certain Centrelink benefits like

Parental Leave. Also, if your child’s birth isn’t registered,

you won’t be able to get a birth certificate for them.

This means your child won’t be able to enrol in school

or sport, get a passport, or when they’re older, get a driver’s

licence, a job, a Tax File Number or open a bank account.

IT USUALLY TAKES ABOUT 20 MINUTES

  • Check everything’s correctYou’ll also need to ask the other parent to check their details

are correct (by ticking the appropriate box). It’s important that

both parents check and certify the information.

Read the checklist below, then go to www.bdm.nsw.gov.au/newborn

and complete the online form. The online form works with most browsers.

If you can’t get it to work on yours, try another one like Google Chrome or

Safari. We know how busy you can be, so you don’t have to complete the form

in one go - once you’ve filled in the first page you can come back to the form

later. Just use the reference number in the email that we’ll send you,

and when you’re less busy you can carry on where you left off.

Make sure you’ve agreed on the baby’s name! This is important because if you change your

mind later, you’ll need to pay to change it.

Hit ‘submit’ Once you’ve finished the registration part, you can choose

to order a birth certificate at the same time if you wish (see back page).

    * You can find this in the Blue Book given to you by the hospital.

Email address of the person registering the baby

    Contact details of each parent

    Baby’s full name

Date, time and place of baby’s birth*

    Baby’s birth weight in grams*

Both parents’ full names and place of birth

    CHECKLIST OF WHAT YOU’LL NEED

For both parents, their mother’s surname at the time of her own birth

    Occupation of both parents

Both parents’ address during most of the pregnancy

Clearly readable scan or photo of three ID documents for both parents (see step 1 on page 2)

If the parents are legally married, date and place of marriage

If there are any other children from this or a different relationship, list their details (full names, date and place of birth)

Name, address and contact details of the two informants (one of whom has to be the applicant). Only one informant is required if only one parent is listed

Credit or debit card if you want to order a birth certificate

  • HOW DO I ORDER A BIRTH CERTIFICATE?

It’s all in the same online process. Once you’ve registered your baby, you can

choose to order a birth certificate as well, or order one later, if you wish.

WHY ORDER A BIRTH CERTIFICATE AT THE SAME TIME?

Doing it now will save you time later.

WHAT KIND OF BIRTH CERTIFICATES ARE THERE?

The standard birth certificate (shown right) costs $60* including postageand handling. It’s the official document proving your child’s identity.

Commemorative certificates (see below) - Why not celebrate the birth of your beautiful baby with one of our great

looking commemorative birth certificates as well? These cost an additional $27* and are a perfect size for framing (279mm x 355mm). You can choose your favourite commemorative from a range of designs when you go online. Just be aware that

the commemorative certificates don’t have the same security features as the standard version, so they may not be accepted

by organisations as proof of identity. Commemoratives are sent separately from the standard certificate.

    * Price correct at time of printing and subject to change without notice.

    KEN DONE FAUNA
