Documentation reviewed by this inspector revealed that the jail is not completing

visual face-to-face observations at least every 30 minutes in areas where inmates

known to be assaultive, potentially suicidal, mentally ill, or who have demonstrated

bizarre behavior are confined as required by Minimum Jail Standards.

Preventive maintenance, to include necessary repairs, shall be conducted to ensure a safe,

secure, and sanitary facility.

During the review of the jailer life safety training, it was determined that the jailers did

not receive life safety training quarterly as required by minimum jail standards.

Emergency Electrical Power. (a) New facilities, new additions, and major renovations to

existing facilities shall be equipped with an emergency back-up electrical generator designed

to operate both manually and automatically upon interruption of the primary electrical power

source. The system shall be capable of operating uninterrupted for a minimum period of 1½

hours without refueling. Back-up electrical power shall be provided for necessary equipment

and life safety systems including, but not limited to: (1) emergency illumination systems;

(2) exit lights; (3) smoke management systems; (4) fire detection and alarm

systems; (5) audible communication systems; (6) security/control systems; (7)

normal ventilation systems required for smoke detection.

During the walk-through of the Bi-State and Annex facilities, the following preventive

maintenance issues were noted:

Training and Drills. Training of staff for emergency situations shall be provided

immediately upon employment and no less than each calendar quarter for all jail

personnel to include fire, emergency, evacuation drills, and location and use of

equipment.

Facilities shall have an established procedure for visual, face-to-face observation at least

once every 60 minutes and at least every 30 minutes in areas where inmates known to be

assaultive, potentially suicidal, mentally ill or who have demonstrated bizarre behavior are

confined.

During the generator load test at Annex 1, the generator did not perform as designed.

The generator did not transfer all power to the building automatically. Power was not

supplied to the security controls or the life safety system. The generator was able to

transfer power to all systems manually.
