• Project Safety Support Department– Review, accept, or reject subcontractor

safety documents including fall protection space program

• Procurement– Withhold notice to proceed (NTP) until

above documents have been accepted• Safety Services Group

– Provide assistance as needed in the field

• Elevated Surface Work Plan– Over 6 feet in

height from grade, floor, or other fixed working surface

– Is unprotected by guardrails, parapet, or other fixed barrier

– Using conventional fall protection equipment

• If conventional fall protection is not feasible the workers must set up a control line at a minimum of 6’ from the leading edge

Control Line Requirements Erected not less than 6’ nor more than 25’ from leading edge Extend entire length of leading edge Be flagged or otherwise clearly marked at not more than 6’ intervals Supported so the lowest point is not not less than 39” or mort than 45” Have a minimum breaking strength of 200 lbs. Minimum stanchion knock over strength of 16 lbs.

The Hierarchy• Fall Prevention

If work at heights is unavoidable, secure the elevated work area with a barrier system (such as guardrails) to prevent falls.

• Fall RestraintWhen a barrier system is not feasible due to

physical characteristics of the work environment or due to the infrequency of access, use an approved fall restraint system. 1000 lb. anchorage required

• Fall Arrest with Rescue PlanIf no other option is feasible, a properly

engineered and approved fall arrest system can be used in conjunction with a rescue plan. 5000 lb. anchorage required

• Elevated Surface work Plan (ESWP)• Must be used when using Fall

Restraint and Fall Arrest and work in the Designated area

• Description, Location, Protection method, Workers Names, Drawing

• ESWP Author, Competent Person, Building Manager—Signatures Required

• Must have a rescue plan (this does not mean calling 911)

• Fall Protection Plan– Used when the Fall

Protection Competent Person determines that conventional Fall Protection will not work: EVIDENCE REQUIRED, DOCUMENTED, & ACCEPTED (by SLAC)

– Created by a Fall Protection Qualified Person (engineer) & implemented by a Fall Protection Competent Person

Fall Restraint & Arrest EquipmentFall Restraint & Arrest Equipment

• Must meet ANSI/ASSE Z359.1-2007 • Must be inspected by competent

person 2 times per calendar year• Must be inspected pre-use by

authorized or competent person• Worn/used per manufacturers

equipment to prevent injury• Fall arrest with an SRL

– You need a minimum of ~8 feet for the equipment to prevent injury
