Understanding TBDo you know all you need to know aboutTuberculosis (TB)?March is TB Awareness Month and

ER24 isurgingpeople to familiarise them-selves with the symptoms of the disease.Seek medical attention immediately if itis suspected that you have developed TB.TB is an infectious disease that spreads

through the air. It is caused by a bacteri-um called mycobacterium tuberculosis. It is spreadwhen someonewhohas activetuberculosis coughs, sneezes or spits. The bacterium is then inhaled by anotherperson.Symptoms include cough, fever, night

sweats, weight and appetite loss, tired-ness, weakness, chest pain and coughingup blood.High riskSpeakingabouthealthcareworkers,Dr

VernonWessels, from ER24 said: “Unfor-tunately they are by the nature of theirwork, at a higher risk of sustainingmanyinfections, including TB, than the rest ofthe population. This is due to them at-tending to patientswithundiagnosedanduntreated infections.“Any person, including trauma cases,

could potentially have untreated TB. Ifyou were inadvertently exposed, it is im-portant to take note of any symptoms thatmay appear, go for testing and have treat-ment started as soon as possible.”Healthcare workers should ensure

they wear the relevant mask when treat-ing patients.Masks“Healthcare workers are trained to use

universal precautions such as gloves andan N95 mask on any patient suspected ofhaving a respiratory infection,” saidWessels. He added it is standard practice for

healthcare workers to wear an N95 orFFP2maskwhen treatingTBpatients, es-pecially if the patient’s response to treat-ment is unknown.“If the patient does not require oxygen

therapy, it is practical for the patient towear an N95 mask when in close contactwith people while still infectious as thiscontains the TB germ and preventsspread to other people. It should be notedthat some N95 masks are fitted with anexpirationvalve tomake itmore comfort-able forwearers as a persondoesnot haveto exhale through the filtermaterial. Thistype of mask would however not work ifa TB patient wears it as their expired airwith theTBgermswould exit through thevalveand into theatmosphere,” saidWes-sels.IfuntreatedTBis suspected, thepatient

should be approached with an N95 maskand preferably taken to an area wherethey will not be in close contact with oth-er people.Caroline Walker, the Advocacy, com-

munication and social mobilisation co-ordinator at the TB/HIV Care Associa-tion, said: “If your immune system isstrong enough, you can inhale the TBgerm but remain healthy. Many peoplehave latent TB. They have the TB germ

but their immune system protects themfrom its spread. Latent TB is not infec-tious. There is a 10%risk of developingactive TB if you have latent TB. If yourimmune system isnot strong enough, youcan develop active TB,” said Walker.TB usually affects the lungs however

it can affect other parts of the body.People who are HIV positive, diabetic,

malnourished or smokers have a higherchance of developing TB.“The HIV virus weakens the immune

system.Thismeans it ismore difficult forthe body to fight off the TB germs and ac-tive TB disease develops. The most com-mon cause of death for HIV positiveadults in South Africa is TB. The high co-infection of TB and HIV in South Africais the reason TB/HIV Care Associationworks on integrating TB and HIV servi-ces,” Walker said.Even though co-infection is high in

South Africa, this does not mean that ev-eryone with TB is HIV positive or thateveryone who is HIV positive has TB. Itis possible to have one disease withoutthe other. TB/HIV Care Associationworks to prevent, find and treat TB andHIV in parts of KwaZulu-Natal and theWestern, Eastern and Northern Cape.“We take services to people instead of

waiting for them to access care at clinics. We make it easier for people to stay incare by providing support for them totake treatmentat theirhomesorworkpla-ces.This isdone throughcommunitycareworkerswho forma linkbetween theclin-ic and the patient, visiting the patient athome, explaining their medication andreminding them of their clinic appoint-ments. Ourmobile teams provide screen-ing for TB and HIV counselling and test-ing in community settings,” saidWalker. She urged people diagnosed with TB to

take their medication as prescribed bytheir doctor or nurse until their treat-ment is completed.“For the first two weeks of treatment

they should avoid close contact with oth-ers. After the first two weeks of takingtreatment, a person should no longer beinfectious. TB patients must continuetaking their tablets until the treatmentis completed.”TB is more likely to be spread in small,

poorly ventilated and overcrowded con-ditions. “This means everyone shouldopenwindowswhenona train, busor taxifor example to allow for ventilation.“Coughing etiquette should be ob-

served. People should turn away fromothers and cough into their arm ratherthan their hand to avoid coughing germsinto the air. If you spit, you should do sointo a tissue and then throw the tissueaway,” said Walker. She added that kissing, shaking hands,

sharing food and cutlery or touching bedlinen or toilet seats, do not spread TB. Tobecome infected, a person has to inhaleTB germs.ER24’s Emergency Contact Centre can

be reached 24 hours a day on 084 124 forany medical emergency.

Head injuries – You are at riskDidyouknowthatheadinjuryisoneof themost common causes of deathin transport-related incidents? It is also a common cause of death

among pedestrians who areknocked by vehicles.ER24 attends to patients who sus-

tain head injuries during collisionson a daily basis.March 20 was World Head Injury

Awareness Day and ER24 is urgingpeople to use safety belts, refrainfrom speeding and driving reckless-ly, to use child car seats and be alertwhile on the road. This will assistin limiting the likelihood and im-pact of head injuries.Speaking about all road users, Dr

Vernon Wessels, from ER24, said,“As with most injuries, preventionis better than cure. The best way toreduce head injuries is to try andavoid the impact in the first place.“Where potential impact is una-

voidable, wear the appropriateheadgear. Use the appropriate re-straint in vehicles. Using a car seatfor children that is correctly fittedprotects them from injury. It alsoprotects otheroccupants frombeinghit by an unrestrained child in theevent of a collision.”

People must seek medical atten-tion if a head injury leads to loss ofconsciousness or coordination, vi-sual disturbances and loss inmemo-ry, no matter how short the effectsmay have been.“Life-threatening brain injuries

can initiallypresentvery subtle anda medical assessment will deter-mineiffurthertestsoradmissionforobservation are required. Bleedingand swelling of the brain is poten-tially treatable with good outcomesif donebefore serious complicationsoccur.“The most catastrophic event is a

missed bleed inside the skull cavitythat can cause severe disability ordeath.“There are, however, subtle ef-

fects thatcanpresentata laterstage, suchaspersonality changes, coordi-nation problems, speech difficultyand memory and concentrationproblems,” said Wessels.ChildrenWessels said in a perfect world,

any head injury should be assessedby a health professional, howeverwe also need to be realistic. “Chil-dren will sustain minor head inju-ries mainly due to the relatively

large size of the head during earlychildhood and immature motorskills.

“However, there are certain signsthat can be used to determine whena medical consultation is appropri-ate.

“These include any loss of con-sciousnessnomatterhowbrief, con-vulsions after the injury, vomiting,a headache thatworsens or does notimprove with rest or oral paraceta-mol and any visual disturbances orbehaviour that is not typical for thechild. If there is any doubt, ratherconsult a doctor,” said Wessels.Understanding head injuriesWessels said head injuries could

range from simple lacerations todevastating bleeding inside theskull cavity that causes pressure onthe brain.

This pressure can cause death ifthe patient is not urgently treated.

“Although injuries involving theskullcavityareusually themostcat-astrophic, external head injuriescanalsocausepermanentdamageordeath especially if the airwayorma-jor blood vessels are affected,” hesaid.

Types of head injuries include

concussion, contusion and skullfractures.ConcussionAconcussion is themost common

typeofhead injury.Aconcussionre-sults when the brain undergoes im-pact that shakes the brain tissue butdoes not cause bleeding or visibledamage to the cells.“A patient may lose conscious-

ness for a short while and is oftenconfused, disorientated or has poorco-ordination. However, the effectsreturntonearnormalfairlyquickly.“Some functions like short-term

memorymaytakemuchlongertore-cover and headaches and dizzyspellscanlast formonthsafteracon-cussion episode. By repeating ques-tionnaire tests over aperiod of time, doctors are able to track the recov-eryprocess.Althoughthebraincellsdonotappearabnormal (aCATscanwill be normal), the functioning ofthecellsareaffectedandarepeat im-pact could have a more serious re-sult,” said Wessels.Usually, aconcussionissustained

through impact to the head, howev-er indirect force can also cause this.A sad example of bleeding that canoccur in the brain of a child is

throughviolentshakingas typicallyoccurs in abuse scenarios.ContusionWith contusion, the brain tissue

isbruisedbythe impactandthiscancauseswelling.Asthebrainisinsidea bony cavity that cannot expand, pressure will build up due to theswelling.Thispreventsadequatebloodsup-

ply to the brain, which suppressesits functioning and can cause per-manent damage to the cells and po-tentially lead to death. The same re-sult occurs when there is a bleed in-side the skull cavity.Skull fractureWithaskullfracturethereisarisk

of bony fragments causing directdamage to the underlying brain tis-sue. In addition, skull fractures areoften associatedwith awoundof theoverlying skin that then causes apathway for germs to travel fromoutside into the skull cavity andcause infectionof thebrain (enceph-alitis) or the membranes coveringthe brain (meningitis). This infec-tion can be life threatening or causepermanent disability.ER24’s Emergency Contact Cen-

Make your own luck in your businessDAVID MALHERBE

Somehowmany people have the desire tobe lucky in some way or another. That iswhy many people regularly buy Lottotickets.

“If I can only win the Lotto, then all myproblems will be solved.” But that does notseem to be true. I recently read in the Youmagazine about a man who won R100m inthe Lotto, spent it all within two years andnow says he wishes that he never won it, be-cause it brought himno luck, rather the con-trary.

According to articles on Entrepre-neur.com, it seems that the roll-of-the-dicekind of luck does not exactly work in busi-ness, rather themake-your-ownkindof luck.Much emphasis is placed on the impor-

tance of preparation. People say that luck iswhen opportunity meets preparation.Adam Toren says we need to be prepared

in three ways.First, we need to always be ready for an

opportunity when it comes our way by hav-ing business cards available, to sell our-selves or our business’ products or servicesand by always looking our best.Secondly, be prepared to ask for an oppor-

tunity to present a business proposal whereyou can supply a needed product or service.Don’t be afraid to get a “no.” If you do notaskyoualreadyhavea “no”without anypos-sibility of a “yes.”Thirdly, always show gratitude. Many op-

portunities andmuch “luck” will come yourway simply by saying thanks. This unfortu-nately is rare. In Luke 17 we read how Jesushealed ten lepers, yet only one returned tothank Him. Much goodwill will come yourway, simply by living a life of gratitude. On the other hand, Steve Tobak reasons

that“luck”oftenhappenswhereopportunitymeets desperation. According to him we are

mostopentonewideaswhenoursurvival isatstake. Oftenwhen peo-ple feel hopelessthey’ll adopt the “I’lltry anything, I’ve gotnothing to lose” mind-set. Necessity is themother of invention. Many successful businesseswere born out

of necessity and desperation, yet some peo-ple deem the owners “lucky.” Perhaps our“luck” is in our own hands.
