In some cases, the facilitators have the same health conditions & goals as you. Use this booklet to learn about each of the programs offered, including program benefits, what you can expect to learn, and how others, just like you, have used these classes to help improve their lives!

1) Call the ADRC to speak with an Information and
Assistance Specialist about your fall risk concerns.
2) Meet with an ADRC volunteer (retired medical
professional) in your home to complete an in-home fall
risk screen. Together, you may find ways to prevent falls,
helping you stay safe & comfortable in your home.
Cost: No cost
“My mother fell while carrying a pack of water from the grocery store into the
house. She was really worried she would have to move to an assisted living or
nursing home. With the help of the ADRC, she had an in-home fall risk screen.
My siblings & I were shocked to learn of all of the services that would support
our mom to say in her own home and save her money/assets, which is what we all
wanted anyways. My mom lived another five years after the fall in her home,
happy and thriving.

Advance Directives & Beyond
As a member of the Prevention Coalition of Brown County, we support the goal to increase
awareness of the importance of Advance Directives. The ADRC, in partnership with local
experts, offer classes to help you understand the basics behind both the Financial and
Healthcare Power of Attorney (POA) documents.
Q: You may have heard the term Advance Directives, but what does it mean?
A: Advance Directives are legal documents that allow you to plan and make your own
healthcare & financial wishes known in the event that you are unable to communicate.
Q: What does it cover?
A: It consists of a Living Will and a medical (healthcare) Power of Attorney,
Financial Power of Attorney, and much more.
Q: Why does anyone over 18 need these documents?
A: To ensure your wishes are carried out and to avoid confusion or conflicts
among your family, friends, and healthcare professionals when you can no longer
speak for yourself or understand information needed to make these healthcare
and/or financial decisions if you are unable to do so.

“Parenting a child with special needs takes everything you’ve got. It’s an
incredible journey and there’s no map. My child’s needs and taking care of
myself are constant battles. Powerful Tools provides perspective on these
