recovery, wellbeing

and a satisfying

life is possible and

achievable for all.

OUR VISION

Partners in Recovery is a new initiative available to people in the Sutherland Shire and St George local government areas.

PARTNERS IN RECOVERY ASSISTS PEOPLE WHO:

• Experience ongoing challenges with social and emotional wellbeing

• Find it difficult to access the supports and services that they want

• Want support that will prioritise their choices and personal autonomy

• Want to improve their social and emotional wellbeing.

Anyone can make a referral to Partners in Recovery

A Partners in Recovery worker can meet with you in a place where you feel comfortable, to discuss the program.

Partners in Recovery is managed by South Eastern Sydney Medicare Local, delivered by local organisations and funded by the Australian Government.South Eastern Medicare Local acknowledges the traditional owners of the land on which we work and pays respect to the Aboriginal Elders past and present.

ONCE REFERRED TO THE PROGRAM, PARTNERS IN RECOVERY STAFF, CALLED SUPPORT FACILITATORS, CAN:

• Work along side you as you plan your personal recovery

• Connect you to services and support that you want, and

• Assist in coordinating services and supports so they are working together to support you

Support Facilitators also build strong partnerships in the community that make it easier and better for you to access what you want.
