endo case, every canal is unique and presents

with its own set of challenges. The only

commonality is knowing that practicing with

the right tools – ones that can provide

consistent and superior results – can make

the difference between success and failure.

CLINICAL RESEARCH DENTAL is your source

BL TIPS Ultrasonic tips with Micro-projections BL Tips micro-projections

Conventional endo tips with diamond-coating

The last 15 years have seen a revolution in the use of ultrasonics for endodontic procedures. Though the integration of ultrasonics made practicing endodontics undoubtly more efficient, it is not without drawbacks – tip breakage, rapid wear of diamond surfaces, and high costs are not uncommon.

BL Tips are specifically designed for conventional or micro-endodontic treatment: locating concealed orifices, removing secondary and calcified dentin inside the cavity or pulp stones, as well as for a variety of re-treatment procedures. Unlike conventional diamond-coated endodontic tips, BL Tips have sharp abrasive micro-projections integrated into the tip that are ideal for removing dentin, as well as for locating MB2 canals in maxillary first and second molars. BL Tips have a significantly longer life span than diamond coated tips, providing low wear and durability as well as ensuring biocompatibility due to their uniform cutting projections.

Irri-Safe Tips passive Ultrasonic irrigation instruments

Manual canal irrigation may not be cleaning as effectively as you think. Complex root anatomy may not allow the irrigant to effectively clean and disinfect the entire canal – as well as does not allow for smear layer and dentin debris removal, which will enhance the sealing ability of resin-based sealers.

Irrigate passively, ultrasonically, and safely with Irri-Safe Tips from Acteon. The Irri-Safe Tip cleans in a way manual irrigation simply cannot. And while Irri-Safe looks like a file, its small tip is actually rounded and non-cutting, never engaging the canal wall. Placed 2mm from the apex for 1-3 minutes, the tip cleans both by cavitation (ultrasonic stimulation of the NaOCI to implode, removing biofilm) as well as acoustic streaming (flutes create multiple waves pushing the NaOCI against the canal walls to dislodge debris). Irri-Safe Tips can be used with any Satelec piezo-electric ultrasonic unit.

Do round burs lead you down the wrong path during endodontic access? EndoGuide Burs are a series of eight burs for non-surgical root canal treatment. Their patented conical-shaped, microdiameter tip acts as a self-centering guide for straight-line access to canals from an incisally positioned, vertical angle, maximizing treatment efficiency and conserving healthy peri-cervical dentin. The EndoGuide Bur’s funnel-shaped design improves canal visibility as well. As compared to a round bur, EndoGuide’s unique blade design polishes dentin to improve canal mapping.

•Tapered, micro-diameter tip creates an ideal glide path for endodontic files•Precision guidance for efficient identification of calcified MB2 canals• Conserves vital peri-cervical dentin to strengthen resistance to fracture

Two EndoGuide kits address the specific endodontic needs of the Anterior/Bicuspid teeth and Molar teeth. Individual EndoGuide Burs are also available.

Effervescent action of ChlorCid or Sodium Hypochlorite with File-Eze helps move debris from the canal

eDta chelating canal prep

Your file has reached working length, but now doesn’t move as freely and easily. Reach for File-Eze® – a 19% EDTA water-soluble gel that’s ideal for lubricating, chelating and debriding root canal preparations. It combats the filing debris that clogs the flutes of your file, as well as the build-up in the canal.

Peroxide-free, File-Eze will not affect the set of resin-based root canal sealers, which can lead to low bond strength and microleakage, while it’s pH of 11-12 makes it optimal for EDTA chelation. File-Eze’s water-based formula also ensures that it is easily removed from the canal.

File-Eze is delivered directly to the canal with a NaviTip tip, with the file entering the canal through the gel. Alternatively, the file can be dipped in File-Eze prior to entering the canal – guaranteeing a smooth path and ensuring the canal is clean.

Calcium hydroxide has long been recognized as an antibacterial agent due to its high pH as well as having the ability to potentially stimulate healing of bone. Because the goal of cleaning the root canal is to eliminate bacteria and remove necrotic tissue, UltraCal® XS – a 35% calcium hydroxide paste with 12.5 pH – is a natural choice for use as an interim endodontic dressing. It also doubles as an excellent emergency and pain management medicament.

Many medicaments used in endodontic therapy effectively kill bacteria – but with twice the calcium ion diffusion of other Ca(OH)2, what makes UltraCal XS stand out from the rest is its ability to stimulate bone healing and growth in cases with immature teeth, furcal involvement or a blunderluss canal.

It’s every clinician’s worst nightmare – an NaOCI accident. Traumatic for your patient, these accidents cause pain and swelling, and can result in a serious decline of patient trust and confidence.

Minimize your risk with NaviTip 31g Sideport Tips from Ultradent. The tip’s unique sideports feature a flexible end that’s capped and rounded for safety in the apical third. Irrigants are expressed through non-clogging dual sideports, which are offset from each other vertically as well as laterally, minimizing the risk of expressing potentially harmful chemicals past the apex.

Irrigate with dentistry’s thinnest needle, the NaviTip 31g Sideport Tip and you will find the thinness of the tip maximizes reflux space in even the most intricate of canals, resulting in efficient irrigant movement and exchange.

CONSEPSIS® 2% Clorhexidine Antibacterial Solution cleans and disinfects in one step. Recommended for procedural endodontic disinfection, as a final endodontic rinse prior to canal obturation, and prior to pulp capping, CONSEPSIS boasts an easy syringe delivery.

CONSEPSIS should be used after smear layer removal for canal disinfection, and sodium hypochlorite should be rinsed and removed prior to using.

NOTE: DO NOT mix CONCEPSIS and Sodium Hypochlorite in the canal as a harmful precipitate will form.

INFINITE FLEX NiTi FILESFile separation is an inherent risk during any root canal procedure regardless of your experience level or expertise, and can occur without previous indication. A file separating in the canal certainly complicates things and causes frustration for you (and your patient) with a possible referral out to have the file retrieved and the root canal completed.

300% More resistance to separationCyclic fatigue and torsional stress are two primary causes of file separation. The inherent memory of traditional NiTi files accentuates torsional forces and promotes cyclic fatigue. In the past, the more aggressive cutting ability of a NiTi file has always corresponded to a higher propensity for file separation due to stress points along the file. Not anymore!

In a recently published university study that compared TYPHOON™ Infinite Flex NiTi Files with a market-leading traditional NiTi file, TYPHOON files proved to be 300% more resistant to fatigue stress.1,2 TYPHOON files do cut aggressively, but thanks to memory-free Controlled Memory NiTi™ Technology, they have up to three times the torsional strength, so your risk of separation is dramatically reduced. With TYPHOON you’ll experience far superior cutting to what you are accustomed, and you’ll gain more confidence – especially in cases with curved canals.

controlled Memory niti technologyTraditional NiTi files are far more flexible than rigid stainless steel instruments; however, this flexibility means they will always try to straighten when flexed within the canal. This causes unbalanced lateral forces along the canal wall and often results in ledging, as well as excess tooth structure being unnecessarily removed.

TYPHOON files are constructed with revolutionary Controlled Memory NiTi technology, which exhibits virtually no memory. With TYPHOON, the file’s lateral forces are balanced throughout the length of the file, making canal navigation effortless – regardless of the curvature of the canal. And, because TYPHOON files adapt perfectly to the canal path, they guarantee precise and conservative removal of tooth structure.

Finished case instrumented with Typhoon, obturated with Alpha and Beta Obturation units.

X-PLORER™ Canal Navigation NiTi Files help negotiate working length quickly and efficiently. More durable and efficient than typical stainless steel hand instruments, X-PLORER files are capable of negotiating tortuous anatomy. Use the X-PLORER series after a #8 or #10 hand file. X-PLORER file 15/.01 will navigate the canal easily and effectively because it’s more flexible than a #15 hand file – and half the taper. Follow with 20/.01 and 20/.02 X-Plorer files and, optionally, X-FACTOR size 25/.02 for an excellent reproducible glide path.

The Typhoon file’s ability to maintain original canal anatomy is staggering. Even in S shaped canals and sharp curvatures transportation is negligible. My cleaning and shaping protocol has been greatly enhanced both in efficiency and appropriate apical prep size/shape with Typhoon files. Patient’s with limited jaw openings are far easier to treat and produce a quality end result with the controlled memory Typhoon files.

EndoREZ Monoblock: radiopaque, easily diagnosed, suitable for retreatment and post and core.

Dentin-EndoRez-gutta percha interface EndoREZ inflitrates dentin and bonds chemically to

gutta percha for a complete seal of the canal.

Hydrophilic EndoREZ flows into dentinal tubules for a complete seal.

Achieving adequate obturation using common techniques such as lateral condensation and Thermafil* carriers, though seemingly simple can present several challenges. Carrier obturators are often unable to completely fill ovoid canals.** There is also the risk of gutta percha stripping off the carrier during insertion. Extremely difficult to retreat, carrier-based obturators require removal of the carrier when a post is required – making post and core cases much more time consuming and difficult. Lateral condensation has a long history of success; however, the process is time consuming and the forces created during lateral condensation have the potential to damage roots.

endoreZ: true simplicityThe technique sensitivies that come with other obturation methods are all non-issues when you reach for EndoREZ. Even in ovoid canals, EndoREZ attains an impervious seal – without unintentional carrier/wall contact which results in gaps or leakage.

resin sealer obturationUnlike any other sealer, biocompatible urethane dimethacrylate (UDMA) resin-based EndoREZ is hydrophilic and self-priming. Delivered apically, it penetrates to the apex and into lateral canals, literally chasing the moisture into the dentin tubules preventing leakage, assisting in microbial control, and creating a durable monoblock. And, since methacrylate-based EndoREZ relies on chemistry rather than heat or pressure to fill the canal, the risk of additional root trauma/fracture is greatly reduced.

The superior flow of EndoREZ makes it a perfect obturation technique, able to penetrate even the more intricate of canal shapes and sizes. Deliver EndoREZ either directly through the mixing tip, or via a Skini Syringe and 29g NaviTip, passively place a master EndoREZ Point (resin-coated gutta percha) into the EndoREZ filled canal, adding accessory points if required and cure. With EndoREZ, the result is always predictable... fills that are radiopaque, extremely easy to re-treat (if necessary) and compatible with all endo-restorative and post and core procedures.

EndoREZ®DUAL-CURE

EndoREZ® Acceleratorneed to do a same-appointment post and core? Dip an EndoREZ Point in EndoREZ Accelerator just before placement in the canal, and you’ll reduce EndoREZ’s set time from 15-30 minutes to a mere 4-6 minutes, on average. EndoREZ Accelerator is 100% biocompatible.

The primary clinical challenge with all injectable obturation procedures is the angle of entry to the canal, especially on maxillary cases. Cordless and compact, with the BETA Cordless Obturation Gun in hand, you are in control. The BETA’s patented 360° swivel needle permits the clinician to access the canal at any angle, and with ease. No more awkward gun or hand positions, no more stopping midway through the procedure to adjust the needle angle, and no more needle breakage.

With four accurate temperature pre-sets and an instant on/off at your fingertips, you control the consistency and temperature of the gutta percha or Resilon™ at all times during obturation, without wasting battery power. Lightweight and balanced for comfort, the BETA is ultra durable – withstanding the wear and tear of repeated use. This virtually maintenance-free unit holds an incredibly robust battery – providing 4 hours of continual use on a single charge, and features a well-insulated heating chamber that keeps the gun surface cool to the touch, for your comfort as well as your patient’s.

Endodontic outcome studies suggest that warm vertical compaction gutta percha techniques enhance success rates.1 Further “warm vertical compaction using the continuous wave of condensation technique...resulted in a greater gutta percha fill by weight compared with standard cold lateral compaction.”2

MTAfrom

Hands-down, MTA is the most biocompatible material used in endodontics. It promotes formation of a dentin barrier, and is the only material that allows osteoblasts to attach and spread from a cementum barrier.1

On the world market since 2001, only MTA Angelus is scientifically and clinically proven in numerous international dental journals, and in several University endodontic programs all over the world.

With a calcium hydroxide containing formula for incomparable biological healing, MTA Angelus also exhibits ideal handling properties, sealing capability, radiopacity, high alkalinity, low solubility, and complete biocompatibility. Indicated for procedures such as direct pulp capping, pulpotomies, furcation perforation, root canal perforation, resorption, and apexification, MTA Angelus offers a quick 10-15 minute set time (versus 2 hours on average for other MTA materials on the market). Intentionally packaged to ensure both economy as well as little to no wasted material, MTA Angelus is available in both 1g and 2g kits, in both grey and white shades.

Mta Fillapex seals and healsMTA Fillapex is dentistry’s first paste:paste MTA-based resin root canal sealer, versatile for every obturation method. It delivers easily and without waste, and exhibits excellent handling properties with an efficient setting time.

Half of MTA Fillapex’s formula contains 13.2% MTA, unmatched in its biological healing capabilities,1,2,3 and providing an impressive hermetic seal in which the MTA particles expand, preventing micro-infiltration. And, the MTA simultaneously releases free calcium ions to accelerate the healing process by stimulating the regeneration of the adjacent tissues.

The other half of MTA Fillapex’s formula contains biologically compatible salicylate resin which is tissue friendly, and therefore a better choice over epoxy-based resins, which have been shown to have mutagenic and more cytotoxic effects.

MTA Fillapex has a high flow rate and a low film thickness, so it easily penetrates the lateral and accessory canals. Regardless of your obturation technique – warm vertical, continuous wave, carrier-based – MTA Fillapex confidently delivers high sealing capability that, unlike other sealers, is not adversely affected by heat. Radiopaque, and easily removed should re-treatment be necessary, MTA Fillapex provides 35 minutes of working time – perfect for cases with multiple root canals.

Super tear-resistant!

When other rubber dams reach their tear threshold, extra strong 6” x 6” PARO® resists, ensuring a contamination-free field for successful endodontic treatment. Latex-free. Tear-resistant. Powder-free. PARO is manufactured from non-latex rubber and is engineered to contain zero sensitizing proteins, making it the purest of rubber dams, so allergic reactions are averted.

OraSeal®

caUlking anD pUttY

Inadvertently tearing your rubber dam during placement or gaps that result around teeth that are too difficult to properly seal can lead to bacterial contamination and compromise the final result of your endodontic procedure. Reach for OraSeal® – and seal out moisture and bacteria for a truly isolated working field.

OraSeal’s one-of-a-kind cellulose formulation adheres not only to the wet dam, but also to wet gingival tissue and mucosal tissues, wet teeth, and wet materials – even under water and saliva. Apply OraSeal Caulking with a Black Mini or White Mac delivery tip to prevent leakage during endodontic treatment. Shape with a wet gloved finger, wet cotton swab or instrument. Root canal therapy is performed in a clean, dry field.

Use OraSeal Putty if you prefer a stiffer consistency.

Big and bulky countertop apex locator units take up precious counter space and require additional sterilizing between patients, while long wires runningbetween the main unit and the patient increase the probability of distortion of the signal. Uncomplicate your endo with the APEX NRG XFR.™

The compact and lightweight APEX NRG XFR incorporates digital signal processing plus xtra fine resolution. Other comparable locators use an analog signal which can be diminished or distorted in the time it takes to travel through long wires. The APEX NRG XFR converts analog signals to digital and transfers them through short cables – preventing external influences and distortions of weak and sensitive signals. A clear and accurate display of information is always the result.

The depth of the file’s progress is represented on the APEX NRG XFR by 8 multi-coloured LEDs, with indicators distributed every 0.25mm. An audible alarm starts at 0.5mm and alarm frequency increases as the biological apex is approached.

Predator® ZirconiaZirconia continues to gain popularity as a crown restorative material because of its superior strength. Cutting, removing, trimming and adjusting this material with traditional diamond burs has proven to be an unpredictable and time-consuming task. Often, tremendous force is required, which generates significant heat build-up, melting the nickel plating that holds the diamond particles to the bur shank. The result is poor cutting efficiency and the need for multiple burs. Specifically designed for this tough material, new PREDATOR® Zirconia burs provide maximum cutting efficiency, with less force, resulting in greater predictability.

Manufactured using a robust bonding technology process, PREDATOR Zirconia diamond burs allow for effortless access through tough zirconia in both endodontic treatment and crown removal. Each bur features a unique, black protective coating which insulates the nickel plating, ensuring a significant reduction in friction and heat build up, while maintaining cutting efficiency by keeping the diamond particles bonded to the bur shank. In most cases, only one PREDATOR Zirconia bur is required to access or remove the restoration and with up to 41% less force than competing zirconia cutting burs. Only premium natural diamond crystals are used on PREDATOR Zirconia burs, which results in superior penetration with low vibration, and a smooth finish on zirconia. Diamond particles have sharper cutting edges and are orientated to the most effective cutting angles to expose more cutting surfaces. Maximum performance, greater efficiency and superior longevity are ensured.

Internal bleaching is now fast, easy, and dependable with Opalescence® Endo. Opalescence Endo is a 35% hydrogen peroxide gel specifically formulated for the “walking bleach” technique when you are whitening endodontically treated, discoloured teeth. This special formulation – with its paste-like, viscous consistency – is sealed within the coronal portion of the pulp chamber, whitening the tooth from the inside out.

Prior to placing Opalescence Endo, a glass ionomer MUST BE PLACED to the level of the CEJ to prevent the peroxide from entering the pulp canals. This seal is necessary since studies have shown that peroxides can pass through the canal walls, causing bone resorption. Opalescence Endo is then back-filled in the chamber, leaving approximately 1.5 – 2.0mm for the placement of UltraTemp® Regular, a temporary luting/filling material, which will seal the gel in the chamber for 3-5 days. UltraTemp allows moisture to pass through it to rehydrate the tooth, but seals bacteria out.

no curing light required (activated By Body temperature) Opalescence Endo is left in the tooth for 3 to 5 days where it is activated by body temperature. Afterwards, the endodontically treated tooth will be visibly whiter. Repeat if necessary until desired result is achieved.

The Opalescence Endo “walking bleach” technique requires very little preparation or chair time, making it both easy and cost-effective.

pre-mixed 1.2ml syringes for ultra convenience•Reduces chair time•Less material waste•Reduces risk of spillage•Easily and precisely dispensed

1. Place Opalescence Endo into canal.

2. Deliver mixed UltraTemp directly to site through the Skini Syringe.

3. Wipe excess easily with wet gauze or cotton swab before setting.

4. Finished. 5. Easily removed.

strengths

Fact is, not all core materials perform predictably in every clinical situation. While some materials claim to be dual-cure, they actually lose significant strength if not light-cured, compromising the strength and longevity of your restorations. Only dual-cure ZIRCULES™ has been clinically proven to maintain its high strength in both light and self-cure situations, providing predictable performance, even when the curing light depth of penetration is limited.

P.O. Box 28040, London, ON, N6H 5E1ODONTOPASTE®

New to Clinical Research Dental, ODONTOPASTE® is a zinc-oxide based endodontic dressing used to reduce pain, as well as to maintain a bacteria-free environment within the root canal. Containing the broad spectrum antibiotic Clindamycin Hydrochloride, as well as Triamcinolone Acetonide – a steroid-based anti-inflammatory agent, and calcium hydroxide, ODONTOPASTE temporarily obturates root canals and prevents bacterial repopulation within both the root canal and the paste itself, in addition to temporarily reducing inflammation and post-operative pain.

Water soluble ODONTOPASTE will not stain the teeth – alleviating the need for bleaching, and reducing your patient’s risk of external cervical resorption. There is also no need for subgingival application as ODONTOPASTE is not photosensitive.

indications For Use•Disinfection of dentinal walls•Pain relief associated with acute pulpitis and apical periodontitis•Interim pulpotomy dressing material for permanent teeth in cases of acute pulpitis where root canal therapy will follow•Following trauma, in prepared canals, to reduce the incidence of inflammatory root resorption•Reduction of existing inflammatory root resorption

ODONTOPASTE is simply a “must have” for any doctor who performs endodontics.

“I regularly use Odontopaste as an intracanal treatment for trauma cases. Odontopaste is also indispensible for patients with exposed pulp that are very sensitive. When I don’t have time to remove all the pulp, I perform a coronal pulpotomy and apply Odontopaste with a cotton pellet until I have time to do the endo. This allows the pulp to “die happy” and the patient is relived of pain almost instantly.” – Dr. Manfred Friedman, London, ON
