School Guitar Group Enrolment Form Dear Parents & Students,

Welcome to the Teaching Services Australia Guitar Program.

The program will be run weekly, during the school day, with half hour group lessons, at a cost of $180 per term (teaching fees); Also take note of section ‘Lesson Resources’. This is emailed to you at the start of each term. Generally guitar tuition may take place on the same day as regular band tuition. Guitar students are NOT required to attend morning band.

In order to participate in the program, each child will need the following:

• A Nylon String Acoustic Guitar (3/4 size to suit primary aged child) • A Lesson Book : Hal Leonard Guitar Method 1, with CD • A padded Guitar Bag, to carry Guitar to and from School • Picks, a necessity for playing the Guitar; and Tuner/ Metronome • Folding Music Stand highly recommended for home practice, but not essential

All of the above products can be purchased by promptly returning the (tear off) form below, within 7 days.

Terms & Conditions for Belonging to the TSA Guitar Group Fees and Payment Methods Fees – Guitar lessons are provided on a term-by-term basis with a term typically consisting of 9 -11 lessons or performance opportunities. Fees are payable at the commencement of each term and includes all tutorials and performances for the term. If a public holiday, pupil free day or other ‘whole school’ activity occurs and less than 9 lessons/performances are achieved, an alternative lesson/performance opportunity will be provided. Due to timetabling restrictions & varying term lengths, this extra lesson/performance may not always take place in the same term. Student Absences – please note that tuition is a ‘full term’ commitment and we employ teaching staff for a full term in advance. Due to the nature of group lessons, individual student absences cannot be made up. As such, no refund is provided for student absences.

Payment options are: a) Direct debit from your credit card, with receipt mailed to you OR

b) Invoice e/mailed to you at term commencement & payable within 14 days by phone with credit card details, or

by mail with cheque/money order/credit card details or EFT online. You may incur late fees or your child may be excluded from Group Lessons if unpaid. Minimum student commitment for teaching is one term. On an on-going basis one term’s notice must be given or one term’s fees will be due. (NB We will give consideration for hardship with prior notification.)

Service / Administration of the Program The office of the school your child attends will not be responsible for handling any TSA business. All communication shall be with TSA via either phone number and/or address above.

Lesson Resources In addition to a student’s individual accessories the guitar students also requires several ‘general’ resources such as sheet music, music stands, amplifiers, etc. This is covered by a $10 “Resource Levy” charged along with tuition fees each term. Some schools choose to cover the cost of this through fundraising (committee/P&C/P&F), for advice on your school’s status call TSA on 9651 7333.
