    Safety Observation Report

    Date: 04-05th November 2011

    Location: GAIL, Vagholia, Vadodara, Gujrat.

    Fire extinguisher to be installedwherever required.

    B Physical Audit PointsUnsafe Act

    1. Most of the workers & staff are unawareof proper use of safety helmet. Safety helmet should were withchin strip.2. Many workers are working at height

    without safety belt. (many of them arewearing on body but not anchoring toanchoring point)

    Every worker should wear andanchor safety belt if workingabove 2M of height.

    3. Some of the workers are workingwithout safety shoe.

    All workers should wear safetyshoe while working at site.

    4. Workers are doing grinding workwithout face shield. (They are usingsafety goggles).

    Worker should wear face shieldduring grinding work.

    Unsafe Condition1. No proper access is provided to manyof the elevated working platforms.

    All elevated working platformshould have proper access (e.g.ladder, staircase)

    2. All elevated working platforms arewithout handrail, mid-rail and toe guard.

    3. Most of the temporary electricalconnections are without plug tops (bare

    8. Hydra (GJ-6-DH-105) is operatingwithout hook latch, reverse horn, frontand rear lights & indicators etc.

    Every vehicle should bethoroughly examined beforedeployment.

    9. Many under construction staircases arewithout railing and workers are usingthose for work.

    All staircases should have properhandrail, mid-rail and toe guard.

    10. Cross bracing is not given to anyscaffolding structure. No skilledscaffolder is available at site.

    If scaffolding is more than 6M ofheight then every 6M crossbracing is required.

    11. Many web slings are in poor condition Good condition and tested slingsshould be use.

    12. Many of the gas cutting sets pressuregauge are not in working condition

    All pressure gauges should be inworking condition to avoid backfire.

    13. Fabricated reinforcement man bucketsare using at site for welding work atheight. Also no proper access is givenfor man bucket.

    Standard man buckets should beuse for height work.

    14. No lifeline provided for horizontal andvertical movement on elevatedstructure.

    Proper lifeline with fall arrestorshould be provided for vertical aswell as horizontal movement onelevated structures.
