The Effects of ArthritisUnfortunately as we get older the effects ofworking in a repetitive way, often with poorposture, can show themselves in the form ofarthritis. According to figures from theArthritis and Rheumatism Council over 1.5million people in the UK currently sufferfrom the symptoms of osteoarthritis butover eight million have early X ray signs ofthe problem.

Our framework comprises 206 individual bones,most of which have a smooth covering ofcartilage known as the articular surface.Cartilage can withstand enormous forces andmechanical stress when the posture is good andthe work varied.

Modern pharmaceuticals can be vital in thetreatment of arthritis, and sometimes lifesaving in certain conditions but in the long-term management of osteoarthritis they have aquestionable part to play.

The prescription for arthritis is the drugibuprofen, which helps to control the pain andinflammation in rheumatic disease and othermuscle and joint problems, however, its long-term use is associated with a number of sideeffects, such as skin rashes and digestive upsets.Ibuprofen is one member of a large drug familyknown as nonsteroidal anti-inflammatory drugs(NSAIDs for short), all of which have similar sideeffects although some have more than others.

The Five Classic Early Warning Signs ofOsteoarthritisIf you can tick three or more of the followingthere’s a good chance that your joints areshowing the early signs of excessive wear andtear.

● Early morning stiffness in the joints thateases with movement.

● Bony swelling around the joint and areduced range of movement.

● Pain and/or tenderness around the joint.● Crunching or grinding sounds from within

the joints on movement.● Joint pain affected by a change in the

weather.

The conventional approachTreatment for arthritis will vary according tothe severity of the problem. Conventionalmedicine can offer powerful drugs to combatthe pain and swelling associated withdegenerative arthritis (osteoarthritis), howeverthey can have serious side effects when takenover a long period of time. The side effectsusually include nausea, diarrhoea, and stomachcomplaints (such as indigestion, ulcers, andstomach bleeding). Most patients withosteoarthritis only need simple pain killers suchas acetaminophen (paracetamol) but someprogress onto the nonsteroidal anti-inflammatory drugs (NSAIDs), for example,ibuprofen.

Although deciding whether to go into aresidential or nursing home is never easy,understanding what the choices are canhelp you to reach the decision that is bestfor you.

If you decide to move into residential careyou will usually live in either a residentialhome or a nursing home.

In a residential home you should get helpwith personal care if you need it, includinghelp with things like washing, dressing andgoing to the toilet.

Staff can also care for you during shortillness, but if you require constant nursingcare - for example if you are frail, or bed-ridden or if you have any sort of medicalcondition or illness that means you need alot of attention from a doctor or nurse, thenyou would probably require a nursing homewhich can provide 24-hour nursing care.

Residential homes have to register with thelocal authority, while nursing homes areregistered with the health authority. Someare dual-registered and offer a wide range ofcare both for people who need a lot ofnursing care and those who don’t need thissort of attention.

Residential homes can be run directly bylocal authorities, by voluntary organisationsor privately, while nursing homes are usuallyrun privately or by voluntary organisations.

Under Community Care legislation, yourlocal authority is responsible for arrangingand paying for your place in residential care -and for collecting a payment towards thecost from you, based on your income andsavings - but it is not a case of you having tomove where you are told there is a place(although if the cost is more than theauthority would normally pay, someone else- usually a relative or charity - may make up

the difference).

As you have the right to choose which homeyou move into, you will probably want tofind out what homes there are in your areaand visit a few to get an idea of what theyare like.

Once you’ve found out about homes in yourarea, get in touch with a few and ask themto send you a brochure, then arrange to visitsome of the homes that appeal to you. Youmight want to visit with a member of yourfamily or a friend, as a second opinion can bevery useful.

When you visit a residential or nursing homethere are lots of things you might want tolook for and ask about, including the range offacilities there are.

Don’t be embarrassed about askingquestions - make a list before you go of thethings you want to find out about.

■ General things to think about include:■ Can you bring your own furniture and

personal possessions?■ Will you be able to make and receive

‘phone calls in private?■ When are meals provided and can you

make yourself a cup of tea or a lightsnack when you feel like it?

■ Are there activities organised and can youcarry on doing the things you enjoy atthe moment?

■ Can you go to bed and get up when youdecide to?

Before you make a final decision, you couldalso ask to look at a recent inspection reportwhich contains a lot of information abouthow a home operates.

It is only sensible to do as much research asyou can when you are making such animportant decision about your future.

Chosing a care home

Men in RetirementDuring their 40s and 50s most menexperience some symptoms of what isnow known as the male menopause.

Although these symptoms may be unlikethose of female menopause, in the sensethat they don’t include hot flushes or nightsweats, they can include headaches, memoryloss, depression, exhaustion, lethargy, loss ofconfidence, loss of decision-making abilities,and a general feeling of being run down. It isnot yet really understood why this happens.

The overall level of the male sex hormonetestosterone does not decrease significantlyuntil men are in their 60s, but it may be thattestosterone stops working efficiently whenmen are middle-aged, due to some as yetundetermined factor. Or midlife crisis mightbe a purely psychological condition.

AGE CYMRU is urging campaigners aged60 and over across Wales to put theirnames forward for a special award.The Sheila McKechnie Foundation ‘TakeAction’ award recognises theachievements of campaigners aged 60and over.
