Who is eligible to enroll?

∗ All full-time employees working 30 or more hours per week are eligible to participate and their benefits will be effective from 1st of the following month after 60 days of employment and they can enroll their dependents. Your eligible dependents are your lawful spouse or domestic partner and your children from birth until age 26, through any age if handicapped and unable to earn a living, or until they can no longer be legally declared as dependents. Dependent age and status requirements may vary by state.

Kemper Worksite Benefits

� Accident Expense Insurance - helps offset unexpected medical expenses such as hospitalization, emergency room care, concussions, lacerations and more by providing a cash payment directly to you. Option to purchase $3,000 or $5,000 annual benefit

� Critical Illness Insurance - helps to protect your assets in the event of a critical illness such as heart attack, stroke, cancer, paralysis, etc. This benefit supplements traditional medical coverage and provides a lump-sum benefit payment upon diagnosis, paid directly to you, for a covered critical condition. The cash benefit is paid out with no restrictions on its use. Policy also includes a $50 wellness benefit which could be used for health screenings. Guarantee issue benefit of $10,000 with option to purchase up to $50,000 with simplified issue

� Whole Life Insurance - benefit builds cash value at a guaranteed interest rate of 4.5% for the life of the policy. Premium will never increase as long as your premiums are paid. Guarantee issue benefit of $100,000 with the option to purchase up to $150,000 with simplified issue
