QUALITATIVE AND QUANTITATIVE COMPOSITION

Each hard gelatin capsule contains cefadroxil monohydrate equivalent to 500 mg

cefadroxil .

Each coated tablet contains cefadroxil monohydrate equivalent to 1 gm cefadroxil .

Each 5 ml of dry syrup after reconstitution contains cefadroxil monohydrate equivalent

to 125 or 250 or 500 mg cefadroxil.

CLINICAL INFORMATION

Indications

For the treatment of the following infections when due to susceptible microorganisms:

upper and lower respiratory infections,

skin and soft tissue infections,

urinary tract infections,

other infections: osteomyelitis and septic arthritis.

Dosage and Administration

Cefadroxil monohydrate is acid stable and may be administered orally without regard to

meals. Administration with food may be helpful in diminishing potential gastrointestinal

complaints occasionally associated with oral cephalosporin therapy.

Route of Administration

For oral use.

Adults

Urinary tract infections

For uncomplicated lower urinary tract infections (i.e., cystitis) the usual dosage is 1 or 2 g

per day in a single dose or in two equally divided doses.

For all other urinary tract infections the usual dosage is 2 g per day in two equally

divided doses.

Skin and skin structure infections

For skin and skin structure infections the usual dosage is 1 g per day in a single dose or

two equally divided doses.

Pharyngitis and tonsillitis

For Group A beta-hemolytic streptococcal pharyngitis and tonsillitis, the usual dosage is

1 g per day in a single dose or two equally divided doses for 10 days.

Upper and lower respiratory tract infections

For mild infections the usual dosage is 1 g per day in two equally divided doses.

For moderate to severe infections the recommended dosage is 1 g to 2 g daily in two

equally divided doses.

Children

The recommended dosage for children is 25 to 50 mg/kg/day in two equally divided

doses (every 12 hours) as indicated. For pharyngitis, tonsillitis and impetigo the

recommended daily dosage may be administered as a single dose or in two equally

divided doses (every 12 hours).

Elderly

No specific dosage recommendations or precautions for use in the elderly except

monitoring of patients with impaired renal function.

Renal impairment

In patients with renal insufficiency (creatinine clearance < 50 ml/min) the dose of

cefadroxil has to be adjusted in accordance with creatinine clearance:

Clinical trials with 5 anuric patients have shown that after an oral dosage of 1 g

approximately 63% of the dose will be excreted during hemodialysis of 6 to 8 hours.

Contraindications

Cefadroxil is contraindicated in:

Hypersensitivity to cefadroxil, to any of the excipients of the medicinal product

or to beta – lactam antibiotics.

Warnings and Precautions

Hypersensitivity reactions

Before starting treatment with cefadroxil it has to be carefully ascertained if the patient

has previously experienced allergy to cefadroxil, other cephalosporins, penicillin or other

medicinal products. Special care has to be taken when administering this medicinal

product to a patient sensitive to penicillin, as occurrence of cross-sensitivity between β-

lactam antibiotics has been clearly proven and this may manifest in up to 10% of patients

with penicillin-sensitivity. Administration of the medicinal product should be

discontinued if hypersensitivity occurs during administration of cefadroxil. Severe

allergic reaction might require intensive care.

Pseudomembranous colitis

Pseudomembranous colitis has been reported with nearly all antibacterial agents, and

may range from mild to life-threatening (see Section Adverse Reactions). Therefore, it is

important to consider this diagnosis in patients who present with diarrhoea subsequent

to the administration of antibacterial agents. After the diagnosis of colitis has been

established, therapeutic measures should be initiated.

Gastrointestinal conditions and colitis

Severity of undesirable effects on gastrointestinal tract decreases when the medicinal

product is administered with food; it does not affect absorption (see Section Adverse

Reactions). Cefadroxil should be prescribed with caution in individuals with a history of

gastrointestinal disease, particularly colitis.

Renal impairment

Cefadroxil should to be used with care in patients with impaired renal function (see Section

Dosage and Administration). Before and during the treatment necessary clinical and

laboratory tests should be performed in patients with known or suspected renal

insufficiency.

Superinfections

In case of long-term use of cefadroxil, the patient may be colonized with microorganisms

not sensitive to the medicinal product (see Section Adverse Reactions). Careful

monitoring of the patient is essential. If superinfection occurs during treatment, necessary

alterations should be made in the treatment scheme.

Direct Coombs test

The result of the Coombs' test can be transiently positive during treatment with

cefadroxil. In haematological studies or crossmatch tests for transfusion when

antiglobulin tests are performed on the minor side or Coombs testing of newborns whose

mothers have received cephalosporin antibiotics before delivery, a positive Coombs test

may be due to the drug.

False-positive glycosuria reaction

Urine from patients treated with cefadroxil may give a false-positive glycosuria reaction

when tested with Benedict's or Fehling's solutions. This does not occur with enzyme

based tests.

Lactose

Hard capsules contain lactose. Patients with rare hereditary problems of galactose

intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption should not

take this medicine.

Sucrose

Suspension prepared from granules for oral suspension contains sucrose. Patients with

diabetes must take this definitely into consideration. Patients with rare hereditary

problems of fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase

insufficiency should not take this medicine.

Interactions

Probenecid

The renal excretion of cefadroxil, and many other cephalosporins, is delayed by

probenecid.

Aminoglycosides and loop diuretics

Renal function should be monitored carefully if cefadroxil is to be combined with drugs that

have a potential for nephrotoxicity, such as aminoglycosides and loop diuretics.

Oral contraceptives

In common with other antibiotics, cefadroxil may affect the gut flora, leading to lower

oestrogen reabsorbtion and reduced efficacy of combined oral contraceptives.

Anticoagulants

Some cephalosporins may potentiate the action of coumarin anticoagulants. No

interactions of this type have been reported during the use of cefadroxil.

The molecule does not have the methylthiotretrazole structure recognized as responsible

for the hypoprothrombinemic effect of these cephalosporins.

To ensure safety in the case of prolonged treatment, an additional measurement of the

prothrombin rate in patients on anticoagulants is recommended.

Antibiotics

Bacteriostatic antibiotics taken concomitantly may hinder the action of beta-lactams.

Pregnancy and Lactation

Fertility

There are no relevant data available.

Pregnancy

Cefadroxil should be used during pregnancy only if clearly needed.

Reproduction studies have been performed in mice and rats at doses up to 11 times the

human dose and have revealed no evidence of impaired fertility or harm to the foetus due

to cefadroxil. There are, however, no adequate and well controlled studies in pregnant

women.

Lactation

Cefadroxil is secreted into breast milk; therefore, this drug should be used with caution

during lactation.

Ability to perform tasks that require judgement, motor or cognitive skills

Cefadroxil has no remarkable influence on the ability to drive and use machines.

prothrombin time, eosinophilia, Coomb’s test positive, leucopenia, haemolytic anaemia

One case of haemolytic anaemia was observed during treatment with cefadroxil from a

patient with a confirmed history of penicillin allergy; a positive Coombs test and red-cell

antibodies were present.

No antidote exists and treatment should be symptomatic.

For absorption of quantities greater than 250 mg/ kg, it is recommended to perform

emesis induction.

Mechanism of Action

Cefadroxil is a cephalosporin administered orally that inhibits the synthesis of the

bacterial wall of active mitotic cells by binding to one or more of the penicillin-binding

proteins. This results in the formation of a defective cell wall that is unstable from the

point of view of osmosis and bacterial cell lysis.

Pharmacodynamic effects

The antibacterial activity depends on the time during which the free concentration in

serum exceeds the MIC.

Mechanism of resistance

Cefadroxil may be active against organisms producing some types of beta-lactamase, for

example, TEM-1, in low to moderate amounts. However, it is inactivated by beta-

lactamases that can hydrolyze efficiently cephalosporins, like many of the extended

spectrum beta-lactamases and chromosomal cephalosporinases, such as AmpC enzymes.

We cannot expect cefadroxil to be active against bacteria with penicillin-binding proteins

that may have a reduced affinity for beta-lactams. Resistance may also be mediated by

bacterial impermeability or by bacterial efflux pumps. More than one of these four modes

of resistance may be present in the same organism.

In vitro, the first-generation oral cephalosporins are less active than penicillin G and V

against Gram-positive microorganisms and are less active than amoxicillin against H.

influenzae.

Breakpoints

The clinical breakpoints to differentiate susceptible pathogens (S) and resistant pathogens

(R) according to EUCAST (25/05/2009, version 2.0) are:

- Enterobacteriaceae: S ≤ 16 mg / L; R > 16 mg / L - for uncomplicated urinary

tract infections only;

- Staphylococcus: The sensitivity of staphylococci to cephalosporins is inferred by

the sensitivity to methicillin;

- Streptococcus A, B, C, G: The sensitivity of groups A, B, C and G streptococci

can be inferred by their sensitivity to benzylpenicillin.

Sensitivity

The prevalence of acquired resistance may vary geographically and over time for selected

species and local information on resistance is desirable, particularly when treating severe

infections. If necessary, advice of a specialist should be sought when the local prevalence

of resistance is such that the utility of the agent, in at least some types of infections, is

questionable.

Pharmacokinetics

Absorption

Cefadroxil is rapidly absorbed after oral administration. Absorption characteristics are

not different between fasted and non-fasted subjects.

Distribution

Following single doses of 500mg and 1g, average peak serum concentrations were

approximately 16 and 28 μ/ml respectively. Measurable serum levels were present 12

hours after administration.

Metabolism

The elimination half-life is about 2 hours.

Elimination

Over 90% of the drug is excreted unchanged in the urine within 24 hours. Peak urine

concentrations are approximately 1800 mcg/ml during the period following a single 500

mg oral dose. Increases in dosage generally produce a proportionate increase in

cefadroxil urinary concentration. The urine antibiotic concentration, following a 1 g dose,

was maintained well above the MIC of susceptible urinary pathogens for 20 to 22 hours.

Ibidroxil 1 gm Tablet: 2 strips of 4 tablets and inner leaflet .

Ibidroxil 125, 250, or 500 mg powder for oral Suspension: bottle of 60 ml of dry powder,

each 5 ml contains 125, 250, or 500 mg cefadroxil and inner leaflet.

Use and Handling

Oral suspension

Shake or tap the bottle to loosen the powder. Add water until the reconstituted suspension

is at level with the line on the bottle. Invert and shake. When constituted as specified, 5

ml of the resulting suspension contains 125 mg or 250 mg, 500 mg of cefadroxil activity

as the monohydrate, depending upon the concentration of the formulation.

After reconstitution, as recommended, cefadroxil for oral suspension is stable for 7 days

at controlled room temperature (15°-30°C) or 14 days when stored under refrigeration

(4°C) or 7 days at 25° C. Shake bottle well before using. Discard unused portion after 7

days at room temperature or 14 days under refrigeration.

التحذيرات واالحتياطات:

تفاعالت اإلفراط في الحساسية:

قبل بدء العالج باستخدام السيفادروكسيل، يجب التأكد بعناية إذا كان المريض يعاني مسبقا من حساسية

يجب أخذ عناية خاصة عندما يتم تناول هذا ، أو أدوية أخرى،أو البنيسيللين سيفالوسبورينللسيفادروكسيل، أو ال

باإلجراءات العالجية.

الحاالت المعدية المعوية والتهاب القولون.

تنخفض نسبة حدة اآلثار غير المرغوب فيها على الجهاز المعوي المعدي عندما يتم تناول الدواء مع الطعام، فهو

ال يؤثر على االمتصاص. ويجب وصف استخدام السيفادروكسيل بحذر في المرضى الذين لديهم تاريخ مسبق من

مرض معوي معدي السيما التهاب القولون.

عندما يتم عمل اختبارات باستخدام محلول بيندكت أو فيهلج، و ال يظهر هذا عندما يتم عمل إختبارات لإلنزيمات.

الالكتوز

تحتوي الكبسوالت الصلبة على الالكتوز. ونجد أن المرضى الذين يعانون من مشكالت وراثية نادرة لعدم تحمل

او نقص ايزومالتيز السكروز عليهم اللكتيز ، أو سوء امتصاص الجالكتوز الجلوكوزالجالكتوز، أو نقص الالب

عدم استخدام أو تناول هذا الدواء.

السكروز

يحتوي المعلق المعد من حبيبات المعلق عن طريق الفم على السكروز. ويجب على المرضى الذين يعانون من السكر

أن المرضى الذين يعانون من مشكالت وراثية نادرة لعدم تحمل الفركتوز، أن يضعوا هذا االعتبار بشكل أكيد. ونجد

سوء امتصاص الجالكتوز والجلوكوز أو عدم كفاية أيسومالتاز السكراز، عليهم عدم استخدام أو تناول هذا الدواء.

األمينوجليكوزيدات و مدرات البول الحلقية:

يجب متابعة وظائف الكلى جيدا عندما يتم استخدام سيفادروكسيل بالتزامن مع هذه األدوية، و التى من الممكن أن

أن هذا المركب ال يحتوى على الميثيل ثيو تريتازول و المعروف أنه المسؤل عن تقليل التجلط الذى تسببه

الكيفالوسبورينات.

ان اإلستخدام المتزامن للمضادات الحيوية المضادة للبكتيريا يمكن ان يعوق عمل البيتاالكتام.

الحمل و الرضاعة

ضرورية لذلك.يستخدم سيفادروكسيل أثناء الحمل اذا كانت هناك حاجة

الحمل.

مرة من الجرعة البشرية وكشفت 11 إلىتم القيام بدراسات التكاثر والتناسل في الفئران والجرذان في جرعات تصل
