Reactions 1252 - 16 May 2009 Public Citizen wants black box warning for lubiprostone The consumer advocacy group Public Citizen has filed a petition with the US FDA calling for a black box warning about the risk of abortion or preterm labour to be added to lubiprostone [Amitiza] label. The group says that lubiprostone, which is prescribed for chronic constipation in adults and irritable bowel syndrome in women, is in the same class as misoprostol, a drug widely used off label in medical abortions. Unlike lubiprostone label, the label for misoprostol carries a warning about the risk of abortion or preterm labour. Public Citizen contends that "studies submitted with lubiprostone’s original drug application showed that the drug caused a significant number of dose-related abortions in guinea pigs, the appropriate abortion model for humans". The group wants FDA to recommend a black box warning for lubiprostone, the strongest warning the FDA can issue. Also, the drug companies should provide patients with an FDA- approved medication guide, and the doctors with a warning letter, advising them of risk of abortion with lubiprostone, says Public Citizen. According to acting president of Public Citizen, Sidney Wolfe, "the current [lubiprostone] label is grossly inadequate as it lacks useful information pertaining to the risks of drug- induced abortion in pregnant women". Also, lubiprostone should be moved from the ‘C’ to ‘X’ pregnancy category of drugs, the same category as misoprostol, which defines drugs that should not be used during pregnancy, Wolfe said.
