• Heridas contaminadas o sucias no se considera profilaxis sino tratamiento ATB (11)

• Cefazolina de elección (5-8-11-13-16), si alergia clindamicina (8)

• Cefazolina 2g pacientes de más de 80 kg(11)

• Dosis inicial antes de 30- 60 minutos de incision (5-7-8-11-13-16-21)



• No dar ATB más de 24 hrs(5-11-13-16)

• No dar ATB despés de cerrar herida. Sin beneficio comprobado (7-8-21)

• Reforzar si tiempo operatorio excede la vida media en dos veces del ATB (7-11-13-16) o pérdida de sangre masiva (> 1500 cc) (11-21)

• Profilaxis ATB antes de realizar torniquete (11)



• Vía de administración endovenosa (7-21)

• Dosis única v/s multiples (4 a 5 días) son equivalentes(7)



•Mantener normotermia (36°C); normovolemia, considerar suplementación con O2 (8)

•Presencia de implantes o drenajes no justifica el uso de ATB en forma prolongada (8)



• Usar vancomicina sólo si Staphylococcus aureus meticilino resistente (una dosis solamente) (8)
