The purpose of our inspection is to determine if the mold remediation was successful in the area(s) noted. The post remediation inspection service included:

Service Conclusion Visual inspection of the constructed containment(s).

PASSED. Inspected containment(s) appeared in good condition. All seals appeared correct and intact.

Visual inspection of the remediated area(s) to ensure that all visible mold growth has been removed, that there are no wet building materials, and that the area(s) are clean and debris free.

PASSED. Inspected area(s) passed our visual inspection. Materials were dry and no mold growth was visible

Ambient air sampling from inside each containment or work area.

PASSED. The air samples collected did not detect, relative to the outside air, the presence of indoor mold growth in these areas at the time of sampling.

Direct sampling from surfaces that were previously cleaned/remediated of mold growth.

PASSED. The surface samples collected did not detect the presence of microbial growth.

DE2 Upstairs Hall Bathroom Containment (Surfaces)

PASSED. No mold growth on our collected sample

A detailed lab report of the air samples collected and photo documentation are included in this packet.

Based on the findings of the visual assessment, our understanding of the history of the property, data obtained and statements from the client and the remediation contractor, and concurrent laboratory analysis of samples collected, the remediation project of the above affected areas Passes Clearance Criteria. On this date, the remediation project is considered successful.

DISCLAIMERS

At this time, Mold Inspection Sciences, Inc. (MIS) has not been provided with written documentation from the owner, builder, or remediation contractor indicating that the underlying cause(s) of moisture have been corrected so that it is reasonably certain that mold will not return from the same cause. MIS makes no guarantee that such underlying causes of moisture and mold growth have been appropriately remedied. It is the responsibility of the client, owner, and/or contractor performing the work to stop or correct the underlying causes of moisture and to verify that such corrective actions have been adequately performed. MIS is not responsible for future problems related to incorrect repairs or undiscovered or new water intrusion issues.

MIS hereby notifies the client that continued moisture in any area will require ongoing future responses to control future mold growth. Possible sources include: plumbing leaks, vapor drive, condensation issues, water spillage, and air conditioning systems.

This Post Remediation Mold Assessment and Clearance Report is provided strictly for the specific area included in the scope of the Post Inspection and is not intended to include any other areas of the structure not included in the scope of this remediation project.

The findings of this Post Remediation Clearance Report are based on the condition of the area within the scope of the remediation and the visual assessment at the time and date of the Post Remediation Assessment.
