Periodic Health Assessment

• The Periodic Health Assessment is a yearly requirement of all Soldiers. To initiate your PHA, go to this link: http://mymedpros.army.mil

• Following the completion of the online portion, you must call the Reserve Health Readiness Program call center to make your appointment.

Behavioral Health Portion

• Read questions carefully• This question ask “in the last two weeks”• Is this a issue that is “situational” such as loss

of employment or death of a family member• Ensure that you tell the PHA provider that

your issues are “situational” if you feel they will resolve quickly.
