follow a thorough step-wise and evidence based approach to clinical practice.

Patient History PATIENT: Male, age 65

COMPLAINING OF (C/O):

Would like to replace fractured UR1-UR3 bridge and missing units UL1-UL4 and

UL6-UL7 as well as failing UL5 post-crown.

Patient also wished to have LR2 restored,LR1,LL1,LL2,LL3 crowns replaced due to

marginal recession, unsound/leaking margins and unsatisfactory aesthetics

HISTORY OF PRESENT COMPLAINT (HPC):

URQ Bridge fractured recently and decemented anteriorly. ULQ teeth extracted >6

months ago. Lower anterior crowns present >7+ years

EXTRACTION STATUS

Forcep and surgical extractions with no retained roots

PREVIOUS DENTAL HISTORY (PDH):

Regular attendance and history of routine dental treatment. Wears P/- C/C

MEDICAL HISTORY (MH):

ASA1, fit and well, no previous medical conditions and taking no prescribed

medications. No known drug allergies

SOCIAL HISTORY (SH):

Non-smoker and does not drink alcohol

STAGED COMFORMATIVE TREATMENT APPROACH:

i) Diagnostic wax-up’s of planned rehabilitation created on a semi-adjustable articulator with custom incisal guidance table.

ii) Replace UR1-UR3 three unit fixed prosthesis (porcelain fused to precious metal)

iii) Restore LR2; Replace LR1,LL1,LL2,LL3 crowns (porcelain fused to precious metal)

iv) Extract failing UL5 post-crowned, GI mobile tooth with poor prognosis

v) Implant rehabilitation ULQ with definitive plan to follow. Maintain sound UL8

Dr Adam Patel graduated from the University of Liverpool, following which he embarked on a multitude of post-graduate courses in restorative, prosthodontics and oral surgery. He holds a Master’s degree in Implantology awarded by The University of Manchester. Adam has also completed Diplomas from The Royal College of Surgeons of Edinburgh and The Royal College of Physicians and Surgeons Glasgow.

He accepts referrals for oral surgery, dental implants and full mouth reconstruction dentistry.
