The reason for writing to you is that I need information about your son/daughter to help with the organisation

of the ski trip. In order to assist in the hiring of boots and skis, insurance cover etc., please complete the form

on the back of this letter and return to me (not the Finance Office) as soon as possible and not later than Friday

11th May. I know this appears a little early to request this information as your child is probably still growing, but

it is to give the supplier an idea of what ski equipment will be required. In addition, I need the replies from Year

11 students before they go on study leave for their GCSE exams.

Every child must be in possession of a passport for the trip. Please ensure that it is up to date (i.e. expires after March 2019) and if it needs to be renewed that this is completed in plenty of time. If your child does not have a UK/EU Passport you will need to ascertain if any visas are required and apply for these. Every child will also need a European Health Insurance Card (EHIC). https://www.nhs.uk/NHSEngland/Healthcareabroad/EHIC/Pages/about-the-ehic.aspx gives information about the EHIC card and also provides a link to apply for one. There is no charge for an EHIC card. It is a condition of the insurance that every child is in possession of a card. It is your responsibility to ensure that your child is in possession of a passport and EHIC. Please apply in plenty of time to the relevant authority. Failure to comply will result in your child not being able to travel, so please act now!

If your child does not have a passport yet, or their passport will need to be renewed before the visit, please still

send the form in before the 11th May. Please provide me with the passport details when you have them.

I will write to you again later on in this academic year with a request for contact details and to complete a

medical form for the trip. An information evening will also be held in the autumn term to discuss the

arrangements for the trip. Dry slope training sessions will be held some time in December for those students

with no previous skiing experience.
