EDWARD ROMANO, DDS

The graduate of the Georgetown University School of Dentistry says that improved smiles are

often the catalyst to other positive changes. He’s seen a number of his patients gain a renewed sense of confidence, shed excess weight and become more active and upbeat.

Dr. Romano was at the forefront of cosmetic dentistry when it was first making inroads in the 1980s and has

been featured in numerous publications and television shows such as NBC’s Dateline for a story on the use of lasers. He’ s also a national speaker who travels coast to coast to prestigious events and learning institutions to lecture on topics related to aesthetic dentistry and methods for running an efficient office. He points out that cosmetic dental techniques have changed considerably since his early days. “In the 1980s, we had to be more aggressive, but now we can get incredible results with minor enamel smoothing of a tooth and super-thin veneers. I’m also not married to a particular product and choose what’s needed for each patient—sometimes that can vary even from tooth to tooth.”

Any such changes are accompanied by some sort of “compensation” such as a pass to the movies. Because he values his patients’ time, they do the same and call if they’re going to be any more than 15 minutes late (which can have an unpleasant trickle-down effect on later appointments if not managed properly).

Dr. Walter graduated from Fairleigh Dickinson University with a dental

degree and completed his residency at St. Joseph’s Regional Medical Center in Paterson where he gained considerable experience treating patients of all ages for general, traumatic and aesthetic issues. Dr. Merriman points out that time in her office is often the only time during the day her patients have to themselves. How White is Too White?

Anice, bright smile is onething, but the whiteningcraze may be getting atadoutofcontrol.Atfirst,it was just Hollywood

celebrities that were flaunting big rows ofblindingly white, ridiculously perfect teeth.But, as in the past, fads adopted by thestars tend to trickle down to the generalpublic.Byallaccounts,teethwhiteninghasbecomethemostpopular,mostrequestedcosmeticdentalprocedurearound.Demandhas driven the growth of self-proclaimedwhiteningspasandabarrageofadvertisingfor whitening products such as specialtoothpastes,stripsandgels, rinses,mouthguard-like tray systems and even chewinggum. Recently, MSNBC reported thatAmericans are now spending more than$1.4 billion on over-the-counter whiteningproducts every year in their quest for theHolyGrail:Whiterteeth.

The professional versus the DIYS-erWhilealloftheseOTCwhiteningproductswork in varying degrees because they allcontainsomeformofbleachingagent,in-office whitening is still the quickest andsafestwaytogetpearlywhites.Thedentistfactors in theconditionofyour teethandusesawhitenerincombinationwithheat,lightora laseralongwithaprotectivegelforthegumsandotherareasofthemouth.

If you go the pro-fessional route, thekey is communicationbecause everyone’sdefinition of “white”is different and youdon’t want to end upwith what many inthe profession havecoined “toilet-bowl”white teeth (not very flattering, I know) orbeingembarrassedtoopenyourmouthlikethecharacterRoss (DavidSchwimmer) fa-mouslywasinaclassicepisodeofFriendswhenhetragically(andhilariously)wayover-whitened.

Ifyouoptforself-whitening,proceed with caution. Thereare an abundance of horrorstoriesoutthereaboutpeoplewho become “bleachorexics”and do repeat applicationsor keep products on waybeyondwhatthemanufacturerrecommends. Or worse, fallasleep with whitening stripsonandwakeupinexcruciatingpain. In fact, a study by theJournaloftheAmericanDentalAssociation found that half ofthosewhobleachedtheirteethexperienced some form ofdiscomfort, from light tinglingand burning gums to shootingpainandsensitivitytosoundorair,althoughusuallythesesymptomsaretemporary.

Keep it realWhat’smore,whileagoodteethwhiteningcanprovideaninstantboostinconfidenceandself-image,itdoesn’tlastforeverandover-whitening can damage the enamelof your teeth causing them to becomeblueish or blue-gray. In more extremecases, the loss of the protective enamelcanevenleadtodecayandrootcanals.

The curator at the National Museumof Dentistry in Baltimore points out thatwhiteningisalsonothingnew.TheGreekswereputtingbleachingcompoundsontheir

teethatthebeginningof the Renaissance,but theydidn’t live aslong as we do todayand didn’t have toworry about keepingtheirteethforanextra50years.

So, here are fewpracticaltipsforthose

thinkingabouttakingthewhiteningdive:

• Go slowly, sotherearenoregrets.• Know your mouth. Crowns, fillings

and other dental work don’t whiten.Having super-white teeth surrounding

an off-white crown or patches ofbondingisn’tagreatlook.

• Compromise. Perhapsultra-white isnotthewaytogoandthere’samoreattainableandmoresustainableshadeofwhitethatwillalsomakeyouhappy.

• Change some of your habits.Quitsmoking, avoid staining foods andbeverages (or at least brush yourteeth immediately after indulging)and visit your dentist for a routinevisiteverysixmonths.

• Also,keepinmindthatmostofthoseperfect smiles on television are aresult of extensive dental work andfingernail-thin veneers being attachedtothefrontofteeth—notjustwhitening.And justsoyouknow, there’salsoanundercurrent of a new dental imagestyle coming down the pike. Sometrendsetters are realizing beauty isn’tflawlessperfectionandarenowstrivingfor the alluring perfect imperfectionsby forking over $1,600 to $2,500 pertoothformorenatural-looking,coutureveneerswithpurposefullyplacedstainsanddiscolorations(nojoke).

Bottom line: You may want to boost yoursmile with a touch of brighteners, but doyou reallywant teeth to glow in thedark?Didn’tthinkso.■

When I entered the profession in 1970, I realized that people’s

biggest objection to going to the dentist was fear of pain,” says Dr. Larry Kaplan. “Since then, delivering pain-free dental services has been my mission, and I teach others around the country techniques to make this happen.”

Part of Dr. Felsenstein’s appeal is his practice of looking his patients in the eyes

future, if you haven’t been

to a dentist recently or think

you may need a dental

procedure in the near future,

you should dismiss any and all

preconceived notions of what

may be involved. Your health is on the line.

Keeping up with all of this change can be a bit challenging

if you’re not a dentist yourself, and knowing which

treatments are right for your specific needs is not always
