If you’re one of those very smart people whopay regular attention to what I have to say (you’reunlike every single member of my family, in otherwords), you will have surely noticed that in my var-ious contributions to radio, TV, and medical articlesin print, especially in this publication, I commentpretty often about that “bone-thinning” conditionknown as osteoporosis or OP. The reason I do that is very simple: for me, os-

teoporosis is a very under-recognized – and verypreventable - cause of many deaths and a great deal of disability.The way I’ve always tried to make the connec-

tion between OP and premature death relevant toyou is to consistently remind you guys that amongaging folks, like you and me, OP is the most com-mon cause of a fracture of the hip, and sadly,roughly 20 % of women who suffer a hip fracturewill die within 6 months of ending up with thatfracture (the risk of dying from a hip fracture iseven higher in men, by the way, although overall,hip fractures are less common in men, in part be-cause so many of us guys just don’t live longenough to suffer an OP-related fracture).Interestingly, however, that morbid connection

of OP to premature death is even worse than thattraditional statistic that I just cited because accord-ing to a recent study, in women, the risk of dyingafter a hip fracture stays elevated, not just for thosecommonly-cited 6 months, but actually stays sig-nificantly higher for at least 10 years following the

fracture (the same long-term elevated death risk isprobably true in men as well, but alas, osteoporo-sis in men has simply not been studied nearly asmuch as it has in women).And if all that is not enough to convince some of

the pretty slow learners out there (like, for exam-ple, all those idiots who are waiting for a cata-strophic injury to occur to an NHL hockey playerbefore adding their voice to the growing publicconsensus that the NHL better do somethingabout the abominable number of serious head in-juries that happen in that league) that they shoulddo all they can, especially earlier in life, to preventOP, there’s also the matter of the major disabilitiesthat happen to many people as a consequence ofa fracture related to OP.Thus, for example, a very large percentage of

women (about half) who manage to survive abroken hip will forever be limited by that fractureby never again re-gaining their pre-fracture mo-bility and independence, meaning that a greatmany hip fracture victims are forever afterwardsconfined to a nursing home or to a wheel-chair(or both, of course, in many cases). But wait, there’s still more, because all that does-

n’t even address the many fractures of the spineand wrist that end up producing chronic and oftenhard-to-treat pain and limited movement in somany people who end up with OP.Not a pretty picture, eh, so one would naturally

This is what a pharmacist does best; helps tomanage your medications and avoid adverse reac-tions. It is thought that drug interactions are an im-portant cause of hospital emergency room visits.Some drug interactions are known, and sometimesthey can even be helpful for your therapy. Othersmight be detrimental to your health; it is all aboutcontext. How can you best protect yourself? Theanswer is easy: ask your Peoples Pharmacist. Yourlocal Peoples Pharmacist has a list of all of your pre-scriptions and can determine if any new medica-tions, vitamins, or herbal supplements will conflict.All you have to do is ask.

Perhaps the most common drug interaction con-cern over the counter (OTC) products that containsimilar ingredients. This is problematic with coldand flu products for children. It can happen withadult products as well. Let's suppose you choose amulti-symptom cough and cold product. Then youwalk across the aisle and pick up something foryour headache and fever. Did you know it is likelythat both of those products contained acetamino-phen? If both products were taken at maximumrecommended dosages, there could be enoughacetaminophen to harm you. The best way to avoidthis problem is to read the labels carefully; look foroverlapping ingredients. Or even better, ask yourPeoples Drug Mart Pharmacist if combining certainOTC products is safe.

Warfarin, warfarin, warfarin, every Pharmacistsfavourite drug for interactions. The water we drinkand the air we breathe can interfere with warfarin.

This might be an exaggeration, but the drug inter-action list for warfarin is quite long. Your physicianand Peoples Pharmacist will ensure that your pre-scriptions will not conflict with warfarin. But whatabout over the counter products? Be wary of OTCpain killers; some might contain acetylsalicylic acid(ASA), ibuprofen or naproxen. All of these productscan interfere with how warfarin works. The samecaution applies to the old stomach remedy Alka-Seltzer, it also contains acetylsalicylic acid (ASA). Itmight be a wise idea to ask your Peoples Pharma-cist if any OTC medications will interact with yourwarfarin before taking them. The same advice ap-plies to vitamin and herbal supplements.

Sometimes diet can interact with your warfarin.Foods rich in vitamin % can block warfarins bloodthinning action and potentially lead to complica-tions. Foods rich in vitamin % include: broccoli,brussels sprouts, kale, spinach and avocados. Thisdoes not mean that you have to fear a bowl of gua-camole, just keep things in moderation. If yourblood tests have been normal for a while, you canenjoy these foods sporadically. Green, leafy vegeta-bles are good for you. $ust don't go on some radi-cal avocado, dill pickle and watercress diet.

the amount of felodipine in the blood more thanexpected. Upon further analysis, they determinedit was actually the grapefruit juice, which was usedto mask the flavour of the alcohol, that caused thedrug increase. How grapefruit juice increased the amount of this

drug to enter your body is rather interesting. Thelining of your small intestine contains an enzymethat destroys many medications before they enteryour body. These enzymes are very useful becausethey also keep out foreign chemicals and pollutants.A component of grapefruit juice can block this andallow more than expected amounts of medicationto enter your body. While at first this might soundadvantageous, the results can be dangerous andunpredictable. The effects from a single glass ofgrapefruit juice can last longer than 24 hours. It isnot certain whether whole grapefruits have thesame effect, but it might be wise to avoid themalso. Which medications can be affected by grapefruit

juice? The list is quite extensive and might be a

teractions. The list of vitamin and herbal supple-ment interactions are too long for discussion in thisarticle. However, there is one that appears moreoften than any other. Calcium, and other minerals,can cause significant drug interactions. The mostcommon interactions occur with certain antibiotics.Another troublesome calcium interaction occurswith medications that help treat osteoporosis,mainly the bisphosphonates. Calcium can bind tothese drugs and prevent them from working. Whileit is important to take calcium if you have osteo-porosis, you must be mindful of your other med-ications. I recommend people do not take theircalcium supplements the evening before and themorning of their usual dose of bisphosphonates.Another overlooked common medication inter-

If you need another reason to get into shape: Phys-ical activity may reduce the risk of dementia-relateddeath, according to a recent study.

Researchers assessed the health of more than 45,000men and nearly 15,000 women, ages 20 to 88 years, inthe United States, and grouped them into one of threefitness categories -- low, middle or high.

After an average follow-up of 17 years, about 4,050participants died. Of those deaths, 164 were attributedto dementia (72 vascular dementia and 92 Alzheimer'sdisease). Of those 164 deaths, 123 of the people werein the low-fitness group, 23 were in the middle-fitnessgroup, and 18 were in the high-fitness group.

People in the high- and medium-fitness groups hadless than half the risk of dying as those in the low-fitness

group, the researchers concluded.The study appears in the journal Medicine & Science

in Sports & Exercise.Following the current physical-activity recommen-

dations from the American College of Sports Medicinewill keep most individuals out of the low-fit categoryand may reduce their risk of dying with dementia.

While deaths in the United States associated withheart disease, breast cancer and stroke have declined inrecent years, deaths related to dementia andAlzheimer's rose 46 percent between 2002 and 2006,according to the release.

The study doesn't prove that exercise will preventdementia, however, other factors may also come intoplay.

Calcium, themost abundant mineral in the body, is found insome foods and is available as a dietary sup-plement. 99% of the body's calcium supply isstored in the bones and teeth where it sup-ports their structure and function. Bone itselfundergoes continuous change, with constantresorption and deposition of calcium into newbone. The balance between bone resorptionand deposition changes with age. action occurs in the pain relief aisle. Most peoplewouldn't think twice about taking an anti-inflam-matory tablet for a headache or sore back. The classof drugs called NSAIDS (non-steroidal anti-inflam-matory drugs) can raise your blood pressure and af-fect blood thinning medications. Ibuprofen (Advil,Motrin), ASA (Aspirin) and naproxen (Aleve) are apart of the NSAID group of drugs. They can raiseblood pressure by temporarily reducing the effec-tiveness of the kidneys. For most people this is usu-ally not an issue, but for those with high bloodpressure the effects can be worrisome. NSAIDS canalso increase the effectiveness of blood thinningmedications and cause bleeding complications. Itmight be best to speak with your physician or Peo-ples Pharmacist before taking an NSAID. Acetamin-ophen (Tylenol) would be a safer choice for helpingto relieve headaches or other aches and pains.

Since we are in the aches and pain aisle, it mightbe a good idea to take another look at cough andcold medications. The most troublesome ingredi-ent in cough and cold medicines are the oral de-congestants. Look for the keywords NstuffyQ orNrunny noseQ on the packages. People with highblood pressure, diabetes, glaucoma, thyroid dis-ease, an enlarged prostate or taking certain antide-pressants should use decongestants with extreme

caution. Or better yet, find another option. Perhapsa non-drug saline nasal spray and a cough suppres-sant would work just a well. Ask your Peoples DrugMart Pharmacist if cough and cold medications areright for you.

Don’t Play A Guessing Game WithYour Daily Medications

Many people today are on more than one medication and when you com-bine this with a busy and active life, it can lead to the confusion of properlytaking your medications. Medication non-compliance is a major concern andaccounts for approximately 25% of all hospital admissions among seniors.

some of this stuff would then immediately deter-mine to do all they can to prevent OP and a sub-sequent fracture, although sadly, studies tell uspretty clearly that most people deal with OP onlyin the same manner that they deal with mostthings they don’t want to think about, that is, theyfall back on the age-old custom of ignoring theproblem until it’s finally unavoidable to confront,which for most people, alas, is when they suffertheir first fracture.So how do you prevent OP? Well, to start, it’s

important to realize that there are some risk factorsyou can do nothing about (it really is too late, forexample, to trade in your family for one in whichOP does not run genetically) but among the majorOP risk factors that you can control, these standout for me:

• don’t smoke• do lots of weight-bearing exercise such as

walking or jogging (hopefully starting assoon as you’ve finished reading this article,and maybe even while you’re reading it) butnot, please note, swimming or cycling (whichare not weight-bearing exercises)

• don’t drink too much alcohol • eat a good diet with lots of calcium-rich foods

(especially dairy products, if you can stom-ach them)

• don’t forget to also include regular healthydoses of vitamin D-rich foods (fish, for example)

• make sure to get enough sunshine exposureto keep your vitamin D levels up, especiallyduring the summer, and (one other majorrisk factor that is often over-looked)

• be as cautious as you can about the overuse,especially long-term, of certain drugs thathave been related to a higher risk of hipfracture, particularly those drugs used forgastric acid reflux known as proton pumpinhibitors or PPIs (Losec, Nexium, Pariet,Prevacid, et al), which is as good a time asany to offer my regular reminder that ifyou’re not completely sure what side effectsor drug interactions your particular medica-tions can produce, take your questions and

to address in this article (again, mostly because ofsome recent news that will affect a great many ofyou) is how to screen for OP, that is, which teststo do and how often to do them.People at high-risk for OP (for example, those

with a significant family history of this condition orsmall, thin women or middle-aged womenwho’ve already suffered a fracture or people whohave been taking cortisone treatment for a longtime, and a whole lot of others) need to get a firstscreening bone scan pretty early in life – probablyaround the age of 50.The rest of us, however, that is, those of us at

average-risk for OP, can wait till the age of 65 toget our first screening bone scan, and the newshere is that a recent study found that a womanwith a normal bone scan at the age of 65 cansafely wait at least another 15 years before havingto undergo yet another scan, which is good newsfor both those of us who hate being tested toomuch, as well as for those who are running ourmedical system because if this practice is widelyadopted, as I’m sure it will be soon, it will cutdown on hundreds of thousands of seeminglyneedless repeat bone scans.Finally, a word of advice that brings me back to

something I sort of alluded to at the top: if youwant to stay well, you really should pay attentionto health news, and if I may humbly say this, agreat way to do that is to stay current with whatI discuss in various media outlets, and to remindyou to do that on a regular basis. For most people, the realization of an approaching mid-point in life usually happens somewhere between age 40 and50; it can be the result of a major wake up call or just anawareness that NthingsQ just are not working quite like theyused to. A good example here is our brain; few of us reachthe middle age milestone without experiencing the dailyfrustration that accompanies the ability to remember names,phone numbers and the actual point of the story we so en-thusiastically began telling. Of course this leads to feararound the onset of early dementia, Alzheimer’s and Ngeneralscatterbrained syndrome;Q the latter being my personalcatch phrase for my own brain lapses when menopausebegan knocking on my door. Cognitive decline is a validconcern for many of us who want to age gracefully and a re-cent study published in the British Medical $ournal suggeststhat our brains do begin to lose their sharp edge after theage of 45. But not to worry, the study may say that 45 is aturning point, but there is still much we can do to keep ourgrey matter in optimal shape.

What was I thinking? Three years ago, after a very longlapse from academia and in the middle of the menopausaltransition, I made a decision to go back to school and beginthe journey toward a Bachelor of Social Work. The first yearwas the hardest as I had to relearn how to learn and I foundthat memorization took more effort than it used to in myyounger days. But developing good study habits has paidoff; I am now in the final months of my third year and canboast achieving honours standing and can actually feel theneurons firing along their new pathways. It is probably agood thing that my brain was under functioning when I ap-plied to the program because had I full capacity to actuallythink this decision through I may not have discovered howamazing the human brain actually is�

Use it or lose it� The brain is like a muscle, and it does riseto the challenge of a good workout. We come into thisworld hard wired to learn. The brain still has the ability to laydown new pathways for knowledge in old age, it also has abuilt in mechanism to prune out pathways that are no longer

travelled. While serious cognitive impairment may be a re-sult of disease, illness or accident, much age related cogni-tive decline is due to a lack of mental stimulation.Understandably, not everyone will want to choose the pathof higher education for stimulation; try incorporating someof these easy and fun ways to get a good brain workout intoyour daily routine:1. Switch it up. Remember how hard you had to concen-trate when you were learning how to tie your shoelaces? Tryother activities like brushing your hair or teeth with your non-dominant hand; over time, the neural pathways will becomestronger and it will not seem as awkward anymore.2. Turn off the TV and engage the brain in an interestingbook, game, new hobby or craft. Word puzzles and numberpuzzles top the list of brain exercises and the local newspa-per and the Internet are both good sources to find similarbrain challenges. 3. Take a class and try something new and completely differ-ent. It doesn’t matter if you choose to learn a new musicalinstrument, explore yoga, ballroom dancing or oil painting- the important thing is that the brain has to learn new bodymind connections. 4. Move your body. Even gentle exercise increases circula-tion and ensures the brain is getting plenty of oxygen andglucose. Be sure to drink your water and eat a healthy dietas well. Studies have correlated regular daily exercise to alower risk of developing cognitive decline including demen-tia and Alzheimer’s.5. Strengthen your memory by playing games that test it.You can also practice memorizing grocery lists, word lists andnew phone numbers.

Remember too that we are social creatures; maintainingfriendships and being present with our loved ones stimulates thebrain as we engage in conversation and exchange of ideas. Theloneliness factor is also eliminated which contributes to a brighteroutlook and overall sense of wellness. Personally I will take a pageout of Gandhi’s book. NLive as if you were to die tomorrow. Recipes For Good Health

Preparing your own sweet-and-sour sauce re-

sults in lower fat and fewer calories than using

store-bought sauce. You can make this recipe

gluten-free by using gluten-free soy sauce.

Cooked long-grain brown rice 1 cup 250 mL

(about 1⁄4 cup, 60 mL, uncooked)

Finely chopped canned 1⁄4 cup 60 mL

water chestnuts

Grated onion1⁄4 cup 60 mL

Salt1⁄4 tsp. 1 mL

Pepper1⁄2 tsp. 2 mL

Lean ground beef3⁄4 lb. 340 g

Can of crushed pineapple 14 oz. 398 mL

(with juice)Apricot jam

1⁄4 cup 60 mL

Rice vinegar 2 tbsp. 30 mL

Soy sauce2 tbsp. 30 mL

Cornstarch 1 tbsp. 15 mL

Finely grated ginger root 2 tsp 10 mL

(or 1⁄2 tsp., 2 mL, ground ginger)

Combine first 5 ingredients in large bowl. Add beef. Mix

well. Shape into 1 inch (2.5 cm) balls. Arrange in single layer

on greased baking sheet with sides. Cook in 400ºF (205ºC)

oven for about 15 minutes until no longer pink inside. Makes

about 31 meatballs.

Combine remaining 6 ingredients in large saucepan. Bring to

a boil. Reduce heat to medium-low. Boil gently, uncovered,

for about 5 minutes, stirring often, until thickened. Add

meatballs. Stir gently until coated. Makes about 4 cups (1 L).

BEFORE: 1/2 cup (125 mL): 333 Calories; 22 g Total Fat

(8 g Sat); 388 mg Sodium

AFTER: 1/2 cup (125 mL): 187 Calories; 6 g Total Fat (2.5

g Mono, 0 g Poly, 2 g Sat); 25 mg Cholesterol; 20 g Car-

bohydrate; 1 g Fibre; 10 g Protein; 340 mg Sodium
