*11:30-12:15: There will be class for artistic appreciation (Gu Zheng, ErHu, Bamboo Flute, Flute and etc.) on every Tuesday. **Instrument refers to recorder. Kids shall bring their own recorder to class.

Brief introductions about classes: Chinese***: Learning the language, including both written and oral practices on vocabularies, sentences; theme-oriented Chinese culture and custom introductions.

Creative Math***: Focusing on cultivation of sense of numbers, with daily practices of basic calculations (based on ages: plus, minus, times, dividends) with fun and inter-active activities;

Dance / Kung Fu: Dance will include both ballet and Chinese dance exercises; while Kung Fu will teach some basic movements (though they could be suitable for all kids, we offer them especially for boys as an option instead of dance).

Theater: Suppose to be fun and challenging. Kid will participate in role play activities to learn and practice rhymes, tongue-twisters, poems, ballads, and etc.

Chinese vocal: Will teach traditional American kids songs with lyrics translated into Chinese.

Calligraphy / Chinese painting / color: Will teach traditional Chinese painting and watercolor painting and the use of Chinese paper and brush. Final art works will be framed at the end of the camp by kids themselves.

Instrument: Students will be taught about the basic techniques of playing recorder.

Artistic appreciation: Meet the artists! Our master instructors and teachers will introduce and perform in person various musical instruments to kids.

By signing below, I understand this contract is between the Mandarin Chinese Language Immersion Magnet School for the registration I submit for the Summer Camp days/hours of Monday-Thursday 7:15 AM - 2:00 PM from June 9-26 and that I agree to the contents of this registration packet and agree to the following for the Summer Camp 2014. Disregard of this contract and/or contents in the registration packet will result in immediate dismissal from the camp. 1. I understand that I am enrolling only for Summer 2014 ONLY. 2. CLASS TIMES: I understand that I may register for one week at a time. 3. PAYMENTS: (Plea se i ni tia l ea ch i tem belo w o n th e l ine) _____Payments MUST BE SUBMITTED BY CHECK OR MONEY ORDER ONLY made out to MCLIMS. Credit Card and Cash is not accepted. _____The entire payment for each week I sign up for is due upon registration. _____I will be assessed a one time registration fee of $15.00 if I submit my registration by April 11 and a one time registration fee of $25.00 if I submit my registration after April 11. _____I will be assessed a $35.00 check return fee for each returned check and all future payments will be accepted only via Money Order. I also understand that I must pay the returned check in the form of 2 money orders: 1st money order to replace the returned check and a 2nd Money Order for the check return fee. 4. Cancellation Policy: There is no proration and no refunds for any week attended. 5. Minimum Enrollment: There must be at least 140 registered students for the camp to be held. 6. SIGNING CHILDREN OUT: ONLY individuals listed on the Authorization Form will be allowed to pick up your

children. Failure to sign the student out from the school will be regarded as an “Incident” and may result in suspension from the program. ONLY adults 18 years or over are authorized to pick up children. SHOWING PHOTO ID IS MANDATORY UPON REQUEST.

7. LATE PICK UP: • If my child is picked up after class time ends, I understand I will be assessed a late pickup fee of $2.00/minute. • After the 3rd late pick-up I understand my registration may be cancelled and I will not be refunded. • The official time for our program will be gauged by the clock located in Pick Up Location in the school. • Please Note: If late pick-up fees are not paid at the time of pick up and no later than the following morning at the time of drop off, your registration will be cancelled and will not be refunded. (All pick up fees must be paid by Check or Money Order only)

8. HOW A STUDENT GETS HOME: Students will not be allowed to leave the campus without the supervision of an Authorized Adult listed on the form. Children will not be allowed to walk home unsupervised.

9. BEHAVIOR EXPECTATIONS: In order to ensure the safety of all students note: Services will be suspended should disrespectful or unsafe behavior or conduct be directed towards the school premises, a student, a staff member, either by a parent of a student or student in enrolled in the Summer Camp. A Zero Tolerance Policy will be enforced.

10. SPECIAL NEEDS: We do not discriminate on the basis of gender, race, religion, or national origin. The Staff is experienced with students from different backgrounds. However, they are not specially trained to address the needs of students who require special behaviorial or medical attention. It is the responsibility of the parent/guardian/person submitting this registration to disclose information regarding any special needs your child may require to the Program Administrator of OAEC.

11. All information published in this registration packet applies to this contract. In addition, I understand and agree to adhere to any guidelines amended to this contract and that I will be notified in writing by the school officials.

12. I understand that my email address and/or phone number(s) may be given to the Summer Camp Curriculum Director, OAEC, should it be necessary to contact me.
